Özet
Yara iyileşmesi, hasar gören dokunun bütünlüğünü ve fonksiyonunu yeniden kazandıran karmaşık, dinamik bir süreçtir. Serbest diş eti grefti, laterale kaydırılan flep, frenektomi, gingivektomi/gingivoplasti gibi periodontal cerrahi işlemler sonrası oluşan yaralar genellikle sekonder iyileşme ile kapanır. Ağız içinde meydana gelen bu yara alanları, hastanın konforunu, beslenmesini ve yaşam kalitesini önemli ölçüde etkilemektedir. Bu nedenle bu çalışmada, sevofluranın rat palatinal mukozasında yara iyileşmesi üzerindeki etkisinin histopatolojik, immünohistokimyasal ve klinik olarak değerlendirilmesi hedeflenmiştir. 30 adet Wistar Albino türü ratın palatal bölgesinden punch yardımıyla 4 mm çapında tam kalınlık doku çıkartılmıştır. Denekler kontrol, serum fizyolojik (SF) ve sevofluran olmak üzere 3 ana gruba ayrılmıştır. Çalışma protokolünün ilk basamağında cerrahi işlem tüm gruplara uygulanmıştır. SF grubuna NaCl %0.9 solüsyonu, sevofluran grubuna ise her gün 1 ml/cm² likit sevofluran uygulanmıştır. Kontrol grubuna herhangi bir deneysel ilaç uygulanmamıştır. Her grupta 5. ve 14. günlerde sakrifikasyon gerçekleştirilmiştir. Elde edilen örnekler, ışık mikroskobu ile histopatolojik olarak kolajen ve granülasyon doku miktarı, inflamasyon şiddeti, fibrozis ve fibroblast olgunlaşma düzeyi, anjiyogenez, epidermal rejenerasyon düzeyi ve yara ağız açıklığı seviyeleri; immünohistokimyasal olarak IL-17A, IL-22 ve pSTAT-3 değerleri; klinik olarak ise yara alanı yüzey ölçümleri ve iyileşme yüzdesi açısından değerlendirilmiştir. Araştırmada istatistiksel analiz normal dağılıma sahip ölçümler için varyans analizi ANOVA testi kullanılarak, normal dağılıma sahip olmayan ölçümler için ise Kruskal Wallis testi ile analiz edilmiştir. Sevofluran grubunda 5. günde anjiyogenez ve epidermal rejenerasyon düzeyinin diğer gruplara kıyasla istatistiksel olarak anlamlı düzeyde yüksek olduğu tespit edilirken (p<0,05), 14. günde ise anlamlı farklılık bulunmamıştır (p>0.05). Bu bulgularla uyumlu olarak 5. günde sevofluran grubunun yara ağız açıklığı değeri kontrol grubuna göre istatistiksel olarak daha düşük saptanmıştır (p<0,05). 5.günde sevofluran grubunun IL-17 değeri kontrol ve SF gruplarına göre anlamlı derecede daha düşük; IL-22 ve pSTAT3 değerleri, kontrol grubuna göre anlamlı olarak yüksek bulunmuştur (p<0,05). Klinik değerlendirmede 5. günde sevofluran grubunda, kontrol ve SF gruplarına göre yara alanları değeri anlamlı olarak daha düşük olduğu tespit edilmiştir (p<0,05). Buna paralel olarak 5. günde sevofluran grubunun yara alanlarının iyileşme yüzdesi değeri diğer gruplara göre daha fazla olduğu görülmüştür (p<0,05). Sevofluranın erken dönem sonuçlarında tespit ettiğimiz yüksek IL-22 seviyesinin ve buna paralel olarak gözlemlenen yüksek pSTAT3 seviyelerinin epidermal rejenerasyonu artırarak, yara ağız açıklığının daha düşük olmasını sağladığı düşünülmektedir. Ayrıca, kontrol ve SF gruplarına göre anjiyogenezis düzeyinin ve yara alanlarının iyileşme yüzdesinin anlamlı derecede yüksek bulunması, IL-17A seviyelerinin ve yara alanlarının istatistiksel olarak anlamlı derecede düşük olması sevofluranın erken dönem yara iyileşmesindeki etkinliğinin güçlü olduğunu göstermektedir. Sonuç olarak, verilerimiz sevofluranın topikal uygulanmasının, periodontal cerrahi sonrası yara iyileşmesinde güvenli bir alternatif olma potansiyeline sahip olduğunu göstermektedir.

Wound healing is a complex, dynamic process that restores the integrity and function of damaged tissue. Wounds formed after periodontal surgical procedures such as free gingival graft, laterally shifted flap, frenectomy, gingivectomy/gingivoplasty are usually closed by secondary healing. These wound areas in the mouth significantly affect the patient's comfort, nutrition and quality of life. Therefore, the aim of this study was to evaluate the effect of sevoflurane on wound healing in rat palatal mucosa histopathologically, immunohistochemically and clinically. 30 Wistar Albino rats were subjected to full-thickness tissue removal with a diameter of 4 mm from the palatal region with the help of a punch. The subjects were divided into 3 main groups as control, saline (SF) and sevoflurane. In the first step of the study protocol, surgical procedure was applied to all groups. NaCl 0.9% was administered to the SF group and 1 ml/cm² liquid sevoflurane was administered to the sevoflurane group every day. No experimental drug was administered to the control group. Each group was sacrificed on the 5th and 14th days. The samples obtained were evaluated histopathologically by light microscopy in terms of collagen and granulation tissue amount, inflammation severity, fibrosis and fibroblast maturation level, angiogenesis, epidermal regeneration level and wound mouth opening levels; immunohistochemically in terms of IL-17A, IL-22 and pStat-3 values; clinically in terms of wound surface measurements and healing percentage. Statistical analysis was performed using analysis of variance ANOVA test for normally distributed measurements and Kruskal Wallis test for non-normally distributed measurements. While angiogenesis and epidermal regeneration levels were found to be statistically significantly higher in the sevoflurane group on day 5 compared to the other groups (p<0.05), no significant difference was found on day 14 (p>0.05). In accordance with these findings, the wound mouth opening value of the sevoflurane group on day 5 was found to be statistically lower than the control group (p<0.05). On day 5, the IL-17 value of the sevoflurane group was significantly lower than the control and SF groups; IL-22 and pSTAT3 values were significantly higher than the control group (p<0.05). In the clinical evaluation, it was found that the wound area value was significantly lower in the sevoflurane group on day 5 compared to the control and SF groups (p<0.05). In parallel with this, it was observed that the percentage of healing of the wound areas of the sevoflurane group on day 5 was higher than the other groups (p<0.05). Although we examined the effect of sevoflurane in both time periods, it can be said that the high IL-22 level we detected in the early period results and the high pSTAT3 levels observed in parallel with this increased epidermal regeneration and caused the wound mouth opening to be lower. In addition, the significantly higher angiogenesis level and healing percentage of wound areas, and the statistically significant lower IL-17A levels and wound areas compared to the control and SF groups indicate that the efficacy of sevoflurane in early wound healing is strong. In conclusion, our data suggest that topical application of sevoflurane has the potential to be a safe alternative for wound healing after periodontal surgery.
