Özet

AMAÇ: Akromegali; artmış büyüme hormonu (BH) salgılanmasına bağlı oluşan ve bu nedenle insülin benzeri büyüme faktörü- 1 (IGF-1) salınımı artışı görülen ve yavaş ilerleyen kronik sistemik bir hastalıktır. Akromegalinin en sık sebebi ön hipofizde büyüme hormonu salgılayan adenom varlığı olup bu hasta grubunda akral kemik ve yumuşak dokularda büyüme görülür. Artmış BH ve IGF-1'e bağlı çeşitli lokal veya sistemik belirtiler ortaya çıkmaktadır. Akromegali hastalarında gastrointestinal sistem belirtilerinden bazıları; kolonda polip sıklığında artış, kolon uzunluğunda artış, mukozal kıvrımlarda artış ve organomegali görülmesidir. Artmış kolorektal kanser riski nedeniyle söz konusu hastalarda kolonoskopi yapılması önerilmektedir. Bu çalışmadaki amacımız; akromegali hastalarının kolonoskopi bulgularının daha net değerlendirilebilmesi adına bağırsak temizliğinin önemini vurgulamak, kolonoskopi bulgularını değerlendirmek, kolonoskopi işlemi öncesi rutin bağırsak temizliğinin yeterliliğini belirlemek ve böylece kolorektal neoplazi riski yüksek olan bu hastalıkta tanının daha net ve doğru konulmasını hedeflemektir. YÖNTEM: Bu çalışmaya Ağustos 2013 ile Ağustos 2023 tarihleri arasında Prof. Dr. Cemil Taşcıoğlu Şehir Hastanesi Endokrinoloji ve Metabolizma Hastalıkları Polikliniği'nde takipli olan 18-80 yaş arası akromegali tanılı 92 hasta vaka grubu olarak dahil edilmiştir. Kontrol grubu olarak da Prof. Dr. Cemil Taşcıoğlu Şehir Hastanesi Gastroenteroloji Polikliniği'nde bilinen kronik hastalığı olmayan ve tarama amaçlı kolonoskopisi yapılan 18-80 yaş arası 92 kişi çalışmaya dahil edilmiştir. Bu çalışmada vaka grubundaki ve kontrol grubundaki hastaların yaşları, cinsiyetleri ve kolonoskopi raporları hasta dosyaları incelenerek elde edilmiştir. Çalışmada bu veriler retrospektif olarak incelenmiştir. Çalışmaya; kolonoskopi işlemi öncesi aynı bağırsak temizliği prosedürü aynı süre ile anlatılan ve bağırsak temizliği için aynı ilaçlar kullanılan, akromegali tanısı olan ve akromegali tanısı olmayan hastaların aynı sedasyon protokolü uygulanması altında, aynı gastroenterolog tarafından yapılmış olan kolonoskopi işlemi sonuçları ve bulguları dahil edilmiştir. Gebe hastalar, 18-80 yaş aralığı dışındaki hastalar, kolonoskopi raporunda bağırsak temizliği belirtilmeyen hastalar ve bilinen malignite öyküsü olan hastalar çalışmaya dahil edilmemiş olup bu özelliklerin varlığı çalışmamızın dışlanma kriterlerini olarak belirlenmiştir. Çalışmaya dahil edilen akromegali hastaları ve kontrol grubu vakalarının kolonoskopi bulguları detaylı şekilde kaydedilmiş ve bağırsak temizlikleri ise Aronchick Bağırsak Hazırlığı Skalası (ABPS) ile değerlendirilmiştir. BULGULAR: Çalışmaya dahil edilenlerin yaş ortalaması 52,1±11,84 yıl, ortanca yaş 53 yıl olarak hesaplandı. Çalışmaya 92 kontrol ve 92 akromegali olmak üzere 184 kişi dahil edildi. Akromegali hastalarının %66.30'u (n=61) kadın, %33.70'i (n=31) erkek olup kontrol grubunun %53.26'sı (n=49) kadınken, %46.74'ü (n=43) erkekti. Çalışmada kontrol ve akromegali grupları arasında cinsiyet açısından istatistiksel anlamlı düzeyde fark gözlenmedi (p>0,05). Kontrol grubunda yer alan katılımcıların yaşı akromegali grubundan istatistiksel anlamlı düzeyde yüksekti (p<0,01). Akromegali grubunda kolonoskopinin yetersiz kolon temizliği ile sonuçlanma sıklığı kontrol grubundan istatistiksel anlamlı düzeyde yüksek bulundu (p<0,01). Çalışmada akromegali grubunda olanların ABPS 4 ve ABPS 5 sıklığı kontrol grubundan yüksekken ABPS 1 ve ABPS 2 olma sıklığı düşüktü. Gruplar arasında istatistiksel anlamlı fark mevcuttu (p<0,01). Çalışmaya dahil edilen kadınlardan akromegali olanların ABPS 1 sıklığı kontrol grubundan düşükken ABPS 2 sıklığı yüksekti. Gruplar arasında istatistiksel anlamlı fark mevcuttu (p<0,01). Erkeklerde kontrol grubunun ABPS 1 sıklığı akromegali grubundan yüksekken ABPS 4 ve ABPS 5 sıklığı düşüktü. Gruplar arasında istatistiksel anlamlı farklılık gözlendi (p<0,01). Erkeklerde akromegali grubunun yetersiz kolon temizliği sıklığı kontrol grubundan istatistiksel anlamlı düzeyde yüksek bulundu (p<0,05). Akromegali grubunda hemoroid sıklığı kontrol grubundan istatistiksel anlamlı düzeyde yüksek bulundu (p<0,01). Akromegali ile kontrol grubu arasında polip varlığı ve divertikül varlığı açısından istatistiksel düzeyde anlamlı fark (sırasıyla; p=0.360, p=0.254) gözlenmedi. Kadınlarda akromegali olanlarda hemoroid sıklığı kontrol grubundan istatistiksel anlamlı düzeyde yüksekti (p<0,01). Kontrol ve akromegali grupları arasında polip sayısı açısından istatistiksel anlamlı bir fark gözlenmedi (p>0,05). Çalışmada kontrol ve akromegali grupları arasında çoklu polip sıklığı açısından istatistiksel anlamlı düzeyde fark gözlenmedi (p>0,05). Kontrol grubunda polip saptananların yaşı polip saptanmayanlardan istatistiksel anlamlı düzeyde yüksek bulundu (p<0,01). Akromegali grubunda polip saptananların yaşı polip saptanmayanlardan istatistiksel anlamlı düzeyde yüksek bulundu (p<0,05). Çalışmada kadınlarda kontrol grubunun yaş ve polip sayısı, akromegali grubundaki kadınlardan istatistiksel anlamlı düzeyde yüksek bulundu (p<0,05). Çalışmada erkeklerde kontrol ve akromegali grupları arasında yaş ve polip sayısı açısından istatistiksel anlamlı farklılık gözlenmedi (p>0,05). SONUÇ: Kılavuzlar akromegali hastalarına tanı anında ve takipte kolonoskopi yapılmasını önermektedir. Kolon patolojilerinin eksiksiz ve doğru değerlendirilebilmesi için yeterli bir bağırsak temizliğinin olması gerekmektedir. Çalışmamızın sonuçları kolonoskopi işlemi öncesi uygulanan bağırsak temizliğinin önemini vurgular niteliktedir. Çalışmamızda; akromegali hastalarında rutin bağırsak hazırlığının genel popülasyona oranla daha yetersiz kaldığı sonucuna ulaşılmış olup, kolorektal neoplazi riskinin arttığı bu hasta grubunda yeterli bağırsak temizliği için yeni bir prosedür belirlenmesi gerekliliği ortaya çıkmıştır. Akromegali hastaları kolonoskopi işlemi öncesi ek bağırsak temizliği yapılması açısından değerlendirilmeli ve en uygun prosedür ile işlem öncesi bağırsak temizliği yapılmalıdır. Bu hasta grubunda, klinisyenlerin de hastaların da kolonoskopi işlemi öncesi hazırlık sürecinde bu açılardan farkındalıklarının artması gerekmektedir.

PURPOSE: Acromegaly is a chronic systemic disease characterized by slow progression due to increased growth hormone (GH) secretion, which leads to elevated insulin-like growth factor-1 (IGF-1) secretion. The most common cause of acromegaly is the presence of a growth hormone-secreting adenoma in the anterior pituitary, and in this patient group, growth in the acral bones and soft tissues is observed. Various local or systemic symptoms emerge due to increased GH and IGF-1. Some gastrointestinal system symptoms in acromegaly patients include an increase in the frequency of polyps in the colon, an increase in the length of the colon, increased mucosal folds, and organomegaly. Due to the increased risk of colorectal cancer in these patients, a colonoscopy is recommended. Our aim in this study is to emphasize the importance of bowel cleansing for the accurate evaluation of colonoscopy findings in acromegaly patients, to assess colonoscopy findings, to determine the adequacy of routine bowel cleansing before the colonoscopy procedure, and thereby to achieve a more accurate and precise diagnosis in this disease, which has a high risk of colorectal neoplasia. METHOD: Ninety-two patients aged 18-80 with a diagnosis of acromegaly, followed up at the Endocrinology and Metabolic Diseases Clinic of Prof. Dr. Cemil Taşcıoğlu City Hospital between August 2013 and August 2023, were included as the case group in this study. As the control group, ninety-two individuals aged 18-80 who had no known chronic diseases and underwent colonoscopy for screening purposes at the Gastroenterology Clinic of Prof. Dr. Cemil Taşcıoğlu City Hospital were included. In this study, the ages, genders, and colonoscopy reports of the patients in the case and control groups were obtained by examining patient files. These data were retrospectively analyzed. The study included colonoscopy results and findings from procedures performed by the same gastroenterologist under the same sedation protocol. These patients, both with and without a diagnosis of acromegaly, underwent the same bowel preparation procedure for the same duration and used the same medications for bowel cleansing prior to the colonoscopy. Pregnant patients, patients outside the age range of 18-80, patients whose colonoscopy reports did not indicate bowel cleanliness, and patients with a known history of malignancy were excluded from the study as these characteristics constituted the exclusion criteria. The colonoscopy findings of the acromegaly patients and the control group included in the study were recorded in detail, and bowel cleanliness was assessed using the Aronchick Bowel Preparation Scale (ABPS). FINDINGS: The average age of those included in the study was calculated to be 52.1±11.84 years, with a median age of 53 years. The study included 184 individuals, consisting of 92 controls and 92 acromegaly patients. Of the acromegaly patients, 66.30% (n=61) were women and 33.70% (n=31) were men, while in the control group, 53.26% (n=49) were women and 46.74% (n=43) were men. No statistically significant difference was observed between the control and acromegaly groups in terms of gender (p>0.05). The age of participants in the control group was statistically significantly higher than those in the acromegaly group (p<0.01). In the acromegaly group, the frequency of colonoscopy resulting in inadequate bowel preparation was statistically significantly higher than in the control group (p<0.01). The frequencies of ABPS 4 and ABPS 5 in the acromegaly group were higher, while the frequencies of ABPS 1 and ABPS 2 were lower compared to the control group. There was a statistically significant difference between the groups (p<0.01). Among the women included in the study, those with acromegaly had a lower frequency of ABPS 1 and a higher frequency of ABPS 2 compared to the control group. There was a statistically significant difference between the groups (p<0.01). In men, the frequency of ABPS 1 was higher in the control group than in the acromegaly group, while the frequencies of ABPS 4 and ABPS 5 were lower. A statistically significant difference was observed between the groups (p<0.01). The frequency of inadequate bowel preparation was statistically significantly higher in men with acromegaly compared to the control group (p<0.05). The frequency of hemorrhoids in the acromegaly group was statistically significantly higher than in the control group (p<0.01). There was no statistically significant difference between the acromegaly and control groups in terms of the presence of polyps and diverticula (p=0.360 and p=0.254, respectively). Among women, those with acromegaly had a statistically significantly higher frequency of hemorrhoids compared to the control group (p<0.01). No statistically significant difference was observed between the control and acromegaly groups in terms of the number of polyps (p>0.05). There was no statistically significant difference observed between the control and acromegaly groups regarding the frequency of multiple polyps (p>0.05). In the control group, the age of participants with polyps was statistically significantly higher than those without polyps (p<0.01). In the acromegaly group, the age of participants with polyps was statistically significantly higher than those without polyps (p<0.05). In the study, the age and number of polyps in women in the control group were statistically significantly higher than in women in the acromegaly group (p<0.05). No statistically significant difference was observed between the control and acromegaly groups in terms of age and number of polyps in men (p>0.05). CONCLUSION: Guidelines recommend performing colonoscopy at diagnosis and during follow-up in acromegaly patients. Adequate bowel preparation is necessary for the complete and accurate evaluation of colon pathologies. The results of our study highlight the importance of bowel preparation before the colonoscopy procedure. Our study found that routine bowel preparation in acromegaly patients is less adequate compared to the general population. In this patient group, where the risk of colorectal neoplasia is increased, there is a need to establish a new procedure for adequate bowel preparation. Acromegaly patients should be evaluated for additional bowel preparation before the colonoscopy procedure and should perform bowel preparation using the most appropriate procedure. In this patient group, both clinicians and patients need to increase their awareness regarding the preparation process before the colonoscopy procedure.
