Özet
Amaç: Bu çalışmanın amacı, inme sonrası disfaji rehabilitasyonunda egzersiz tedavisi ile birlikte uygulanan yüzeyel nöromusküler elektriksel stimülasyon (NMES) tedavisinin hastaların yutma, beslenme ve fonksiyonel durumları üzerine olan etkisini incelemektir. Gereç ve Yöntem: Retrospektif vaka kontrollü çalışmada 1 Mart 2019 tarihi ile 1 Eylül 2019 tarihleri arasında Sağlık Bilimleri Üniversitesi İstanbul Fizik Tedavi Rehabilitasyon Sağlık Uygulama ve Araştırma Merkezi'nde yatarak inme rehabilitasyonu görmüş, Standardize Mini Mental Test (SMMT) skoru ≥ 24 ve Yatak Başı Su Yutma Testi sonucu disfajik olan hastalar çalışmaya dahil edildi. Yalnızca yutma egzersiz tedavisi alan ve egzersiz tedavisiyle birlikte NMES tedavisi alan toplam 26 hasta iki gruba ayrıldı. Hasta verileri taranarak Fonksiyonel Oral Alım Sklası (FOAS), Yatak Başı Su Yutma Testi, Yutma Değerlendirme Ölçeği (EAT-10), Fonksiyonel Ambulasyon Skalası (FAS), Brunnstrom evrelemesi, Barthel İndeksi, Vücut Kitle İndeksi (VKİ), Mini Beslenme Testi (MNA) sonuçları tedavi öncesi ve sonrası karşılaştırıldı. Bulgular: Yatak başı su yutma testi, FAS, EAT-10, Barthel İndeksi, MNA toplam ve değerlendirme sonuçları gruplar kendi içlerinde değerlendirildiğinde tedavi sonrası, tedavi öncesine göre anlamlı farklılık gözlenmiş olup, gruplar arasında farklılık görülmemiştir. FOAS skorunda sadece egzersiz grubunda tedavi sonrası anlamlı fark saptanmıştır. NMES grubunda Brunnstrom üst ekstremite, egzersiz grubunda Brunnstrom el değerlendirmesinde anlamlı değişim gözlenmiştir. VKİ ve Malnutrisyon Gösterge Puanında her iki grupta da tedavi sonrası anlamlı değişim olmamıştır. Sonuç: İnme sonrası disfaji rehabilitasyonunda hem yutma egzersizleri hem de yutma egzersiziyle birlikte uygulanan NMES tedavisinin yutma, beslenme ve fonksiyonel durum üzerine olumlu etkisi olduğu saptanmıştır.

Aim: The aim of this study is to examine the effect of superficial neuromuscular electrical stimulation (NMES) therapy applied together with exercise therapy on the swallowing, nutrition and functional status of patients in post-stroke dysphagia rehabilitation. Materials and Methods: In the retrospective case-controlled study, patients who underwent stroke rehabilitation (having Standardized Mini Mental Test (SMMT) score ≥ 24 and Bedside Water Swallowing Test result in dysphagia) at Istanbul Physical Medicine and Rehabilitation Health Application and Research Center of the Health Sciences University between March 1, 2019 and September 1, 2019 were included. A total of 26 patients who received swallowing exercise therapy alone and NMES therapy together with exercise therapy were divided into two groups. By scanning patient data, Functional Oral Intake Scale (FOIS), Bedside Water Swallowing Test, Eating Assessment Tool (EAT-10), Functional Ambulation Scale (FAS), Brunnstrom Stages, Barthel Index, Body Mass Index (BMI), Mini Nutritional Assessment Test (MNA) results were compared before and after treatment. Results: When the bedside water swallow test, FAS, EAT-10, Barthel Index, MNA total and evaluation results were evaluated within the groups, a significant difference was observed after the treatment compared to before the treatment, and no difference was observed between the groups. There was a significant difference in the FOIS score only in the exercise group after treatment. A significant change was observed in Brunnstrom upper extremity assessment in the NMES group and Brunnstrom hand assessment in the exercise group. There was no significant change in BMI and Malnutrition Indicator Score after treatment in both groups. Conclusion: In post-stroke dysphagia rehabilitation, both swallowing exercises and NMES treatment applied together with swallowing exercise have a positive effect on swallowing, nutrition and functional status.
