Özet

Giriş ve amaç: Meme kanseri, 2020 yılında dünya genelinde en sık teşhis edilen kanser türü olmuştur. Neoadjuvan kemoterapi (NAKT) lokal ileri evre meme kanseri olgularında, tercih edilen tedavi şeklidir. Fakat NAKT daha iyi bir cerrahi sonuç sağlaması ve prognoz öngörüsü gibi sahip olduğu birçok avantaj nedeniyle erken evrelerde belirli subtiplerde de tercih edilebilmektedir. NAKT sonrası radyolojik görüntüleme operasyon ve medikal tedavi planını yapmakta kilit noktayı oluşturur. Günümüzde NAKT yanıtını değerlendirmede meme manyetik rezonans görüntüleme (MRG) önemli bir yer tutmaktadır. NAKT sonrası gerçekleştirilen meme MRG'de hedef, bilinen malign kitle/kitlelerin tedaviye verdikleri yanıt anlamında boyut ölçümleriyle sınırlıdır. Bu nedenle tam protokol inceleme kemoterapi alan bu grup hasta için uzun sürmesi nedeniyle hem zahmetli ve hem de maliyetlidir. Bu çalışmanın amacı, meme kanseri hastalarında NAKT yanıtının değerlendirilmesinde kısa ve tam protokollü meme MRG'nin tanısal performansını birbirleriyle ve radyologların meme görüntüleme konusundaki uzmanlık düzeylerine göre karşılaştırmaktır. Gereç ve Yöntem: Ocak 2019-Ekim 2024 tarihleri arasında hastanemiz radyoloji kliniğinde NAKT sonrası çekilen meme MRG incelemeleri retrospektif olarak tarandı, 78 olgu çalışmaya dahil edildi. PACS sistemi üzerinden görüntüler incelenerek, lezyonların NAKT yanıtı 20 yıllık tecrübesi olan uzman meme radyoloğu ve 5 yıllık radyoloji asistanı olan iki gözlemci tarafından bağımsız olarak tam protokolde ve tam protokol sekanslarından elde edilerek oluşturulan kısa protokolde ayrı ayrı değerIendirildi. Veriler Pyton programı ile analiz edildi. Bulgular: 78 olgudan 18'i cerrahi sonrası patolojik değerlendirmede tam yanıt göstermiştir. Tam yanıt gösteren 18 hastanın 12'si uzman tarafından, 11'i ise asistan tarafından kısa protokolde radyolojik tam yanıt olarak değerlendirilmiştir; tam protokolde ise 12'si uzman ve 10'u asistan tarafından radyolojik tam yanıt olarak değerlendirilmiştir. 10 Ortalama rezidü tümör boyutu uzman ve asistan doktor tarafından kısa protokolde sırasıyla 12,85 mm ve 12,96 mm; tam protokolde ise 16,22 mm ve 14,35 mm ölçülmüştür. Operasyon sonrası patolojik spesmende ortalama boyut 12,22 mm'dir. Kısa protokol ve patoloji ölçümleri arasında uzman doktorun ortalama ölçüm farkı 0,63 mm (standart deviasyon: 17,08), asistan doktorun ise 0,74 mm (standart deviasyon 15.23) bulunmuştur. Tam yanıt değerlendirmesinde uzman doktorun tam protokolde sensitivitesi (%83,3), spesifitesi (%66,7) ve doğruluğu (%83,3) kısa protokol sonuçlarına göre daha yüksek bulunmuştur. Asistan doktorun ise kısa protokolde sensitivitesi (%91,7), spesifitesi (%61,1) ve doğruluğu (%84,6)'nun tam protokole göre daha yüksek olduğu görülmüştür. Kısa protokol değerlendirmesinde gözlemciler arası uyum orta düzeyde bulunmuştur. (κ = 0,49) Sonuç: MRG, NAKT sonrası patoloji ile en uyumlu sonuçlar veren modalite olsa da tedavi etkisine bağlı olarak lezyon değerlendirmesinde hatalı sonuçlara yol açabilir. Kısa protokol MRG'de geç sekansların değerlendirilmemesi, geç kontrastlanma gösteren fibrozisin yanıltıcı etkisini en aza indirerek, lezyon boyutunun tam protokolle karşılaştırıldığında patolojiye daha yakın ölçülmesini sağlamıştır. Araştırmacılardan asistan doktorun patolojik tam yanıt değerlendirmesinde sensitivite ve doğruluk seviyesi kısa protokolde daha iyi bulunmuştur. Uzman doktorda ise tam tersine tam protokol sonuçları daha iyi olsa da kısa protokol sonuçları tam protokole oldukça yakın bulunmuştur. Kısa protokollü MRG yalnızca tedavi sonunda cerrahi planlama için değil, medikal tedavi etkinliğini değerlendirmek için tedavi ortasında zaman kaybı yaşamadan ve hasta anksiyetisini minimumda tutarak, hızlı bir değerlendirme için tercih edilebilecek umut vadeden bir protokoldür.

Introduction and Aim: Breast cancer became the most commonly diagnosed cancer worldwide in 2020. Neoadjuvant chemotherapy (NACT) is the preferred treatment modality for locally advanced breast cancer cases. However, due to its numerous advantages, such as improving surgical outcomes and predicting prognosis, NACT is also increasingly being used in specific subtypes of early-stage breast cancer. Radiological imaging after NACT is crucial for surgical and medical treatment planning. Today, breast magnetic resonance imaging (MRI) holds a significant role in evaluating NACT response. Breast MRI after NACT primarily focuses on assessing the treatment response of known malignant lesions based on size measurements. However, full protocol breast MRI can be time-consuming and costly, making it challenging for this patient group. This study aims to compare the diagnostic performance of abbreviated and full protocol breast MRI in evaluating NACT response in breast cancer patients, considering the expertise levels of radiologists specialized in breast imaging. Materials and Methods: Breast MRI examinations performed after NACT at our hospital's radiology clinic between January 2019 and October 2024 were retrospectively reviewed. A total of 78 cases were included in the study. Images were evaluated via the PACS system, and the NACT response of lesions was independently assessed by two observers: a senior breast radiologist with 20 years of experience and a radiology resident with 5 years of experience. The evaluations were performed using both full protocol and abbreviated protocol (constructed from selected sequences of the full protocol). Data were analyzed using Python. Results: Of the 78 cases, 18 showed a complete pathological response upon postoperative pathological evaluation. Among these 18 patients, 12 were assessed as having a radiological complete response in the abbreviated protocol by the senior radiologist, while 11 were identified by the resident; in the full protocol, 12 were evaluated as complete responses by the senior radiologist and 10 by the resident. x The average residual tumor size measured in the abbreviated protocol was 12.85 mm and 12.96 mm by the senior radiologist and resident, respectively, while in the full protocol, the sizes were 16.22 mm and 14.35 mm, respectively. The average size in the postoperative pathological specimen was 12.22 mm. The average measurement discrepancy between the abbreviated protocol and pathology was 0.63 mm (standard deviation: 17.08) for the senior radiologist and 0.74 mm (standard deviation: 15.23) for the resident. For assessing complete response, the sensitivity (83.3%), specificity (66.7%), and accuracy (83.3%) of the senior radiologist using the full protocol were higher than those of the abbreviated protocol. Conversely, for the resident, the sensitivity (91.7%), specificity (61.1%), and accuracy (84.6%) of the abbreviated protocol were higher than the full protocol. Interobserver agreement in the abbreviated protocol was moderate (κ = 0.49). Conclusion: Although MRI provides results most consistent with pathology following NACT, it may yield inaccurate results in lesion evaluation due to treatment effects. In the abbreviated protocol, the misleading effect of delayed contrast enhancement in fibrosis was minimized by excluding late sequences, resulting in lesion size measurements closer to pathological findings compared to the full protocol. The sensitivity and accuracy levels of the resident in assessing complete pathological response were better in the abbreviated protocol, while the senior radiologist achieved higher diagnostic performance with the full protocol, although the abbreviated protocol results were close. Abbreviated breast MRI protocol is a promising option not only for surgical planning at the end of treatment but also for evaluating medical treatment efficacy mid-therapy, offering a quick assessment that minimizes time loss and patient anxiety
