Özet
Giriş ve Amaç: Perioperatif bakımdaki gelişmelere rağmen, büyük ameliyatlardan sonra ağrı, organ disfonksiyonu ve uzun süreli iyileşme gibi yaygın sorunlar ve sonuçlar gelişebilir. Etkili analjezinin, daha az morbidite ve hastanede kalış süresiyle ilişkili olduğu ve cerrahi sonuçları iyileştirdiği konusunda bir fikir birliği vardır. Bu nedenle, optimum analjezi etkili postoperatif bakım için bir ön koşuldur. Bu çalışmanın amacı, jinekolojik kanser tanısı ile major abdominal cerrahi geçiren hastalarda USG rehberliğinde rektus kılıf bloğu (RKB) ile kombine transversus abdominis plan (TAP) bloğu veya quadratus lumborum bloğu (QLB) ile kombine TAP veya RKB /QLB olmaksızın TAP bloğun postoperatif ağrı yönetiminde etkinliğini karşılaştırmaktır. Hipotez: Kombine kullanılan fasyal alan blokları TAP bloğa göre daha düşük ağrı skorları ve daha az opioid tüketimine neden olur. Postoperatif daha iyi cerrahi sonuçlara katkı sağlar. Gereç ve Yöntem: Bu çalışmaya, Dr. Abdurrahman Yurtaslan Onkoloji Eğitim ve Araştırma Hastanesi Etik Kurulu tarafından onay alındı (Protokol No:2023- 15/536 Tarih: 20.12.2023). ASA I-IV olan, 18 yaş üstü ve jinekolojik onkoloji cerrahisi (endometrium, over tümörü, debulking cerrahisi) nedeniyle elektif major cerrahi yapılan ve genel anestezi uygulanan, sorumlu anestezist kararıyla postoperatif analjezi amacıyla abdominal fasyal düzlem bloğu uygulanmış olan 94 hasta çalışmaya dâhil edildi. Hastalar TAP blok uygulanan (27 hasta), TAP ve QLB kombine uygulanan (34 hasta) ve TAP ve RKB kombine uygulanan (33 hasta) olarak üç gruba ayrıldı. Hastaların preoperatif anestezi değerlendirme formundan bilgileri kaydedildi. Postoperatif 0.dk., 15. Dakika, 2. saat, 6. saat, 12. saat ve 24. saatlerde Ramsey Sedasyon skoru, kalp hızı ve kan basıncı kaydedildi. Bu ölçüm zamanlarında ağrı şiddeti vizüel ağrı skalası (VAS, 0= ağrı yok 10= en şiddetli ağrı) ile değerlendirildi. Hastaların postoperatif ek analjezik ihtiyacı (VAS skoru 4/10 üzerinde olan hastalar) analjeziğin türü ve miktarı, gaz-gaita çıkışı, mobilizasyon süresi, bulantı kusma olup olmadığı, ilk oral alım süreleri ve hastanede yatış süreleri kaydedildi. Bulgular: Jinekolojik onkoloji cerrahisi geçiren hastaların postoperatif analjezi yöntemlerine göre ASA skorları ve jinekolojik kanser türlerinin dağılımları arasında anlamlı farklılık görülmedi. (p>0,05). Toplam morfin tüketimi açısından TAP bloğu grubu, diğer iki gruba kıyasla anlamlı derecede daha yüksek dozda morfin ihtiyacı gösterdi (p<0,05). TAP bloğu grubunda hastaların morfin tüketim ortalaması 3,44 ± 1,42 mg, QLB+TAP bloğu grubunda 1,29 ± 1,55 mg, RKB+TAP bloğu grubunda ise 1,27 ± 1,1 mg olarak saptandı. TAP bloğu uygulanan hastalarda ağrı skoru zamanla daha belirgin şekilde yüksek olup ve bu grup diğer iki gruba göre daha yüksek VAS skorlarına sahipti. NSAID kullanılan hastaların oranı QLB+TAP ve RKB+TAP blok gruplarında sırasıyla %14,7 ve %9,1 iken, TAP bloğu grubunda bu oran %33,3 olarak daha düşük bulundu (p<0,05) Bulantı/kusma, QLB+TAP bloğu grubunda %23,5, RKB+TAP bloğu grubunda %18,2 iken, TAP bloğu grubunda %70,4 ile belirgin şekilde daha yüksekti (p<0,05). İlk gaz-gaita çıkış süresi, QLB+TAP ve RKB+TAP gruplarında benzerken (sırasıyla 20,53 ± 7,66 saat ve 19,39 ± 6,08 saat), TAP bloğu grubunda 30,96 ± 6,3 saat ile belirgin şekilde daha geç gerçekleşti (p<0,05). TAP bloğu grubunda hastalar ortalama 18,96 ± 4,55 saat sonra mobilize olurken, QLB+TAP ve RKB+TAP blok gruplarında bu süre sırasıyla 15,26 ± 4,08 saat ve 15 ± 4,2 saat olarak bulundu (p<0,05). Oral alım başlama süresi, TAP bloğu uygulanan grupta anlamlı şekilde daha uzun olup (16,78 ± 4,38 saat), QLB+TAP ve RKB+TAP blok gruplarında ise sırasıyla 13,85 ± 4,56 saat ve 12,79 ± 3,86 saat olarak tespit edildi(p<0,05). Ramsey Sedasyon Skorları karşılaştırmasında QLB+TAP ve RKB+TAP bloğu uygulanan hastalarda sedasyon seviyesinin TAP bloğu grubuna kıyasla daha uzun süre korunabildiğini, TAP bloğu uygulanan hastalarda ise erken dönemden itibaren sedasyon seviyesinde daha hızlı bir azalma olduğu görüldü. Tartışma: Bu bulgular, QLB+TAP ve RKB+TAP blok kombinasyonlarının postoperatif süreçte ağrı yönetimi, mobilizasyon, bağırsak fonksiyonlarının normale dönmesi ve opioid tüketiminin azaltılması açısından avantaj sağlayabileceğini göstermektedir. TAP bloğu uygulanan grupta daha yüksek morfin tüketimi, daha geç mobilizasyon ve daha uzun hastanede kalış süresi dikkat çekici bulunmuştur. QLB+TAP ve RKB+TAP bloklarının, TAP bloğuna kıyasla postoperatif dönemde daha etkili ağrı kontrolü sağladığını ve opioid ihtiyacını azaltabileceğini göstermektedir. Literatürde kombine bloklar ile yapılan az sayıda çalışmada da kombine blokların opioid tüketimini azalttığı gösterilmiş olmakla birlikte kombine blokların ERAS ile ilişkisine dair yeterli çalışma bulunmamaktadır. Sonuç: Çalışmamızın bulgularıyla, tek başına TAP bloğunun jinekolojik kanser cerrahisi sonrası postoperatif ağrı yönetiminde QLB+TAP veya RKB+TAP kombinasyonuna göre daha az etkili olduğunu ve analjezik etkinliğindeki sınırlılık nedeniyle daha fazla opioid kullanımı sonucu bazı olumsuz yan etkilerle (bulantı, kusma, mobilizasyon gecikmesi, gaz gaita çıkış süresinin uzaması, hastanede kalış süresinin artması gibi) daha fazla ilişkili olabileceğini gösterdik. Bu sonuçlar, kombine blok tekniklerinin multimodal analjezi stratejilerinin önemli bir parçası olduğunu ve ERAS kapsamında kombine blokların çalışılması gerektiğini göstermektedir

Introduction and Aim: Despite advancements in perioperative care, common postoperative complications such as pain, organ dysfunction, and prolonged recovery continue to pose challenges following major surgeries. Effective analgesia is widely acknowledged to be associated with reduced morbidity, shorter hospital stays, and improved surgical outcomes. Therefore, optimal analgesia is a prerequisite for effective postoperative care. This study aims to compare the effectiveness of ultrasound-guided rectus sheath block (RSB) combined with transversus abdominis plane (TAP) block or quadratus lumborum block (QLB) combined with TAP block against TAP block alone in postoperative pain management in patients undergoing major abdominal surgery for gynecologic cancer. Hypothesis: The combination of fascial plane blocks results in lower pain scores and reduced opioid consumption compared to TAP block alone, contributing to better postoperative surgical outcomes. Materials and Methods: This study was approved by the Ethics Committee of Dr. Abdurrahman Yurtaslan Oncology Training and Research Hospital (Protocol No: 2023-15/536, Date: 20.12.2023). A total of 94 patients, classified as ASA I-IV, aged over 18, who underwent elective major gynecologic oncology surgery (endometrial cancer, ovarian tumor, debulking surgery) under general anesthesia and received postoperative abdominal fascial plane block for pain management, were included in the study. Patients were divided into three groups based on their analgesic method: TAP block alone (27 patients), TAP and QLB combination (34 patients), and TAP and RSB combination (33 patients). Data were recorded from preoperative anesthesia evaluation forms and intraoperative monitoring sheets. Postoperatively, patients were monitored for pain scores using the Visual Analog Scale (VAS; 0 = no pain, 10 = worst pain) at 0 min, 15 min, 2 hr, 6 hr, 12 hr, and 24 hr. Additional analgesic requirements (VAS ≥ 4/10), analgesic type and dose, bowel movement, mobilization, nausea/vomiting, time to first oral intake, intensive care unit admission, and length of hospital stay were recorded. Results: There was no significant difference between the ASA scores and distribution of gynecological cancer types in patients undergoing gynecological cancer surgery according to postoperative analgesia methods (p>0.05). The TAP block group required a significantly higher dose of morphine compared to the other two groups (p<0.05). The mean morphine consumption of patients in the TAP block group was 3.44 ± 1.42 mg, 1.29 ± 1.55 mg in the QLB+TAP block group, and 1.27 ± 1.1 mg in the RKB+TAP block group. Pain scores were significantly higher over time in TAP block group, and this group had higher VAS scores than the other two groups. The proportion of patients using NSAIDs was 14.7% and 9.1% in the QLB+TAP and RKB+TAP block groups, respectively, while this rate was found to be 33.3% lower in the TAP block group (p<0.05). Nausea/vomiting was 23.5% in the QLB+TAP block group, 18.2% in the RKB+TAP block group, and significantly higher in the TAP block group (70.4%) (p<0.05). The time to first gas/stool exit was similar in the QLB+TAP and RKB+TAP groups (20.53 ± 7.66 hours and 19.39 ± 6.08 hours, respectively), while it was significantly longer in the TAP block group (30.96 ± 6.3 hours) (p<0.05). In the TAP block group, patients were mobilized after an average of 18.96 ± 4.55 hours, while in the QLB+TAP and RKB+TAP block groups this period was found to be 15.26 ± 4.08 hours and 15 ± 4.2 hours, respectively (p<0.05). The time to first oral intake was significantly longer in the TAP block group (16.78 ± 4.38 hours), and in the QLB+TAP and RKB+TAP block groups it was found to be 13.85 ± 4.56 hours and 12.79 ± 3.86 hours, respectively (p<0.05). In the comparison of Ramsey Sedation Scores, it was seen that the sedation level could be maintained longer in patients with QLB+TAP and RKB+TAP blocks compared to the TAP block group, while in patients with TAP blocks, a faster decrease in sedation level was observed from the early period. Discussion: These findings suggest that QLB+TAP and RSB+TAP block combinations may offer advantages in terms of postoperative pain management, earlier mobilization, restoration of bowel function, and reduction in opioid consumption. In the TAP block group, higher morphine consumption, delayed mobilization, and longer hospital stay were notably observed. Our results indicate that QLB+TAP and RSB+TAP blocks provide more effective postoperative analgesia compared to TAP block alone, potentially leading to reduced opioid requirements. Although a limited number of studies in the literature have shown that combined blocks can reduce opioid consumption, there is still a lack of sufficient research exploring the relationship between combined block techniques and ERAS (Enhanced Recovery After Surgery) protocols. Conclusion: Based on the findings of our study, we demonstrated that TAP block alone is less effective in postoperative pain management following gynecologic cancer surgery compared to the QLB+TAP or RSB+TAP combinations. Due to its limited analgesic efficacy, the use of TAP block alone may be associated with increased opioid consumption and, consequently, a higher incidence of adverse effects such as nausea, vomiting, delayed mobilization, prolonged time to first flatus and defecation, and extended hospital stay. These results highlight that combined block techniques are an important component of multimodal analgesia strategies and should be further investigated within the context of ERAS protocols.
