Özet
Manuel lenf drenajı (MLD), bozulmamış lenfatikler yoluyla lenf akışını artırmayı ve bu akışı sağlıklı lenfatikler ile diğer bölgelere yönlendirmeyi sağlayan, dört temel el tutuş tekniğinden (döndürme, duran daireler, pompalama ve kepçeleme) oluşan özel bir masaj tekniğidir. Bu çalışmanın amacı; MLD'nin farklı el tutuş tekniklerinin üst ve alt ekstremitede lokal dokudaki su oranı ve doku kalınlığına etkisinin karşılaştırılmasıdır. Çalışmaya sağlıklı 24 kadın ve 3 erkek gönüllük esasıyla katıldı. Üst ve alt ekstremitelere 20 tekrarlı uygulanan duran daireler ve kepçeleme el tutuş teknikleri öncesi ve sonrasında, radyoloji ekibi tarafından deri altı doku kalınlıkları ve fizyoterapist tarafından lokal dokudaki su oranı değerlendirildi. Ekstremite çevre ölçümü Leg Q Meter ölçüm aleti kullanılarak esnemeyen mezura ile yapıldı. Lokal doku su oranı doku dielektrik sabiti ölçüm cihazı olan MoistureMeterD Compact ile değerlendirildi. Deri altı doku kalınlıkları ultrasonografik görüntüleme cihazı ile yapıldı. Bu çalışmanın sonuçlarına göre, üst ve alt ekstremitede lokal doku su oranı ve deri altı doku kalınlığına etkileri açısından duran daire ve kepçeleme el tutuş tekniklerinin birbirlerine üstünlüğü bulunmadı (p>0,05). Objektif ölçüm yöntemleri kullanılarak yapılan ölçümlerde duran daire ve kepçeleme el tutuş tekniklerinin uygulama öncesi ve sonrası belirgin fark oluşturmaması, her iki tekniğin de MLD ve“self”-MLD uygulamalarında birbiri yerine kullanılabileceğini ortaya koydu.

Manual lymphatic drainage (MLD) is a special massage technique consisting of four basic hand grip techniques (rotary, stationary circle, pump and scoop) that allows to increase lymph flow through intact lymphatics and redirect this flow to other areas with healthy lymphatics. The purpose of this study was to compare of the effects of different hand grip techniques of MLD on the water rate and tissue thickness in the local tissue in the upper and lower extremities. 24 healthy women and 3 men participated in the study on a voluntarily. Before and after stationary circles and scoop hand grip techniques were applied to the upper and lower extremities 20 times, subcutaneous tissue thickness was evaluated by the radiology team and the water rate in the local tissue was evaluated by the physiotherapist. Extremity circumference measurement was applied with a non-stretchable tape measure using the Leg Q Meter measuring device. Local tissue water ratio was evaluated with MoistureMeterD Compact, a tissue dielectric constant measuring device. Subcutaneous tissue thicknesses were measured with an ultrasonographic imaging device. According to the results of this study, stationary circle and scoop hand grip techniques were not superior to each other in terms of their effects on local tissue water ratio and subcutaneous tissue thickness in the upper and lower extremities (p>0.05). The fact that stationary circle and scoop hand holding techniques did not create a significant difference before and after the application in the measurements made using objective measurement methods revealed that both techniques can be used interchangeably in MLD and“self”-MLD applications.
