Özet

Amaç: Genel anestezi altında elektif laparoskopik kolesistektomi geçiren hastaların postoperatif analjezisinde erektör spina plan bloğu (ESPB) ve modifiye torakoabdominal sinir bloğunun (M-TAPAB) etkinliğini karşılaştırarak günlük pratiğimize uygun olabilecek etkili bir analjezi yöntemi belirleyebilmek. Birincil amacımız ESPB veya M-TAPA bloğun postoperatif 1. saatteki ağrı skorlarını karşılaştırıp birbirlerine üstünlüklerini tanımlamak. Sekonder amacımız ise 24 saatteki total opioid tüketimini karşılaştırmak. Gereç ve Yöntem: Etik kurul onayı alındıktan sonra, Sağlık Bilimleri Üniversitesi Antalya Eğitim ve Araştırma Hastanesi'nde elektif laparoskopik kolesistektomi geçirecek 18-65 yaş arası, American Society of Anesthesiologists (ASA) skoru I-II-III olan, vücut kitle indeksi (VKİ) <35 kg/m2 olan 64 hasta dahil edildi. Çalışma gözlemsel olarak planlandı. Laparoskopik kolesistektomilerde ESP ve M-TAPA bloklarından önce hastalardan onam alındı ve çalışmaya dahil edildi. Tüm hastalara postoperatif analjezi amacıyla hasta kontrollü analjezi (HKA) başlandı. Hastaların demografik verileri (cinsiyet, yaş, ASA skoru, vücut kitle indeksi), intraoperatif hemodinamik verileri, intraoperatif ve postoperatif opioid tüketimi, postoperatif Numerik Rating Skala (NRS) skorları (1, 6., 12. ve 24. saatte), postoperatif bulantı-kusma (POBK) skorları, ek analjezik ihtiyacı, blok ile ilişkili olası komplikasyonlar, Quality of Recovery-15 skoru, kayıt altına alındı. Bulgular: Postoperatif ağrı skorları açısından ESPB ve M-TAPA bloğu grupları arasında fark yoktu. İntraoperatif remifentanil ve postoperatif morfin tüketimleri arasında anlamlı fark bulunmadı. Yine postoperatif kurtarma analjezisi olarak ketorolak gereksinimleri arasında fark görülmedi. ESPB grubunda POBK insidansı ve ondansetron ihtiyacı istatistiksel olarak daha düşük bulundu. Omuz/sırt ağrısı ESPB grubunda hiçbir hastada görülmedi, M-TAPA bloğu grubunda ise 13 hastada görüldü ve bu istatistiksel olarak anlamlı bulundu. Postoperatif QoR-15 anketinin sonuçları arasında fark yoktu. Çalışmaya alınan hiçbir hastada lokal anestezik toksisitesi, vasküler enjeksiyon, pnömotoraks, ilaç alerjisi gibi bir komplikasyon gelişmedi. Sonuç: Bu çalışmanın sonucunda laparoskopik kolesistektomilerden sonra multimodal analjezinin parçası olan ESP ve M-TAPA bloklarının postoperatif ağrı skorları ve opioid tüketimi açısından fark yaratmadığını bulduk. İki blok da laparoskopik kolesistektomi sonrası etkin postoperatif analjezi yöntemi olarak uygulanabilir.

Objectıve: The aim of this study is to compare the effectiveness of erector spinae plane block (ESPB) and modified thoracoabdominal nerve block (M-TAPA) in postoperative analgesia of patients undergoing elective laparoscopic cholecystectomy under general anesthesia, in order to determine an effective analgesic method suitable for daily clinical practice. Our primary objective is to compare the pain scores at the 1st postoperative hour between ESPB and M-TAPA blocks and to determine their superiority. The secondary objective is to compare the total opioid consumption within 24 hours. Materıals and Methods: After obtaining ethics committee approval, 64 patients aged between 18-65 years, with American Society of Anesthesiologists (ASA) scores of I-II-III and a body mass index (BMI) of <35 kg/m², scheduled for elective laparoscopic cholecystectomy at the University of Health Sciences Antalya Training and Research Hospital were included in the study. The study was designed as observational. Informed consent was obtained from all patients prior to the administration of ESP or M-TAPA blocks. Postoperative analgesia was provided using patient-controlled analgesia (PCA) for all patients. Data collected included demographic characteristics (gender, age, ASA score, BMI), intraoperative hemodynamic parameters, intraoperative and postoperative opioid consumption, postoperative Numeric Rating Scale (NRS) scores (at 1st, 6th, 12th, and 24th hours), postoperative nausea and vomiting (PONV) scores, need for additional analgesics, complications related to the blocks, and Quality of Recovery-15 (QoR-15) scores. Results: There was no significant difference in postoperative pain scores between the ESPB and M-TAPA block groups. No significant differences were found in intraoperative remifentanil or postoperative morphine consumption. Similarly, no difference was observed in the need for ketorolac as rescue analgesia. The incidence of PONV and the need for ondansetron were significantly lower in the ESPB group. Shoulder/back pain was not observed in any patient in the ESPB group, while it was observed in 13 patients in the M-TAPA group, which was statistically significant. No difference was found between the groups in terms of postoperative QoR-15 survey results. No complications such as local anesthetic toxicity, vascular injection, pneumothorax, or drug allergy were observed in any of the patients included in the study. Conclusıon: As a result of this study, we found that ESPB and M-TAPA blocks, as part of multimodal analgesia after laparoscopic cholecystectomy, did not show significant differences in postoperative pain scores or opioid consumption. Both blocks can be effectively used as postoperative analgesia techniques following laparoscopic cholecystectomy.
