Özet

Giriş: Obezite tüm dünyada insidansı giderek artan, önlenebilir ölümlerin en önemli sebeplerinde biri olarak ciddiyetini koruyan bir sağlık problemidir. Bu probleme yönelik tedavi yöntemleri her gün gelişmekte, hastalığa ve tedavilere özgü mekanizmalar her geçen gün daha iyi anlaşılmaktadır. Tedavi seçenekleri arasında önemli bir paya sahip olan bariatrik cerrahi (metabolik cerrahi, obezite cerrahisi) prosedürlerinin etkilerini bilmek ve olası sonuçlarını tahmin edebilmek, sürecin başarı ile tamamlanabilmesinde önemlidir. Bu amaçla insan hayatı için önemli role sahip olan 'Vitamin B12 ' ve 'Folik Asitin', 'Homosistein' gibi kardiyovasküler hastalıklar açısından önemli bir yere sahip biyobelirteç ile ilişkini incelemek istedik. Gereç ve Yöntem: Bu çalışmaya, Gazi Üniversitesi Genel Cerrahi Anabilim Dalı'nda Eylül 2019 – Eylül 2020 yılında obezite nedeni ile bariatrik cerrahi uygulanmış 126 hasta dahil edildi. Hastaların demografik özelliklerinin yanı sıra, preoperatif, postoperatif 1.ay, 6.ay, 12.ay takiplerde görülmüş olan kilo, BKİ, Vitamin B12, Folat, HbA1c, LDL Kolestrol, HDL Kolestrol, VLDL Kolestrol, Total Kolestrol, Trigliserid, Ferritin, Serum Demir, Albümin düzeyleri incelendi. Bulgular: Belirlenen takip noktalarında istenilen biyokimyasal parametrelere sahip hastalar belirlenerek Roux-en-Y Gastrik Bypass (RYGB) (n=43) ve Sleeve Gastrektomi (SG) (n=7) olan hastalar olarak iki grup oluşturuldu. Belirlenen biyokimyasal parametreler incelendiğinde, homosistein, HbA1c, , HDL Kolesterol, VLDL Kolesterol, Total Kolesterol, Trigliserid, Ferritin, Serum Demir seviyelerindeki zaman içindeki değişimi istatiksel olarak anlamlı idi (p<0,001). Folat (p:0,064) ve Albüminin (p:0,257) zaman içindeki değişimi ise anlamsızdı. Vitamin B12 ve LDL kolestrol zaman içindeki değişimi SG grubunda anlamsız iken (p:0,409 ve p: 0,486) RYGB grubunda anlamlı idi (p<0,01). Homosisteinin zaman içindeki değişimini etkileyen faktörleri bulabilmek için marjinal model kurulmuştur. Vitamin B12, ve Folatın homosistein ile ters yönlü ilişkisi bu model ile gösterilmiştir (p<0,05). Sonuçlar: Kardiyovasküler hastalıklar için önemli bir biyobelirteç olarak görülen Homosistein, bariatrik cerrahi sonrası hastalarda anlamlı seviyelere yükselebilir. Hastalarda belirgin vitamin eksikliği düzeyi olmasa bile değişen kan seviyeleri hiperhomosisteinemi gelişmesi durumuna katkı sağlayabilir. Bariatrik cerrahi olan hastalarda gelişen komplikasyonların daha iyi anlaşılabilmesi, olası komplikasyonların önlenebilmesi ve sürecin daha iyi yönetilebilmesi adına Vitamin B12, Folat ve homosistein ilişkisinin önemli olduğunu belirtmek istiyoruz.

Introduction: Obesity is a health problem whose incidence is increasing all over the world and remains serious as one of the most important causes of preventable deaths. Treatment methods for this problem are developing every day, and mechanisms specific to the disease and treatments are better understood day by day. Knowing the effects of bariatric surgery (metabolic surgery, bariatric surgery) procedures, which have an important share among the treatment options, and being able to predict the possible results are important for the successful completion of the process. For this purpose, we wanted to examine the relationship between 'Vitamin B12' and 'Folic Acid', 'Homocysteine', which have an important role in human life, and biomarkers that have an important place in terms of cardiovascular diseases. Materials and Methods: 126 patients who underwent bariatric surgery for obesity in September 2019 – September 2020 at Gazi University, Department of General Surgery were included in this study. In addition to the demographic characteristics of the patients, weight, BMI, Vitamin B12, Folate, HbA1c, LDL Cholesterol, HDL Cholesterol, VLDL Cholesterol, Total Cholesterol, Triglyceride, Ferritin, Serum Iron, Albumin levels were examined. Results: Patients with the desired biochemical parameters were determined at the determined follow-up points, and two groups were formed as patients with Roux-en-Y Gastric Bypass (RYGB) (n=43) and Sleeve Gastrectomy (SG) (n=7). When the determined biochemical parameters were examined, the changes in homocysteine, HbA1c, HDL Cholesterol, VLDL Cholesterol, Total Cholesterol, Triglyceride, Ferritin, and Serum Iron levels over time were statistically significant (p<0.001). The change in Folate (p:0.064) and Albumin (p:0.257) over time was insignificant. While the changes in vitamin B12 and LDL cholesterol over time were insignificant in the SG group (p:0.409 and p:0.486), they were significant in the RYGB group (p<0.01). The marginal model was established to find the factors affecting the variation of homocysteine over time. The inverse relationship of Vitamin B12, and Folate with homocysteine was demonstrated with this model (p<0.05). Conclusion: Homocysteine, which is seen as an important biomarker for cardiovascular diseases, may increase to significant levels in patients after bariatric surgery. Even if patients do not have a significant level of vitamin deficiency, varying blood levels may contribute to the development of hyperhomocysteinemia. We would like to state that the relationship between Vitamin B12, Folate and homocysteine is important in order to better understand the complications that develop in patients undergoing bariatric surgery, to prevent possible complications and to better manage the process.
