Özet
Amaç: Üroteliyal karsinom, dünyada en sık görülen kanserler arasındadır. Tanı anında çoğunluğu metastatik evrede olmamasına karşın takipte büyük oranda metastatik evreye progresyon gösterebilmektedir. Tedavi seçenekleri arasına kontrol noktası inhibitörü immünoterapötiklerin eklenmesiyle sağkalım süreleri uzamaktadır. Periferik kandan değerlendirilen nötrofil/lenfosit oranı (NLO); tümör mikroçevresinin prokanseröz ve anti-tümör özelliklerini yansıtmaları açısından hem prognostik hem de immünoterapötiklerin yanıt oranları üzerinde etkili olabilecek bir inflamatuar biyobelirteçtir. Bu çalışmada üroteliyal karsinom tanısı almış hastaların tanı anı, sistektomi öncesi ve sonrası ile nüks dönemi nötrofil/lenfosit oranlarının değerlendirilerek bu parametrelerin hastalığın nüksü üzerine prognostik ve prediktif etkisinin araştırılması amaçlanmıştır. Materyal ve Metod: Üroteliyal karsinom tanısı alarak sistektomi ameliyatı uygulanmış 85 hastanın tanı anı, sistektomi öncesi, sistektomi sonrası ve nüksün tespit edildiği andaki periferik kandaki mutlak nötrofil sayısı ve mutlak lenfosit sayısı tespit edilerek oranı elde edilmiştir. Hastaların verileri hastane kayıtları ve hasta dosyaları üzerinden retrospektif olarak edinilmiştir. Bulgular: Hastaların sistektomi sonrası nüks edene kadar geçirdikleri hastalıksız sağkalım (disease-free survival) süreleri ile NLO arasındaki ilişki incelendiğinde tanı dönemi, sistektomi öncesi ve sonrası NLO değerleri ile anlamlı ilişki saptanmadı. Nüks olan hastaların nüks olduğu dönemdeki NLO değerleri ile nüks olmayan hastaların sistektomi sonrası hastalıksız takip dönemindeki NLO değerleri karşılaştırıldığında NLO artışıyla nüks gelişmesi (p<0,001) ve sistektomi sonrası hastalıksız sağkalım (p=0,01) arasında anlamlı ilişki saptandı. Preoperatif ve postoperatif karşılaştırmalarında anlamlı ilişki saptanmadı. Sonuç: NLO'nun üroteliyal karsinom tanısı ile sistektomi uygulanmış hastalarda nüksü ve hastalıksız sağkalım süresini kısalttığı predikte edilmiştir. İmmunoteröpatiklerin üreoteliyal karsinom tedavisinde kullanımının artmasıyla, NLO'nun prognostik bir biyobelirteç olarak klinik uygulamada yer alması için prospektif multimerkezli çalışmalara ihtiyaç vardır.

Aim: Urothelial carcinoma is among the most common cancers in the world. Although most of them not in the metastatic stage at the time of diagnosis, they can progress to the metastatic stage to a large extent during follow-up.With the addition of checkpoint inhibitör immunotherapeutics among the treatment options, survival times are prolonged. Neutrophil/lymphocyte ratio (NLR) evaluated from peripheral blood; it is an inflammatory biomarker that can affect both prognostic and immunotherapeutic response rates in terms of reflecting the procancerous and anti-tumor properties of the tumor microenvironment. In this study, it was aimed to evaluate the prognostic and predictive effects of these parameters on the recurrence of the disease by evaluating the neutrophil/lymphocyte ratios at the time of diagnosis, before and after cystectomy, and during the relapse period of patients diagnosed with urothelial carcinoma. Material and Method: Absolute neutrophil count and absolute lymphocyte count were determined in the peripheral blood at the time of diagnosis, before cystectomy, after cystectomy, and at the time of recurrence, of 85 patients who were diagnosed with urothelial carcinoma and underwent cystectomy. The data of the patients were obtained retrospectively from hospital records and patient files. Results: When the relationship between the patients' disease-free survival (disease-free survival) until recurrence after cystectomy and NLR was examined, no significant correlation was found with the diagnosis period, pre- and post-cystectomy NLR values. When the NLR values of the patients with relapse were compared with the NLR values of the patients who did not relapse during the disease-free follow-up period after cystectomy, a significant correlation was found between the increase in NLR and the development of relapse (p<0.001) and disease-free survival after cystectomy (p=0.01). No significant relationship was found in the preoperative and postoperative comparisons. Conclusion: It has been predicted that NLR shortens recurrence and diseasefree survival in patients who underwent cystectomy with the diagnosis of urothelial X carcinoma. Prospective multicenter studies are needed to include NLR as a prognostic biomarker in clinical practice, with the increasing use of immunotherapeutics in the treatment of ureothelial carcinoma.
