Özet
Amaç: Bu çalışmanın amacı, premolar dişlerde hazırlanan feather-edge (F) ve butt-joint (B) preparasyon tasarımlarına sahip 0,4 mm kalınlığındaki laminate veneer restorasyonlarda çeşitli seramik materyallerinin marjinal uyum ve kırılma dayanımı üzerine etkisinin incelenmesidir. Yöntem: Çalışmamızda iki farklı preparasyon tasarımına [Feather-edge, (F); Butt-joint, (B)] sahip epoksi rezin güdük modeller hazırlandı (N=80) ve yazılım programında (Exocad GmBH) 0,4 mm kalınlığındaki laminate veneer restorasyonları tasarlandı. Her bir preparasyon grubu lityum disilikat ile güçlendirilmiş cam seramik [IPS e.max CAD, Ivoclar Vivadent (EmC); IPS e.max Press, Ivoclar Vivadent (EmP)], geliştirilmiş lityum disilikat seramik [Cerec Tessera, Dentsply Sirona (CT)] ve hibrit seramik [Vita Enamic, Vita Zahnfabrik (VE)] olmak üzere dört farklı materyal grubuna ayrılarak örnekler üretildi (n=10). Laminate veneer restorasyonlarının marjinal aralık (MA) değerleri (µm), silikon replika tekniği kullanılarak ölçüldü. Her bir örneğin silikon replika kaydından elde edilen tarama görüntülerinin dental yazılım programına (Exocad) aktarılmasının ardından belirlenen dört noktadan (servikal, okluzal, mezial, distal) ölçüm yapıldı.
Objective: To investigate the effects of various ceramic materials on fracture strength and marginal fit of 0.4 mm thick laminate veneer restorations with feather-edge (F) and butt-joint (B) preparation designs prepared on premolar teeth. Method: In our study, epoxy resin models with two different preparation designs [Feather-edge, (F); Butt-joint, (B)] were fabricated (N=80) and 0.4 mm thick laminate veneer restorations were designed in the software program (Exocad GmBH). Each preparation group was divided into four different material groups: lithium disilicate reinforced glass ceramic [IPS e.max CAD, Ivoclar Vivadent (EmC); IPS e.max Press, Ivoclar Vivadent (EmP)], advanced lithium disilicate ceramic [Cerec Tessera, Dentsply Sirona (CT)] and hybrid ceramic [Vita Enamic, Vita Zahnfabrik (VE)]. The marginal gap (MG) values (µm) of laminate veneer restorations were measured using the silicone replica technique. After transferring the scan images obtained from the silicone replica of each specimen to the dental software program (Exocad), measurements were made at four points (cervical, occlusal, mesial, distal). The specimens, cemented to the dies using dual cure resin cement (Variolink Esthetic DC, Ivoclar Vivadent) were subjected to 5,000 cycles of thermal cycling. Fracture test was performed on a universal testing machine (TCS, Test Control Systems) at 0.5 mm/min from the apex of the buccal cusp and fracture strength (FS) values (N) were recorded at the first fracture. Statistical analysis of marginal gap (MG) and fracture strength (FS) values were performed in SPSS 26 program (2-way ANOVA, independent t-test and Tukey Post Hoc; p<0.05). After the fracture test, the specimens were classified into 4 groups as Type I (chipping), Type II (crack), Type III (partial fracture) and Type IV (total fracture). Results: Different preparation designs and ceramic materials had a significant effect on MG and FS values (p<0.05). The F design showed significantly higher MG and lowest FS values than B design (p<0.05). The highest MG value was seen in group F-EmP (89.10 ± 1.45 µm) and the lowest MG value was seen in group B-VE (75.15 ± 1.03 µm). VE [B-VE (75.15 ± 1.03 µm), F-VE (82.17 ± 1.08 µm)] showed significantly lower MG values than other materials (p<0.05). The highest FS value was seen in the B CT (461.40 ± 58.15 N) group, and the lowest FS value was seen in the F-EmP (304.00 ± 34.61 N) group. In both preparation designs, significantly higher FS values were observed in the CT group compared to the EmP group (p<0.05), while no significant difference was found among the other materials (p>0.05). Conclusions: Different preparation designs and ceramic materials affect the marginal adaptation and fracture strength of laminate veneer restorations on premolars. Butt-joint preparation demonstrated superior marginal adaptation and fracture strengths with respect to feather-edge preparation. Pressed ceramics exhibited lower performance in both marginal adaptation and fracture strength than the CAD/CAM ceramics, regardless of the preparation design.
