The relationship between sperm midpiece details and DNA fragmentation in human sperm

Özet

Our study aimed to investigate the relationship between sperm morphology and sperm DNA damage in men with teratozoospermia issues. Between December 2022 and June 2024, 239 infertile men aged 18-45 were prospectively enrolled. Semen samples from each participant were thoroughly analyzed, and defects in the head, neck-middle piece, and tail regions were recorded based on 50 randomly selected spermatozoa per patient, totaling 11,950 spermatozoa. Data comparing abnormal versus normal morphology groups were used to calculate a cutoff value for DNA fragmentation index (DFI), which was then applied to compare patient data. Subsequently, correlations between midpiece measurements and DFI were assessed. A cutoff value of 19.50 was determined for sperm DFI. The ratio of normal neck and midpiece in DFI <%20 group was found to be %35.60 ± 3.51, whereas in DFI ≥%20 group, it was %34.56 ± 3.44 (p = 0.030). The ratio of thin-thick neck and midpiece in DFI <%20 group was %5.71 ± 2.25, compared to %6.31 ± 2.43 in DFI ≥%20 group (p = 0.045). While parameters like midpiece width and length were shown to be associated with sperm DNA fragmentation, the data demonstrated that midpiece area had the strongest correlation with DFI (r=-0.529 and r=0.460, p<0.001). This study highlights a robust relationship between teratozoospermia and sperm midpiece area with sperm DNA fragmentation. These findings, along with other multidimensional approaches and data visualization methods, may assist researchers in quantitatively determining the complex characteristics of sperm and elucidating the functional significance of this diversity.
