Özet

Amaç: Bu çalışmada robotik radikal prostatektomi (RRP) materyallerinde, yüksek dereceli prostatik adenokarsinomlarda histomorfolojik bulguların mikrosatellit instabilitesi (MSI) ve programlanmış ölüm ligandı-1 (PD-L1) ekspresyonunu öngörme potansiyeli ve bu biyobelirteçlerin prognostik değeri araştırılmıştır. Gereç ve Yöntem: 2014-2024 yılları arasında neoadjuvan tedavi almamış grade grup 4 ve 5 (GG4-5) 92 olgu retrospektif olarak incelenmiştir. Tüm olgulara MSI ve PD-L1(73-10 klonu) immünhistokimyası uygulanmıştır. PD-L1 için tümör proporsiyon skoru (TPS) ve immün hücre proporsiyon skoru (ICPS) hesaplanmış olup %1 ve üzeri pozitif kabul edilmiştir. Başta kribriform patern, tümörü infiltre eden lenfositler (TİL) olmak üzere birçok histopatolojik parametrenin MSI ve PD-L1 ile ilişkisi araştırılmıştır. Genel sağkalım (GS), biyokimyasal rekürrenssiz sağkalım (BRS) ve metastazsız sağkalım (MS) verileri analiz edilmiştir. Bulgular: Yanlış eşleşme tamir proteini ekspresyon kaybı (dMMR) olan olgular %2.2 (n=2) oranında olup tüm vakalarda MSH2 ve MSH6 ekspresyon kaybı saptanmıştır. Olguların %14.1' inde (n=13) PD-L1 TPS, %27.2' sinde (n=25) ICPS pozitifliği saptanmıştır. MSI ile yüksek TİL skoru (≥%50) ve PD-L1 TPS arasında istatistiksel olarak anlamlı ilişki bulunmuştur (sırasıyla p=0.005, p=0.019). PD-L1 TPS pozitifliği için bağımsız prediktif faktörler nadir histolojik patern/subtip varlığı (p=0.021), duktal adenokarsinom komponenti (p=0.042) ve yüksek TİL skoru (p=0.031) iken PD-L1 ICPS için yüksek TİL skoru (p=0.005) ve tümör hacmidir (p=0.016). Tümör hacmi ≥%18.5 olan olguların %80' inde PD-L1 ICPS pozitiftir. Sağkalım analizlerinde MSI, PD-L1 TPS veya ICPS pozitifliğinin GS, BRS, MS üzerinde anlamlı prognostik etkisi yoktur. Sonuç: Bulgularımız, yüksek dereceli prostatik adenokarsinomlarında MSI ve PD-L1 ekspresyonu için prediktif histomorfolojik bulguları ortaya koymaktadır. Yüksek TİL skoru hem MSI durumu hem de PD-L1 ekspresyonu ile anlamlı ilişki gösterirken, nadir histolojik patern/subtip varlığı ve duktal adenokarsinom komponenti PD-L1 TPS pozitifliği için; tümör hacmi PD-L1 ICPS pozitifliği için bağımsız prediktif faktör olarak bulunmuştur. Bu bulgular, immünoterapi potansiyeli olan hastaların belirlenmesinde histomorfolojik özelliklerin önemini göstermekte olup, daha geniş kohortlarda validasyon gerektirmektedir.

Aim: In this study, we investigated the potential of histomorphological findings to predict microsatellite instability (MSI) and programmed death ligand 1 (PD-L1) expression and the prognostic value of these biomarkers in high-grade prostate adenocarcinoma in robotic radical prostatectomy (RRP) specimens. Materials and Methods: Between 2014 and 2024, 92 patients with grade group 4 and 5 (GG4-5) disease who did not receive neoadjuvant treatment were retrospectively analyzed. MSI and PD-L1 (clone 73-10) immunohistochemistry was performed in all cases. Tumor proportion score (TPS) and immune cell proportion score (ICPS) were calculated for PD-L1, and 1% and above were considered positive. The relationship between MSI and PD-L1 and many histopathologic parameters, especially cribriform pattern and tumor infiltrating lymphocytes (TIL), was investigated. Overall survival (OS), biochemical recurrence-free survival (BRFS), and metastasis-free survival (MFS) data were analyzed. Results: The rate of cases with mismatch repair deficiency (dMMR) was 2.2% (n=2), and loss of MSH2 and MSH6 expression was found in all cases. PD-L1 TPS positivity was found in 14.1% (n=13) and ICPS positivity in 27.2% (n=25) of cases. A statistically significant correlation was found between MSI and high TIL score (≥50%) and PD-L1 TPS (p=0.005, p=0.019, respectively). Independent predictive factors for PD-L1 TPS positivity were presence of rare histologic pattern/subtype (p=0.021), ductal adenocarcinoma component (p=0.042), and high TIL score (p=0.031), whereas for PD-L1 ICPS, high TIL score (p=0.005) and tumor volume (p=0.016). PD-L1 ICPS was positive in 80% of cases with tumor volume ≥18.5%. In survival analyses, MSI, PD-L1 TPS, or ICPS positivity had no significant prognostic effect on GS, BRFS, or MFS. Conclusion: Our results reveal predictive histomorphologic findings for MSI and PD-L1 expression in high-grade prostatic adenocarcinoma. While high TIL score was significantly associated with both MSI status and PD-L1 expression, the presence of rare histologic pattern/subtype and ductal adenocarcinoma component were found to be independent predictive factors for PD-L1 TPS positivity, and tumor volume was found to be an independent predictive factor for PD-L1 ICPS positivity. These findings demonstrate the importance of histomorphologic features in identifying patients with immunotherapy potential and require validation in larger cohorts.
