Özet
Amaç: Bu araştırmanın amacı; bir yoğun bakım ünitesinde SBAR yöntemiyle yapılan nöbet tesliminin hemşirelerin hasta teslimine ilişkin algılarına etkisini değerlendirmektir. Yöntem: Araştırmamız, Ekim 2022- Ocak 2023 tarih aralığında Dr. Behçet UZ Çocuk Hastalıkları ve Cerrahisi Eğitim ve Araştırma Hastanesi'nin Çocuk Yoğun Bakım Ünitesi'nde çalışan 40 hemşire ile yarı-deneysel bir çalışma olarak yapıldı. Araştırmanın verileri, literatür eşliğinde araştırmacı tarafından hazırlanan“Katılımcı Tanıtım Formu”ve“Hasta Teslim Değerlendirme Ölçeği”kullanılarak ön test ve son test olarak toplandı. Veriler hemşirelere“SBAR Teslim Formu”kullanılmaya başlamadan önce ve bu teslim formu üç ay kullanıldıktan sonra Katılımcı Tanıtım Formu ve Hasta Değerlendirme Ölçeği'ni içeren veri toplama formu elden dağıtılıp yüz yüze görüşme yöntemiyle elde edildi. Bulgular: Yoğun bakımda çalışan hemşirelerin Hasta Teslimi Değerlendirme Ölçeği'nden SBAR teslim modelini uygulama öncesi ve sonrasında aldıkları toplam puanlar incelendiğinde; uygulama öncesinde hemşirelerin aldıkları toplam puan (Mean±SS) 73,02±9,15 olurken, uygulama sonrasında alınan toplam puan değeri 77,05 ±8,98 olarak saptanmış; bu fark istatistiksel olarak anlamlı bulunmuştur. Ayrıca SBAR uygulama öncesi ve sonrasında teslimlerde verilen bilgilerin hasta/hemşire bakım kalitesini skala karşılaştırılmasına bakıldığında; SBAR teslim modeli uygulama öncesinde en az 3, fazla 10 puan olurken, SBAR uygulama sonrasında bu puan en az 5, en fazla 10 puan olmuştur. Yoğun bakımdaki nöbet devir teslimlerinde verilen bilgilerin hasta bakım kalitesine olan etkisinin puan ortalaması 8, 47 sayısından 8, 92 sayısına yükseldiği görülmüştür. Teslimde SBAR uygulandıktan sonra bakım kalitesine verilen puanların artışında istatistiksel olarak anlamlı bir fark olduğu saptanmıştır(p=0.045, p < 0.05). Sonuçlar: Bu araştırma bulgularına göre yoğun bakımda çalışan hemşirelerin hasta tesliminde SBAR teslim modelini kulanmasıyla hasta teslimine ilişkin algılarını olumlu yönde etkilemiştir. SBAR teslim formunun sağlık hizmetlerinde kullanımını yaygınlaştırmak için hizmetiçi eğitimlere ihtiyaç vardır.

Aim: The aim of this research; The aim of this study is to evaluate the effect of SBAR-based shift delivery in an intensive care unit on nurses' perceptions of patient delivery. Method: Our research was conducted between October 2022 and January 2023 by Dr. It was conducted as a semi-experimental study with 40 nurses working in the Pediatric Intensive Care Unit of Behçet UZ Pediatrics and Surgery Training and Research Hospital. The data of the study were collected as pre-test and post-test by using the“Participant Introduction Form”and“Patient Delivery Evaluation Scale”prepared by the researcher in the light of the literature. The data were obtained by hand-delivering the data collection form containing the Participant Identification Form and the Patient Evaluation Scale before the“SBAR Submission Form”started to be used and after using this submission form for three months, by face-to-face interview method. Results: When the total scores of the nurses working in the intensive care unit were examined before and after applying the SBAR delivery model from the Patient Delivery Evaluation Scale; While the total score (Mean±SD) of the nurses before the application was 73.02±9.15, the total score obtained after the application was 77.05 ±8.98; this difference was found to be statistically significant. Moreover, when the patient/nurse care quality scale comparison of the information given before and after the SBAR application is examined; While the SBAR delivery model was at least 3 and more than 10 points before the application, this score was at least 5 and at most 10 points after the SBAR application. It was observed that the effect of the information given during the handovers in the intensive care unit on the quality of patient care increased from 8.47 to 8.92. After applying SBAR at delivery, the increase in the scores given to the quality of care was statistically significant (p=0.045, p < 0.05). Conclusion: As a result of the research, according to the findings after the application of the SBAR delivery model of the nurses working in the intensive care unit, it positively affected the nurses' perceptions of patient delivery. In-service training is needed to increase the use of the SBAR delivery form in health services.
