Özet

Sevofluran, pediyatrik anestezide en sık kullanılan anestezik ajanlardan biridir. Sevofluran ilişkili nörotoksisitenin (SİN) ve bununla ilişkili kognitif disfonksiyonun kesin mekanizması hala bilinmemektedir. Nuclear Enriched Abundant Transcript 1 (NEAT1), LncRNA moleküllerinden birisidir. NEAT1'in embriyonik kök hücre gelişimi, nöronal eksitabilite ve immun yanıt gibi pek çok fizyolojik fonksiyonda rol aldığı gösterilmiştir. miR-124, NEAT1'in valide edilmiş hedeflerinden birisidir. Bu çalışmada postnatal 7.günde olan BALB/c yavru farelerin sevofluran anestezisi sonrası NEAT1 ve hedefi olan miR-124'ün ekspresyonun değerlendirilmesi ve olası ekspresyon değişikliğinin hipokampal apoptoz ile ilişkisinin araştırılması amaçlanmıştır. Bu amaçla, postnatal 7.günde olan BALB/c (WT) ve NEAT1 Knockout (KO) fareler Kontrol WT grubu (n=5), Sevofluran WT grubu (n=5), Kontrol KO grubu (n=5) ve Sevofluran KO grubu (n=5) olmak üzere 4 gruba ayrılmıştır. Sevofluran gruplarına 6 saat boyunca 6 L. dk-1 akım hızında oksijen içinde % 2,5 sevofluran solutulmuş, altı saatin sonunda fareler sakrifiye edilerek hipokampusları çıkarılmış NEAT1 ve miR-124 ekspresyon değişiminin belirlenmesi için Real Time PCR yöntemi kullanılmıştır. Sevofluran toksisitesinin paraspekl oluşumuna etkisinin belirlenmesi NEAT1 FISH (fluorescence in situ hybridization) ve SFPQ İmmün Boyamasının birlikte yapılması ile değerlendirilmiştir. Ekspresyon farklılığı ΔΔCt yöntemi ile analiz edilmiştir. NEAT1 ekspresyonunun sevofluran uygulanan farelerde kontrol grubuna göre anlamlı (p<0.001) şekilde arttığı, miR124 ekspresyonunun ise anlamlı (p<0.01) şekilde azaldığı saptanmıştır. Kresil violet boyamalarına göre nöronal dansitenin sevofluran grubunda prefrontal kortekste anlamlı olarak azaldığı saptanmıştır (p < 0.001). Sevofluran grubunda CA1 bölgesinde kontrole göre Bcl-2 pozitif hücre sayısında azalma saptanmıştır. KO gruplarda ise, Bcl-2 immun boyamasında azalma gözlenirken sevofluran grubunda artış gözlenmiştir. Sevofluran grubunda kontrole göre prefrontal kortekste ve hipokampüste cleaved-caspase 3 ile boyanan hücrelerde artış saptanmıştır. Kontrol ve sevofluran grubu farelerde yapılan NeuN boyamasında hipokampüs (CA1) bölgesinde anlamlı azalma saptanmıştır. KO gruplarında NeuN boyamasında sevofluran grubunda artış gözlenmiştir. Sonuç olarak, immun boyamalarda NEAT1 KO farelerde geri dönüşün saptanması sevofluran ilişkili nörotoksisitede NEAT1'in rolü olduğunu göstermiştir.

Sevoflurane is one of the most commonly used anesthetic agents in pediatric anesthesia. The exact mechanism of sevoflurane-associated neurotoxicity (SIN) and associated cognitive dysfunction is still unknown. Nuclear Enriched Abundant Transcript 1 (NEAT1) is one of the LncRNA molecules discovered in 2007. NEAT1 has been shown to be involved in many physiological functions such as breast development, corpus luteum formation, embryonic stem cell development, neuronal excitability and immune response. miR-124 is one of the validated targets of NEAT1. The aim of this study was to evaluate the expression of NEAT1 and its target miR- 124 after sevoflurane anesthesia in postnatal day 7 BALB/c mice and to investigate the relationship between possible expression changes and hippocampal apoptosis. A total of 10 BALB/c and NEAT1 Knockout mice, Control WT group (n=5), Sevoflurane WT group (n=5), Control KO group (n=5) and Sevoflurane KO group (n=5) were included in the study on postnatal day 7. Mice were inhaled with 2.5% sevoflurane in oxygen at a flow rate of 6 L min-1 for 6 hours. At the end of 6 hours, mice were sacrificed by decapitation method and their hippocampuses were removed and Real Time PCR method was used to determine NEAT1 and miR- 124 expression changes. The effect of sevoflurane toxicity on paraspecule formation was evaluated by NEAT1 FISH (fluorescence in situ hybridization) and NONO/SFPQ Immunostaining. Whether sevoflurane toxicity is mediated by NEAT1 was examined by functional experiments using NEAT1 KO mice. It was determined that NEAT1 expression increased significantly (p<0.001) in sevoflurane-treated mice compared to the control group, while miR124 expression decreased significantly (p<0.01). According to cresyl violet staining, neuronal density was found to decrease significantly in the prefrontal cortex in the sevoflurane group (p < 0.001). A decrease in the number of Bcl-2 positive cells was detected in the CA1 region in the sevoflurane group compared to the control. In the KO groups, a decrease in Bcl-2 immunostaining was observed, while an increase was observed in the sevoflurane group. An increase in cells stained with cleaved-caspase 3 was detected in the prefrontal cortex and hippocampus in the sevoflurane group compared to the control group. A significant decrease was detected in the hippocampus (CA1) region in NeuN staining in control and sevoflurane group mice. In the KO groups, an increase in NeuN staining was observed in the sevoflurane group. In conclusion, detection of reversal in NEAT1 KO mice in immunostainings showed that NEAT1 has a role in sevoflurane-related neurotoxicity.
