Özet
Giriş ve Amaç: Pnömoni sık görülen ve potansiyel olarak ciddi seyredebilen bir hastalıktır. Erişkin yaşta daha sık görülmekte ve pnömoni nedenli ölümlerin %90'a yakını 65 yaş üstü popülasyonda meydana gelmektedir. Pnömonide prognozu öngörmek için birçok farklı skorlama sistemi geliştirilmiştir. Bu skorlamaları birbiriyle karşılaştıran çalışmalar mevcuttur, ancak sonuçları arasında farklılıklar vardır. Çalışmamızda yakın geçmişte tanımlanan CHUBA (Confusion, Hypoxemia, Urea, Bedridden, Albümin) skorlama sistemi ile A-DROP (Age, Dehydration, Respiratory Failure, Orientation Disturbance, Low Blood Pressure) ve CURB-65 (Confusion, Urea, Respiratory Rate, Blood Pressure, 65 yaş) skorlama sistemlerinin prognostik değerliliğini karşılşatırmayı amaçladık. Gereç ve Yöntem: Çalışmamız tanımlayıcı, gözlemsel, retrospektif veri toplanan, kesitsel bir çalışmadır. Dokuz Eylül Üniversitesi Hastanesi Erişkin Acil Servisine 01.09.2022- 01.09.2023 tarihleri arasında başvuran ve toplum kökenli pnömoni tanısı alan 65 yaş ve üzerindeki tüm hastaların çalışmaya dahil edilmesi planlandı. Kriterlere uyan 956 hasta tespit edildi. Hastane kökenli pnömoni düşünülen, takip verilerine ulaşılamayan, karşılaştırılan skorlama sistemlerinin bileşenlerinden en az birine ait veriye ulaşılamayan, mortaliteyi değiştirecek daha majör bir problemi olan toplam 286 hasta çalışmadan dışlandı. Çalışmaya dahil edilen 670 hastanın demografik özellikleri, eşlik eden hastalıkları, laboratuvar bulguları ve görüntüleme bulguları kaydedilip; CURB-65, A-DROP ve CHUBA skorları hesaplandı. Bu skorların 48 saatlik ve yedi günlük mortaliteyi ve kötü prognostik durum gelişimini öngörmedeki başarıları karşılaştırıldı. Sonuçlar SPSS programında analiz edildi. İstatistiksel anlamlılık düzeyi p<0,05 olarak belirlendi. Bulgular: Çalışmaya dahil edilen 670 hastanın %61,8'i erkek ve yaş ortancası 79 (IQR:72-85) idi. Hastaların 48 saatlik sonlanımda 49'unun (%7,3) ve yedi günlük sonlanımda 88'inin (%13,1) öldüğü, her iki dönemde de kadınlarda ölüm oranının daha fazla olduğu saptandı (p=0,011 ve p=0,007). İlk 48 saatlik sonlanımda yatağa bağlı olma (p<0,001), demans (p<0,001) ve serebrovasküler hastalık (p=0,038) olanlarda mortalite daha yüksek iken, KOAH olanlarda mortalite daha düşük idi (p<0,001). Yedi günlük sonlanımda yatağa bağlı olma (p<0,001), demans (p<0,001) ve serebrovasküler hastalık (p=0,005) olanlarda mortalite daha yüksek iken KOAH açısından fark yoktu. Kötü prognostik durum gelişmesi açısından da yatağa bağlı olma (p<0,001), demans (p=0,006) ve serebrovasküler hastalık (p=0,017) olanlar daha riskli idi. Hastaların 447'si (%66,7) multilober pnömoni, 215'i (%32,1) lober pnömoni ve 8'i (%1,2) bronkopnömoni idi. Radyolojik tutulum alanları karşılaştırıldığında; 48. saat mortalite, 7. gün mortalite ve kötü prognostik durum açısından gruplar arasında fark saptanmadı. Bilinç değişikliği olan hastaların hem 48. saat ve 7. gündeki ölüm oranları, hem de kötü prognostik durum gelişme durumları bilinç değişikliği olmayanlara göre daha fazla idi (her üçü için p<0,001). Ölen hastaların laktat ortancası yaşayanlardan daha yüksekti. Laktat ortancaları; 48. saat için ölenlerde 3,0 mmol/L yaşayanlarda 1,7 mmol/L (p<0,001), 7. günde ölenlerde 2,5 mmol/L ve yaşayanlarda 1,7 mmol/L (p<0,001), kötü prognostik durum gelişenlerde 2,2 mmol/L ve gelişmeyenlerde 1,6 mmol/L (p<0.001) olarak hesaplandı. Hastaların 48 saatlik sonlanımları değerlendirildiğinde; eğri altında kalan alan (Area Under Curve-AUC) değerleri CURB-65 için 0,833, A-DROP için 0,829, CHUBA için 0,854 hesaplandı. Her üç skorlama için cut-off değeri 1.5 alındığında sensitiviteleri benzer iken (sırasıyla %98, %98 ve %96), CHUBA skorlamasının spesifitesi A-DROP ve CURB-65'den daha yüksek idi (sırasıyla %51, %26 ve %17). Yedi günlük sonlanımda AUC değerleri ise CURB-65 için 0,781, A-DROP için 0,778, CHUBA için 0,815 idi. Her üç skorlama için cut-off değeri 1.5 alındığında A-DROP ve CURB-65'in sensitiviteleri (%94 ve %96) ve spesitifiteleri (%28 ve %27) benzer iken, CHUBA skorlamasının sensitivitesi daha düşük (% 88) ve spesifitesi daha yüksek (%53) idi. Her üç skorlama için; skor puanı arttıkça hem 48 saatte hem de 7. gündeki mortalite oranı kademeli olarak artmaktaydı (hepsi için p<0,001). Kötü prognostik durum gelişmesi açısından değerlendirildiğinde NİMV ihtiyacı ile ölüm arasında anlamlı ilişki saptanmadı (p=0,218). İMV ihtiyacı gelişen, inotrop verilen, yoğun bakım yatış ihtiyacı olan, KPR yapılan ve entübe edilen hastalarda 7. günde ölüm oranı daha yüksek idi (hepsi için p<0,001). Kötü prognostik durum gelişmesi açısından AUC değerleri CURB-65 için 0,719, A-DROP için 0,730, CHUBA için 0,740 olarak hesaplandı. Her üç skorlama için cut-off değeri 1.5 alındığında A-DROP ve CURB-65'in sensitiviteleri (%90 ve %90) ve spesitifiteleri (%30 ve %31) benzer iken, CHUBA skorlamasının sensitivitesi daha düşük (%77) ve spesifitesi daha yüksek (%58) idi. Sonuç: Çalışmamız sonuçlarına göre acil serviste toplum kökenli pnömoni tanısı alan 65 yaş ve üzeri hastalarda CURB-65, A-DROP ve CHUBA skoru hastaların 48 saat ve yedi günlük mortalitesini ve kötü prognostik durum gelişme ihtimalini göstermede oldukça başarılıdır. Her üç skorlama sisteminde de skor arttıkça mortalite ve kötü prognostik durum ihtimali kademeli olarak artmaktadır. Geriatrik hastalarda mortaliteyi ve kötü prognostik durumu gösterme açısından bu skorlama sistemleri arasında fark yoktur.

Introduction and Objectives: Pneumonia is a common and potentially serious disease. It is more common in adults and nearly 90% of deaths caused by pneumonia occur in the population over the age of 65. Many different scoring systems have been developed to predict prognosis in pneumonia. There are different studies comparing these scores with each other, but there are differences in their results. In our study, we used the recently defined CHUBA (Confusion, Hypoxemia, Urea, Bedridden, Albumin), A-DROP (Age, Dehydration, Respiratory Failure, Orientation Disturbance, Low Blood Pressure) and CURB-65 (Confusion, Urea, Respiratory Rate, Blood Pressure, 65 years old) scoring systems. We aimed to compare the prognostic value of these scoring systems. Materials and Methods: Our study is a descriptive, observational, retrospective data collecting cross-sectional study. We planned to include all patients aged 65 and over, who were admitted to Dokuz Eylul University Hospital Adult Emergency Service between 01.09.2022 and 01.09.2023 and diagnosed with community-acquired pneumonia. 956 patients meeting the criteria were identified. A total of 286 patients who were considered to have hospital-acquired pneumonia, whose follow-up data could not be obtained, whose data on at least one of the components of the compared scoring systems could not be obtained, and who had a more major problem that would change mortality, were excluded from the study. Demographic characteristics, comorbidities, laboratory findings and imaging findings of 670 patients included in the study were recorded; CURB-65, A-DROP and CHUBA scores were calculated. The success of these scores in predicting 48-hour and seven-day mortality and the development of poor prognostic conditions were compared. The results were analyzed in the SPSS program. Statistical significance level was determined as p<0.05. Results Of the 670 patients included in the study, 61.8% were male and the median age was 79 years (IQR: 72-85). It was determined that 49 (7.3%) of the patients died at the 48-hour endpoint and 88 (13.1%) at the seven-day endpoint, and the death rate was higher in women in both periods (p=0.011 and p=0.007). At the end of the first 48 hours, mortality was higher in those with bedridden state (p<0.001), dementia (p<0.001) and cerebrovascular disease (p=0.038), while mortality was lower in those with COPD (p<0.001). At seven-day outcome, mortality was higher in patients with bedridden state (p<0.001), dementia (p<0.001) and cerebrovascular disease (p=0.005), while there was no difference in terms of COPD. In terms of developing a poor prognostic outcome, those with bedridden state (p<0.001), dementia (p=0.006) and cerebrovascular disease (p=0.017) were more at risk. 447 (66.7%) of the patients had multilobar pneumonia, 215 (32.1%) had lobar pneumonia, and 8 (1.2%) had bronchopneumonia. When radiological involvements are compared; There was no difference between the groups in terms of 48th hour mortality, 7th day mortality and poor prognostic outcome. Both the mortality rates at 48 hours and the 7th day and the development of poor prognostic outcome in patients with a change in consciousness were higher than in patients without a change in consciousness (p<0.001 for all three). The median lactate of deceased patients was higher than that of living patients; 3.0 mmol/L in those who died at the 48th hour, 1.7 mmol/L in those who survived (p<0.001), 2.5 mmol/L in those who died on the 7th day and 1.7 mmol/L in those who survived (p<0.001). It was calculated as 2.2 mmol/L in those who developed poor prognostic condition and 1.6 mmol/L in those who did not (p<0.001). When the 48-hour results of the patients were evaluated; Area Under Curve (AUC) values were calculated as 0.833 for CURB-65, 0.829 for A-DROP, and 0.854 for CHUBA. When the cut-off value was taken as 1.5 for all three scorings, their sensitivities were similar (98%, 98% and 96%, respectively), while the specificity of CHUBA scoring was higher than A-DROP and CURB-65 (51%, 26% and 17%, respectively). At the seven-day endpoint, AUC values were 0.781 for CURB-65, 0.778 for A-DROP, and 0.815 for CHUBA. When the cut-off value was taken as 1.5 for all three scorings, the sensitivities (94% and 96%) and specificities (28% ve 27%) of A-DROP and CURB-65 were similar, while the sensitivity (88%) of CHUBA scoring was lower and specificity (53%) was higher. For all three scorings; As the score increased, the mortality rate in both 48 hours and the 7th day gradually increased (p<0.001 for all). When evaluated in terms of the development of a poor prognostic outcome, no significant relationship was found between the need for NIMV and death (p = 0.218). The mortality rate on the 7th day was higher in patients who required IMV, received inotropes, required intensive care admission, underwent CPR, and were intubated (p<0.001 for all). In terms of the development of a poor prognostic outcome, AUC values were calculated as 0.719 for CURB-65, 0.730 for A-DROP, and 0.740 for CHUBA. When the cut-off value for all three scoring is taken as 1.5, the sensitivities (90% and 90%) and specificities (30% and 31%) of A-DROP and CURB-65 were similar, while the sensitivity (77%) and specificity (58%) of CHUBA score was lower. Conclusion: According to the results of our study, CURB-65, A-DROP and CHUBA scores are very successful in showing the 48-hour and seven-day mortality of patients and the possibility of developing a poor prognostic situation in patients aged 65 and over who were diagnosed with community-acquired pneumonia in the emergency department. In all three scoring systems, as the score increases, the probability of mortality and poor prognostic outcome gradually increases. There is no difference between these scoring systems in terms of indicating mortality and poor prognostic status in geriatric patients.
