Özet
Amaç: Diyabet, sürekli tıbbi bakım ve hasta eğitimi gerektiren; akut komplikasyonları önlemek ve uzun vadeli komplikasyon riskini azaltmak için destek verilmesi gereken kronik bir hastalıktır. Diyabet yönetimi yüksek sağlık okuryazarlığı (SOY) becerileri gerektirir. Düşük SOY, bireyin tedaviye uyumunu ve dolayısıyla hastalığın yönetimini negatif yönde etkileyen önemli bir faktördür. Çalışmamızda tip 2 diabetes mellitus (DM) tanılı hastalarda ilaca uyumun SOY ile ilişkisinin değerlendirilmesi ve ilaca uyumun arttırılmasına yönelik stratejilerin geliştirilmesine katkıda bulunulması amaçlanmıştır. Gereç ve Yöntem: Çalışmamıza 01.06-01.08.2023 tarihleri arasında Haydarpaşa Numune Eğitim ve Araştırma Hastanesi Aile Hekimliği Poliklinikleri'ne başvuran 18-65 yaş arası tip 2 DM tanılı 220 hasta dahil edildi. Veriler Katılımcı Bilgi Toplama Formu, Sağlık Okuryazarlığı Ölçeği (SOYÖ) ve Morisky Tedavi Uyum Ölçeği (MTUÖ-8)'nin yer aldığı anket formu kullanılarak yüz yüze toplandı. Katılımcı Bilgi Toplama Formu'nda; yaş, cinsiyet, medeni durum, boy, kilo, vücut kitle indeksi (VKİ), eğitim düzeyi, HbA1c düzeyi, gelir düzeyi, meslek, ailede diyabet varlığı, diyabet tanı süresi, ek kronik hastalık varlığı, diyabet eğitimi, düzenli ilaç kullanımı, diyabet tedavisi, son 1 yılda diyabet takibi için sağlık kuruluşu başvurusu, düzenli takipli olunan sağlık kuruluşu varlığı, toplam kullanılan ilaç sayısı, sigara ve alkol alışkanlıkları, diyabete bağlı komplikasyon varlığı sorgulandı. Kendi sağlık durumlarını 0-10 puan aralığında değerlendirmeleri istendi. Hastalar SOYÖ ve MTUÖ-8'deki sorularla değerlendirildi. Veriler SPSS Statistics 21.0 programında değerlendirildi. İstatistiksel olarak p<0.05 anlamlı kabul edildi. Bulgular: Çalışmamızda hastaların diyabet SOYÖ ortalama toplam puanının orta düzeyde olduğu (35,42±10.05), MTUÖ-8 puanlarına göre bireylerin %60.2'sinin tedaviye düşük uyum gösterdiği saptandı. Çalışmamızda eğitim düzeyine göre üniversite mezunu olanların ilkokul, ortaokul ve lise mezunu olanlara göre, VKİ normal olanların preobez ve obez olanlara göre, geliri giderinden fazla olanların geliri giderine denk ve az olanlara göre, memur olanların ev hanımı, işçi, emekli, serbest meslek ve diğer gruba göre SOYÖ ve MTUÖ-8 puanları daha yüksek bulundu. SOY düzeyleri ve tedaviye uyumları açısından istatistiksel olarak anlamlı fark saptandı (p:<0.001, p:<0.001, p:<0.001, p:0.004, p:<0.001, p:<0.001, p:<0.001, p:<0.001). Çalışmamızda diyabet eğitimi alanların almayanlara göre, OAD ve YTD tedavisi alanların sadece YTD ve YTD+OAD+İnsülin tedavisi alanlara göre, düzenli ilaç kullanımı olanların olmayanlara göre, son 1 yılda diyabet için sağlık kuruluşu başvurusu olanların olmayanlara ve düzenli takipli olduğu sağlık kuruluşu olanların olmayanlara göre ve komplikasyon gelişmeyen bireylerin gelişenlere göre SOYÖ ve MTUÖ-8 puanları daha yüksek bulundu. SOY düzeyleri ve tedaviye uyumları açısından istatistiksel olarak anlamlı fark saptandı (p:<0.001, p:0.001, p:0.003 p:0.020, p:<0.001, p:<0.001, p:<0.001, p:<0.001, p:<0.001, p:<0.001, p:0.007, p:0.046). Cinsiyete göre SOY düzeyleri ve tedaviye uyumları açısından istatistiksel olarak anlamlı fark saptanmadı (p=0,121, p=0,118). Medeni duruma göre SOY düzeyleri ve tedaviye uyumları açısından istatistiksel olarak anlamlı fark saptanmadı (p=0,708, p=0,107). Ailesinde diyabet hastası olanların olmayanlara göre SOY düzeyleri ve tedaviye uyumları açısından istatistiksel olarak anlamlı fark saptanmadı (p=0.068, p=0,599). DM tanı süresi kısaldıkça SOY düzeyleri açısından istatistiksel olarak anlamlı fark saptanırken (p<0,001) tedaviye uyum açısından anlamlı fark saptanmadı (p=0,892). Ek kronik hastalığı olanların olmayanlara göre SOY düzeyi ve tedaviye uyumları açısından istatistiksel olarak anlamlı fark saptanmadı (p=0,100, p=0,416). Günde toplam 1-4 adet ilaç kullananların hiç ilaç kullanmayan ve 5 ve üzeri ilaç kullananlara göre SOY düzeyleri açısından istatistiksel olarak anlamlı fark saptanırken (p=0,009) tedaviye uyumları açısından anlamlı fark saptanmadı (p=0,673). Sigara kullananların kullanmayanlara göre SOY düzeyleri ve tedaviye uyumları açısından istatistiksel olarak anlamlı fark saptanmadı (p=0,080, p=0,660). Alkol kullananların kullanmayanlara göre SOY düzeyleri açısından istatistiksel olarak anlamlı fark saptanırken (p=0,032) tedaviye uyumları açısından anlamlı fark saptanmadı (p=0,235). Yaş arttıkça SOYÖ puanı azalırken MTUÖ-8 puanı azalmaktadır. SOY düzeyi açısından istatistiksel olarak anlamlı fark saptanırken tedaviye uyumları açısından anlamlı fark saptanmadı (p=0,001), (p=0,228). VKİ ve HbA1c düzeyleri arttıkça SOYÖ ve MTUÖ-8 puanları azalmaktadır. SOY düzeyleri ve tedaviye uyumları açısından istatistiksel olarak anlamlı fark saptandı (p<0,001, p<0,001). Bireylerin kendi sağlık durumlarına verdikleri puan arttıkça SOYÖ ve MTUÖ-8 puanları artmaktadır. SOY düzeyleri ve tedaviye uyumları açısından istatistiksel olarak anlamlı fark saptandı (p<0,001, p=0,030). SOYÖ puanını istatistiksel olarak anlamlı etkileyen değişkenler Genelleştirilmiş Lineer Modeller (GLM) yöntemi kullanılarak analiz edildi. GLM de bağımlı değişken SOYÖ puanı; eğitim düzeyi, VKİ, gelir düzeyi, meslek, diyabet eğitimi, diyabet tanı süresi, diyabet tedavisi, düzenli ilaç kullanımı, son 1 yıl içerisinde takip için sağlık kuruluşuna başvurma, düzenli takipli olunan sağlık kuruluşu varlığı, kullanılan ilaç sayısı, diyabete bağlı komplikasyon varlığı, MTUÖ-8 puanı bağımsız değişken olarak kabul edildi. Kurulan modelde, üniversite ve lise mezunlarının ilkokul mezunlarına göre, gelir düzeyi orta ve yüksek olanların düşük olanlara göre SOYÖ puanı yüksek bulunup istatistiksel olarak anlamlı katkıya sahip olduğu saptandı (p<0,05). Diyabet eğitimi almayanların alanlara göre, tanı süresi 16 yıl ve üzerinde olanların 0-5 yıl olanlara göre daha düşük SOYÖ puanına sahip olduğu izlendi(p<0,05). MTUÖ-8 puanında 1 birimlik değişimin SOYÖ puanında 0,941 birimlik bir değişime yol açtığı ve bu değişimin istatistiksel olarak anlamlı olduğu saptandı (p<0,05). Sonuç: Tip 2 DM'li hastaların SOY düzeyleri artıkça tedaviye uyum düzeylerinin de anlamlı olarak artış gösterdiği saptandı.

Aim: Diabetes is a chronic disease that requires continuous medical care and patient education; it necessitates support to prevent acute complications and reduce the risk of long-term complications. The management of diabetes requires high health literacy (HL) skills. Low HL is a significant factor that negatively affects an individual's adherence to treatment and, consequently, the management of the disease. In our study, we aimed to assess the relationship between medication adherence and HL in patients diagnosed with type 2 diabetes mellitus (DM) and contribute to the development of strategies to improve medication adherence. Materials and Methods: The study included 220 type 2 DM patients between ages 18-65 who applied to the Family Medicine Outpatient Clinics of Haydarpasa Numune Training and Research Hospital between June 1 and August 1, 2023. Data were collected with face-to-face interviews using a questionnaire form that included the Participant Information Collection Form, Health Literacy Scale (HLS), and Morisky Medication Adherence Scale (MMAS-8). The Participant Information Collection Form included questions about age, gender, marital status, height, weight, body mass index (BMI), education level, HbA1c level, income level, occupation, presence of diabetes in the family, duration of diabetes diagnosis, presence of additional chronic diseases, diabetes education, regular medication use, diabetes treatment, healthcare facility visits for diabetes follow-up in the last year, presence of a regularly visited healthcare facility, total number of medications used, smoking and alcohol habits, and the presence of complications related to diabetes. Participants were asked to evaluate their own health status on a scale of 0-10. Patients were assessed with the HLS and MMAS-8. The data were analyzed using the SPSS Statistics 21.0 program. Statistically, p < 0.05 was considered significant. Results: In our study, it was observed that patients had a moderate average total score on the Health Literacy Scale (HLS) (35.42±10.05), and according to the Morisky Medication Adherence Scale (MMAS-8) scores, 60.2% of individuals exhibited low adherence to treatment. Based on educational levels, individuals with a university degree had higher HLS and MMAS-8 scores compared to those with primary, middle school, and high school education. Similarly, individuals with a normal BMI had higher scores than those with pre-obesity and obesity. Furthermore, those with income exceeding expenses had higher scores compared to those with equal or lower income than expenses. Individuals working as civil servants had higher HLS and MMAS-8 scores compared to housewives, workers, retirees, freelancers, and others. Statistically significant differences were detected in HL levels and adherence to treatment concerning educational levels, BMI categories, income-expenditure status, and occupation (p:<0.001, p:<0.001, p:<0.001, p:0.004, p:<0.001, p:<0.001, p:<0.001,p:<0.001). Statistically significant differences were found in the HLS and MMAS-8 scores based on whether individuals received diabetes education, the type of treatment received (oral antidiabetic drugs (OAD) and/or insulin), regular medication use, healthcare facility visits for diabetes in the last year, regular follow-ups at a healthcare facility, and the presence or absence of complications. Higher scores were observed for individuals who received diabetes education compared to those who did not, for those receiving OAD and insulin treatment compared to those receiving only insulin or a combination of YTD (YTD+OAD+Insulin), for those with regular medication use compared to those without, for those who had healthcare facility visits for diabetes in the last year compared to those who did not, for those with regular follow-ups at a healthcare facility compared to those without, and for individuals without complications compared to those with complications. Statistically significant differences were identified in HL levels and adherence to treatment based on these factors (p:<0.001, p:0.001, p:0.003 p:0.020, p:<0.001, p:<0.001, p:<0.001, p:<0.001, p:<0.001, p:<0.001, p:0.007, p:0.046). No statistically significant differences were detected gender-wise in terms of HL levels and adherence to treatment (p=0.121, p=0.118). Marital status-wise, no statistically significant differences were observed in terms of HL levels and adherence to treatment (p=0.708, p=0.107). Regarding family history of DM, , no statistically significant differences were detected in terms of HL levels and adherence to treatment (p=0.068, p=0.599). Regarding DM duration, while a statistically significant difference was detected in terms of HL levels (p<0.001), no significant difference was detected in terms of treatment compliance (p=0.892). Regarding presence of comorbid chronic diseases, no statistically significant difference was found in terms of HL level and treatment compliance (p=0.100, p=0.416). Regarding number of medication per day, a statistically significant difference was detected in terms of HL levels (p=0.009). However, no significant difference was detected in terms of treatment compliance (p=0.673). No statistically significant difference was found in terms of SOY levels and treatment compliance between smokers and non-smoker (p=0.080, p=0.660). While a statistically significant difference was found in terms of SOY levels of alcohol users compared to non-users (p = 0.032), no significant difference was found in terms of treatment compliance (p=0.235). As age increases, HLS score decreases and MMAS-8 score decreases. While a statistically significant difference was detected in terms of HL level, no significant difference was detected in terms of treatment compliance (p = 0.001), (p = 0.228). As BMI and HbA1c levels increase, HLS and MMAS-8 scores decrease. A statistically significant difference was detected in terms of HL levels and treatment compliance levels (p<0.001, p<0.001). As the score individuals give to their own health status increases, HLS and MMAS-8 scores increase. A statistically significant difference was detected in terms of HL levels and treatment compliance levels (p<0.001, p=0.030). Variables that had a statistically significant effect on the HLS score were analyzed using the Generalized Linear Models (GLM) method. When the dependent variable was considered the HLS score; education level, BMI, income level, occupation, diabetes education, duration of diabetes diagnosis, diabetes treatment, regular medication use, visiting a healthcare facility for follow-up in the last year, presence of a healthcare facility with regular follow-up, number of medications used, presence of diabetes-related complications, MMAS-8 score were accepted as an independent variables. In the established model, it was determined that university and high school graduates had higher HLS scores than primary school graduates, and those with medium and high income levels had higher scores than those with lower income levels, making a statistically significant contribution (p<0.05). It was observed that those who did not receive diabetes education had lower HLS scores compared to those who did, and those whose diagnosis period was 16 years or more compared to those whose diagnosis period was 0-5 years (p <0.05). It was determined that a 1-unit change in the MMAS-8 score led to a 0.941 unit change in the HLS score and this change was statistically significant (p<0.05). Conclusion: It was determined that as the health literacy levels of patients with type 2 DM increased, their treatment compliance levels also increased significantly.
