Özet
Amaç: Çalışmamız Status Epileptikus'ta (SE) mortalite ve morbidite üzerine etki eden faktörlerin değerlendirilmesi ve prognozun göstergelerinin belirlenmesi amacıyla yapıldı. Gereç ve Yöntem: Çalışmamızda Ankara Eğitim ve Araştırma Hastanesi Acil Servisi'ne 1 Ocak 2017 ve 31 Aralık 2023 tarihleri arasında başvuran ve nöroloji konsültasyonu sonucu nöroloji doktoru tarafından SE tanısı konulan hastalar retrospektif olarak incelendi. SE'de mortalite ve morbiditeye etki edebilecek entübasyon gerekliliği, yaş, elektroensefalografi bulguları, hastanede yatış süresi, bilinen epilepsi hastalığı varlığı, komorbid hastalıklar, nörogörüntüleme bulguları gibi parametreler araştırıldı. Hastaların komorbid hastalıklarının değerlendirilmesi amacıyla Charlson Komorbidite Ölçeği kullanıldı. Hastaların engellilik ve bağımlılık durumları Modifiye Rankin Skoru (mRS) kullanılarak hastaneye giriş, hastaneden çıkış ve 3. ay poliklinik veya acil servis başvurularında değerlendirildi. Prognoz göstergesi olarak Status Epileptikus'ta Epidemiyolojiye Dayalı Mortalite Skoru (EMSE), Status Epileptikus Şiddet Skoru (STESS) ve END-IT skorları kullanıldı. Bulgular: Çalışmamızda SE tanısı alan 101 hastada toplamda 105 SE atağı kaydedildi. Hastaların yaş ortalaması 45.90 ± 18.83, hastanede yattığı gün sayısı ortalaması ise 21.16 ± 26.99 olarak bulundu. Hastaların %44.8'i (n=47) kadındı. İlk kez epileptik olay geçiren hasta sayısı 38 (%36.2) olarak saptandı. SE ataklarının %14.2'si ölümle sonuçlandı. Entübasyon gerektiren hastalarda ölüm oranı (%37.5), gerektirmeyen hastalara göre (%4.1) anlamlı olarak daha yüksek bulundu (p<.001). Epilepsi tanısı olan hastalarda ölüm oranı (%9.2), olmayanlara göre (%27.6) anlamlı olarak daha düşük bulundu (p=.027). Hastaların hastaneye giriş, hastaneden çıkış ve 3 ay sonundaki mRS medyan ve çeyrekler arası dilim (IQR) değerleri sırasıyla 4 (IQR, 3-5), 2 (IQR, 0-4) ve 2 (IQR, 0-5) olarak bulundu. STESS skoru medyan 2 (IQR 1-3), END-IT skoru medyan 2 (IQR, 1-2) ve EMSE skoru ortalaması 72.19 ± 39.26 olarak bulundu. STESS değeri 3'ün altında olan hastalarda ölüm (%8.3), 3 ve 3'ün üzerinde olan hastalara göre anlamlı olarak daha düşük bulundu (%27.3) (p=.016). END-IT değeri 3'ün altında olan hastalarda ölüm oranı (%7.5), 3 ve 3'ün üzerinde olan hastalara göre (%36.0) anlamlı olarak daha düşük bulundu (p=.001). Mortalite tahmini için 59'dan büyük EMSE değeri için AUC=.575 (SE=.067), duyarlılık değeri %80.00 seçicilik değeri ise %46.67 olarak bulundu. Ölüm ile cinsiyet, SE süresi ve EMSE değeri arasında anlamlı bir ilişki bulunmadı (p>.05). Sonuç: END-IT SE'de prognostik değerlendirme amacıyla daha önce ülkemizde kullanılmamış olup END-IT'e ülkemizde ilk kez çalışmamızda yer verilmiştir. SE yüksek mortalite oranına sahip olup tanı anında prognoz tahmininde skorlama sistemleri kullanılabilir. END-IT ve STESS, EMSE'ye göre daha az veri kullanılarak hesaplanabildiğinden daha pratik olabilir. END-IT ve STESS'i mortaliteyi tahmin etmek yerine, SE'de iyi prognozu tahmine etmek için kullanmak daha uygun olabilir. Çalışmamızda EMSE ile SE'de mortalite arasında bir ilişki saptamadık. Daha objektif veriler elde edebilmek için prospektif çalışmalara ihtiyaç vardır.

Purpose: Our study was conducted to observe the effects on mortality and morbidity in Status Epilepticus (SE) and to determine the prognosis. Materials and Methods: In our study, patients who applied to Ankara Training and Research Hospital Emergency Service between January 1, 2017, and December 31, 2023, and were diagnosed with SE by the neurologist as a result of neurology consultation were retrospectively examined. Parameters such as the need for intubation, age, electroencephalography findings, length of hospital stay, known presence of epilepsy, comorbid diseases, and neuroimaging findings that may affect mortality and morbidity in SE were investigated. The Charlson Comorbidity Index was used to evaluate the patients comorbid diseases. Disability and dependency status of the patients were evaluated using the Modified Rankin Scale (mRS) at hospital admission, hospital discharge, and 3-month outpatient clinic or emergency room admissions. The Epidemiology-Based Mortality Score in Status Epilepticus (EMSE), Status Epilepticus Severity Score (STESS) and END IT scores were used as prognosis indicators. Results: In our study, a total of 105 SE attacks were recorded in 101 patients diagnosed with SE. The average age of the patients was 45.90 ± 18.83 years, and the average number of days they were hospitalized was 21.16 ± 26.99 days. Women comprised 44.8% (n=47) of the patients. The number of patients who had an epileptic seizure for the first time was 38 (36.2%). SE attacks resulted in death in 14.2% of cases. The mortality rate in patients requiring intubation (37.5%) was found to be significantly higher than in patients not requiring intubation (4.1%) (p<.001). The mortality rate in patients with a diagnosis of epilepsy (9.2%) was found to be significantly lower than in those without a diagnosis of epilepsy (27.6%) (p = .027). The mRS median and interquartile range (IQR) values of the patients at hospital admission, hospital discharge and at the end of 3 months were found to be 4 (IQR, 3-5), 2 (IQR, 0-4) and 2 (IQR, 0-5), respectively. The median STESS score was 2 (IQR 1-3), the median END-IT score was 2 (IQR, 1-2), and the average EMSE score was 72.19 ± 39.26. Mortality was found to be significantly lower in patients with a STESS value less than 3 (%8.3), than in patients with a STESS value of 3 or higher (27.3%) (p=.016). The mortality rate in patients with an END-IT value less than 3 (7.5%), was found to be significantly lower than in patients with a value of 3 or higher (36.0%) (p=.001). For mortality prediction, an EMSE value greater than 59 had an AUC of .575 (SE=.067), sensitivity of 80.00%, and selectivity of 46.67%. There was no significant relationship between death and gender, SE duration, and EMSE value (p>.05). Conclusion: END-IT has not been used before in Turkey for prognostic evaluation in SE, and END-IT was introduced in our study for the first time in our country. SE has a high mortality rate, and scoring systems can be used to predict prognosis at the time of diagnosis. END-IT and STESS may be more practical because they can be calculated using less data than EMSE. It may be more appropriate to use END-IT and STESS to predict a favorable prognosis in SE rather than predicting mortality. In our study, we did not detect a relationship between EMSE and mortality in SE. Prospective studies are needed to obtain more objective data.
