Özet
Giriş: Akne vulgaris pilosebase ünitenin kronik inflamatuvar bir deri hastalığıdır. Hormonların, aknenin hem patogenezi hem de tedavisinde önemli rol oynadığı düşünülmektedir. Akne tedavisinde kullanılan sistemik isotretinoin akne patogenezinde rol oynayan bazı hormon seviyelerinde değişikliğe yol açtığı gösterilmiştir. Ancak buna dair literatürde çelişkili sonuçlar veren az sayıda araştırma bulunmaktadır. Amaç ve Yöntem: Bu çalışmanın amacı isotretinoin tedavisinin adrenal ve seks steroid hormon düzeyleri etkisini araştırmaktır. 39 orta şiddetli ve şiddetli akne vulgaris hastasının tedavi öncesi ve tedavinin 3. ayında, 39 sağlıklı gönüllünün bir kez adrenal ve seks steroid hormon düzeylerine LCMS/MS ve ECLIA yöntemi ile bakıldı. Ayrıca hastalar hirsutizim ve menstrüasyon düzensizlik açısından her vizitte sorgulandı. Bulgular: Hastaların tedavi öncesindeki androjen hormonları, 17-OH- Pregnenolon, 11-Deoksikortizol, Progesteron ve 11-Deoksikortikosteron düzeyleri kontrol grubuna göre istatistiksel olarak daha yüksekti. Tedavinin 3. ayındaki Kortizol, Progesteron, Pregnenolon, 11-Deoksikortikosteron düzeyleri , tedavi öncesine göre istatistiksel açıdan anlamlı olarak yüksekti (p<0.001). Vakaların %35'inde takipler sırasında menstrüasyon düzensizliği tespit edildi. Hasta grubunda takiplerin 3. ayında menstrüasyon düzensizliği, tedavi öncesine göre istatistiksel olarak anlamlı oranda yüksekti (p=0.001). Sonuç: Adrenal ve seks steroid hormonları akne vulgaris patogenezinde rol oynamaktadır. Ayrıca isotretinoin tedavisiyle birlikte adrenal ve seks steroid hormonları değişim göstermektedir.

Background: Acne vulgaris is a chronic inflammatory skin disease primarily affecting the pilosebaceous unit. Hormones play an important role in both the pathogenesis and treatment of acne, Systemic isotretinoin is one of the most effective treatments for acne, recently there has been interest on the potential role of isotretinoin on hormone levels playing role in acne pathogenesis, even though there are few studies with contradictory results addressing this issue. Nevertheless, There are no extensive and comprehensive studies on this topic yet. Objectives and Methods: The aim of this study is to investigate the effects of isotretinoin treatment on the levels of adrenal and sex steroids. Hormone levels were analyzed in 39 patients with moderate to severe acne vulgaris before treatment and at the 3rd month of treatment, as well as in 39 healthy volunteers, using steroid LCMS/MS and ECLIA methods. Results: The Cortisol, Progesterone, Pregnenolone, and 11-Deoxycorticosterone levels of the patients in the 3rd month of treatment were statistically significantly higher compared to first-admission levels (p<0.001). Androgen hormones, 17-OH-Pregnenolone, 11-Deoxycortisol, Progesterone, and Deoxycorticosterone levels at the first admission in the patients' group were statistically higher than the control group. 35% of cases in the 3rd month had menstrual irregularities. Menstrual irregularity in the patient group at the 3rd month of follow-up was statistically significantly higher compared to before treatment (p<0.001). Conclusions: Adrenal and sex steroid hormones play a significant role in both the pathogenesis of acne vulgaris and the efficacy of isotretinoin.
