Özet

Diş hekimliğinde, diş kayıplarının dental implantlar ve köprü protezleri ile tedavi edilebilmesi önemli bir role sahiptir. Eksik dişlerin yerine yerleştirilen dental implantlar, doğal dişlerin işlevsel ve estetik özelliklerini taklit ederek hastaların yaşam kalitesini artırır. Genellikle implant-implant bağlantılı veya dişlerden destek alınarak kantilever uzantılı sabit protezler yapılabildiği gibi, bazı durumlarda hem doğal dişlerin hem de implantların birlikte kullanıldığı diş-implant bağlantılı köprü protezler de alternatif bir çözüm olarak sunulabilir. Bu çalışmanın amacı, diş-implant bağlantılı köprü protezlerinin sonlu elemanlar analizi yöntemi kullanılarak incelenmesi ve çiğneme kuvvetlerinin bu yapılar üzerindeki etkilerinin detaylı bir şekilde belirlenmesi amaçlanmıştır. Bu çalışmada alt çenesi tam dişsiz ve alt çene posterior bölgesinde yeterli kemik hacmine sahip bir bilgisayarlı tomografi (BT) görüntüsünden özel bilgisayar yazılımları kullanılarak 5 farklı diş-implant bağlantılı model oluşturulmuştur. İlk model implant-implant, ikinci model implant-diş, üçüncü model diş-implant, dördüncü model implant-kemik yıkımı olan diş bağlantılı, beşinci model diş-kemik yıkımı olan implant bağlantılı modeldir. İmplantlar, (NeOss-Proactive® Straight®- 4 mm x 11 mm) ve dişler alt çenenin sağ posterior bölgesine (1. küçük azı ve 1. büyük azı diş bölgeleri) dik olarak yerleştirilmiştir. Üç üyeli metal altyapılı sabit protezlere dik ve oblik kuvvetler uygulanmıştır. Bulgular değerlendirildiğinde tüm modellerde en yüksek stres yoğunlaşmalarının implantların boyun bölgesinde ve implant çevresindeki kortikal kemiğin koronal bölgelerinde meydana geldiği görülmüştür. Kemik yıkımı simüle edilen modellerde oluşan stres yoğunluğu diğer modellere göre daha yüksek gözlenmiştir. Çalışmamızın diş-implant bağlantılı köprü protezlerin daha güvenilir, başarılı ve hasta dostu bir tedavi seçeneği düşünülmesine yönelik gelecekteki araştırmalara yol gösterebileceği düşünülmektedir.

In dentistry, the treatment of tooth loss with dental implants and bridges has an important role. Dental implants, which are placed in place of missing teeth, improve the quality of life of patients by mimicking the functional and aesthetic properties of natural teeth. Generally, fixed prosthesis with implant-implant connection or cantilever extension can be made with the support of teeth, but in some cases, tooth-implant connected bridge prosthesis using both natural teeth and implants can be offered as an alternative solution. The aim of this study is to investigate tooth-implant connected bridge prosthesis using finite element analysis method and to determine the effects of masticatory forces on these structures in detail. In this study, 5 different tooth-implant connected models were created using special computer software from a computed tomography (CT) image of a fully edentulous lower jaw with sufficient bone volume in the posterior region of the lower jaw. The first model is implant-implant, the second model is implant-tooth, the third model is tooth-implant, the fourth model is implant-tooth connected with bone destruction, and the fifth model is tooth-bone destroyed implant connected. Implants (NeOss- Proactive® Straight®- 4 mm x 11 mm) and teeth were placed vertically in the right posterior region of the lower jaw (1st premolar and 1st molar regions). Vertical and oblique forces were applied to the three-membered metal-based fixed prosthesis. The results showed that the highest stress concentrations occurred in the neck region of the implants and in the coronal regions of the cortical bone around the implants in all models. The stress concentrations in the models simulating bone destruction were higher than the other models. It is thought that our study may guide future research to consider tooth-implant connected bridge prosthesis as a more reliable, successful and patient-friendly treatment option.
