Özet

Amaç: Cerrahinin tetiklediği stres yanıtının vücutta oluşturduğu fizyolojik etkilerin postoperatif dönemde olumsuz sonuçlara neden olabildiği bilinmektedir. Prehabilitasyon, hastaların operasyona fiziksel ve psikolojik olarak hazırlanıp fonksiyonel kapasitelerinin iyileştirilmesi için yapılan müdahalelerdir. Bu çalışmanın amacı akciğer rezeksiyonu planlanan hastalarda prehabilitasyonun postoperatif derlenme kalitesi üzerine etkilerini incelemektir. Sağlık durumu hasta bildirimine dayanan ölçekle ile değerlendirildi. Ayrıca hastane yatış süresi, yoğun bakım ihtiyacı ve postoperatif komplikasyonlar da incelendi. Gereç ve Yöntem: Bu prospektif, randomize kontrollü çalışmaya 80 hasta dahil edildi. Kontrol grubuna (Grup K, n=40) standart yaklaşım olan detaylı anamnez ve preoperatif değerlendirme yapıldı. Medikal optimizasyon sağlandı; sigaranın kesilmesi ve günlük yürüyüş önerildi. Prehabilitasyon grubuna (Grup P, n=40) ek olarak preoperatif bir hafta inspiratuar egzersiz ve 5000 adım/gün aerobik egzersiz yaptırıldı. Grup P'de 2 hasta çalışma dışı bırakıldı. Primer çıktı olarak postoperatif ikinci gün derlenme kalitesine Quality of Recovery-15 (QoR-15) anketi ile bakıldı. Diğer çıktılar postoperatif birinci ay derlenme kalitesi, hastanede kalış süresi, yoğun bakım ihtiyacı, postoperatif pulmoner komplikasyonlar ve Clavien-Dindo skorları olarak belirlendi. Bulgular: Hastaların demografik ve intraoperatif verileri benzerdi. Postoperatif ikinci gün QoR-15 skorları, Grup K'da (81.5 ± 17.8) Grup P'ye göre (92.9 ± 14.5) anlamlı olarak düşük bulundu. (p=0.002) Postoperatif birinci aydaki QoR-15 skorlarında ise iki grup arasında anlamlı fark saptanmadı. İki grup arasında hastanede kalış süresi, YBÜ yatış ihtiyacı, postoperatif pulmoner komplikasyonlar ve Clavien-Dindo skorları benzerdi. Sonuç: Akciğer rezeksiyonlarında, standart bakıma eklenen yedi günlük solunum ve aerobik egzersizlerden oluşan prehabilitasyon programının erken postoperatif dönemde derlenmeyi olumlu etkilediği hasta bazlı ölçekle gösterilmiştir. Çalışmamızda prehabilitasyon programının postoperatif komplikasyonlar ve hastane yatış süresi ile geç dönem derlenme değerlendirmesi üzerine olumlu etkisi gösterilememiştir.

Objective: It is known that the physiological effects of the stress response triggered by surgery on the body can cause negative outcomes in the postoperative period. Prehabilitation is the interventions performed to prepare patients physically and psychologically for the operation and to improve their functional capacity. The aim of this study was to examine the effects of prehabilitation on the quality of postoperative recovery in patients planned for lung resection. Health status was assessed with a scale based on patient report. In addition, length of hospital stay, need for intensive care and postoperative complications were examined. Material and Methods: This prospective, randomized controlled study included 80 patients. The control group (Group K, n=40) underwent a standard approach of detailed anamnesis and preoperative evaluation. Medical optimization was provided; smoking cessation and daily walking were recommended. The prehabilitation group (Group P, n=40) additionally underwent one week of inspiratory exercise and 5000 steps/day of aerobic exercise preoperatively. Two patients in Group P were excluded from the study. The primary outcome was the quality of recovery on the second postoperative day using the Quality of Recovery-15 (QoR-15) questionnaire. Other outcomes were determined as the quality of recovery at the first postoperative month, length of hospital stay, need for intensive care, postoperative pulmonary complications and Clavien-Dindo scores. Results: Demographic and intraoperative data of the patients were similar. Postoperative second day QoR-15 scores were significantly lower in Group K (81.5 ± 17.8) compared to Group P (92.9 ± 14.5). (p=0.002) No significant difference was found between the two groups in terms of QoR-15 scores in the first month postoperatively. Hospital stay, need for ICU admission, postoperative pulmonary complications and Clavien-Dindo scores were similar between the two groups. Conclusion: In lung resections, a prehabilitation program consisting of seven days of respiratory and aerobic exercises added to standard care has been shown to have a positive effect on early postoperative recovery using a patient-based scale. In our study, the positive effect of the prehabilitation program on postoperative complications, hospital stay, and late-term recovery assessment could not be demonstrated.
