Özet
Giriş ve Amaç: Obezite, giderek artan ve ciddi morbidite ve mortalite riski taşıyan kronik bir hastalıktır. Vücut Kitle İndeksi (VKİ) 30 kg/m² ve üzeri olan bireyler obez olarak tanımlanmakta olup, obeziteye bağlı olarak diyabet, kardiyovasküler hastalıklar ve hipertansiyon gibi sağlık sorunları ortaya çıkmaktadır. Obezite tedavisinde bariatrik cerrahi, sürekli kilo kaybı sağlamada en etkili yöntemlerden biri olarak kabul edilmektedir. Sleeve gastrektomi, mide hacmini küçülterek gıda alımını sınırlandıran ve metabolik faydalar sunan yaygın bir prosedürdür. Ancak ameliyat başarısı, yalnızca cerrahi işlemin kendisiyle değil, hastaların preoperatif parametreleri ve cerrahi öncesi verilen eğitime uyumlarıyla da ilişkilidir. Bu çalışmanın amacı, sleeve gastrektomi uygulanan hastalarda preoperatif hasta parametreleri ve preoperatif eğitime uyumun postoperatif kilo kaybı üzerindeki etkisini değerlendirmektir. Materyal ve Metod: Bu retrospektif çalışma, Aralık 2019-Aralık 2023 tarihleri arasında Ankara Bilkent Şehir Hastanesi Obezite Merkezi'nde sleeve gastrektomi ameliyatı geçiren 113 hastayı kapsamaktadır. Dahil edilme kriterleri olarak; 18 yaş ve üzeri hastalar, sleeve gastrektomi ameliyatı geçirenler ve takip sürecini tamamlayan bireyler seçildi. Diğer bariatrik cerrahi prosedürleri uygulanan hastalar ve revizyon cerrahisi geçiren hastalar dışlama kriteri olarak belirlenmiştir. Değerlendirme parametreleri arasında yaş, cinsiyet, preoperatif ve postoperatif kilo, vücut kitle indeksi, bel/kalça oranı, kan basıncı, HbA1c ve lipid profili gibi veriler bulunmaktadır. Verilerin analizi IBM SPSS Statistics 21.0 programı kullanılarak yapılmış olup, istatistiksel anlamlılık düzeyi p<0.05 olarak kabul edilmiştir. Bulgular: Hastaların yaş ortalaması 42.22±11.55 yıl olup, %83.2'si kadın, %16.8'i erkektir. Hastaların %29.2'sinde diyabet, %18.6'sında hipertansiyon gibi ek hastalıklar tespit edilmiştir. Erkek hastaların postop 3. ay kilo kaybı ortalaması - 28.37±12.48 kg, kadın hastaların ise -22.45±8.85 kg bulunmuştur (p=0.035). Postop 12. ayda toplam kilo kaybı erkeklerde -42.32±15.81 kg, kadınlarda -35.30±12.69 kg'dır (p=0.020). Preoperatif 3 ay total ve fazla kilo kaybı ile postoperatif 3.ay ve postoperatif 12.ay fazla kilo kaybı arasında orta düzeyde, pozitif yönlü istatistiksel olarak anlamlı bir ilişki tespit edilmiştir (p<0.005). Preoperatif 3 ay kilo kaybı arttıkça postoperatif 3.ay ve 12.ay kilo kaybı değerinin arttığı görülmektedir. Sonuç: Bu çalışma, sleeve gastrektomi geçiren hastalarda preoperatif hasta parametreleri ve preoperatif eğitime uyumun postoperatif kilo kaybı üzerinde önemli bir etkisi olduğunu göstermiştir. Erkek hastaların kadınlara göre daha fazla kilo kaybı yaşadığı tespit edilmiştir. Preoperatif eğitim ve hasta uyumunun ameliyat sonrası kilo kaybını desteklediği saptanmıştır. Elde edilen bulgular, obezite cerrahisi uygulamalarında hasta seçim kriterleri ve preoperatif eğitim süreçlerine rehberlik edebilecek niteliktedir.

Introduction and Aim: Obesity is a chronic disease that is increasingly prevalent and poses a significant risk of morbidity and mortality. Individuals with a Body Mass Index (BMI) of 30 kg/m² or higher are classified as obese, and obesity is associated with health issues such as diabetes, cardiovascular diseases, and hypertension. Bariatric surgery is considered one of the most effective methods for achieving sustained weight loss. Sleeve gastrectomy is a commonly performed procedure that reduces stomach volume, thereby limiting food intake and providing metabolic benefits. However, surgical success is not solely dependent on the procedure itself but also on patients preoperative parameters and adherence to preoperative education. The aim of this study is to evaluate the effects of preoperative patient parameters and compliance with preoperative education on postoperative weight loss in patients undergoing sleeve gastrectomy. Materials and Methods: This retrospective study includes 113 patients who underwent sleeve gastrectomy at Ankara Bilkent City Hospital Obesity Center between December 2019 and December 2023. Inclusion criteria included patients aged 18 years and older who underwent sleeve gastrectomy and completed the follow-up process. Patients who underwent other bariatric surgical procedures or revision surgery were excluded from the study. The evaluation parameters included age, gender, preoperative and postoperative weight, BMI, waist-to-hip ratio, blood pressure, HbA1c, and lipid profile. Data analysis was performed using IBM SPSS Statistics 21.0, and statistical significance was set at p<0.05. Results: The mean age of the patients was 42.22±11.55 years, with 83.2% being female and 16.8% male. Comorbid conditions such as diabetes (29.2%) and hypertension (18.6%) were identified. The average weight loss at postoperative 3 months was -28.37±12.48 kg for male patients and -22.45±8.85 kg for female patients (p=0.035). The total weight loss at postoperative 12 months was - 42.32±15.81 kg for males and -35.30±12.69 kg for females (p=0.020). A moderate, positive, and statistically significant relationship was found between total and excess weight loss in the preoperative 3 months and excess weight loss at postoperative 3 and 12 months (p<0.005). The results indicate that as preoperative 3 month weight loss increases, postoperative 3-month and 12-month weight loss also increases. Conclusion: This study demonstrates that preoperative patient parameters and compliance with preoperative education have a significant impact on postoperative weight loss in patients undergoing sleeve gastrectomy. Male patients experienced greater weight loss than females. Furthermore, adherence to preoperative education and preparation was found to support postoperative weight loss. These findings provide guidance for patient selection criteria and preoperative education processes in bariatric surgery applications.
