İlk renal prezentasyonda hemodiyaliz ihtiyacının olmaması (OR:0,12; GA:0,019-0,76; p=0.024) ve düşük düzey proteinüri varlığı (OR:0.88; GA: 0,787-0,983; p=0.024) tam tedavi yanıtının 12. aydan önce gelişeceğinin öngördürücüleri olarak belirlendi. Yüksek hasar skoru (OR:1,469; GA: 1,03-2,095; p=0.034), renal patolojide ağır wireloop bulguları (OR: 13,642; GA: 2,157-86,26; p=0.005), yüksek kronisite indeksi (OR: 1,969; GA:1,089-3,558; p=0.025) ve hastaneye yatış gerektiren infeksiyon öyküsü (OR: 15,653; GA:2,666-91,904; p=0.002) mortalite için öngördürücüler olarak belirlendi.

Immunosuppressive Treatment Responses, Relapse Rates, and Renal Prognosis in the Lupus Nephritis Cohort of Istanbul Medical Faculty of Rheumatology Department Introduction: Systemic lupus erythematosus (SLE) is an autoimmune disease that affects many organs and systems. Lupus nephritis (LN) is one of the most common and severe manifestations, causing significant morbidity and mortality. In the management of LN, the primary goals are to control disease activity, prevent relapses, and avoid damage. In this context, recognizing early predictors of renal response, relapse, and renal damage is crucial in the management of LN patients. Objective: We aimed to compare the clinical, laboratory, and histopathological characteristics, induction and maintenance therapies, and treatment responses of patients diagnosed with biopsy-proven LN followed by the Istanbul Medical Faculty of Rheumatology Department, according to nephritis class, and also determine the predictors of relapse, damage, and remission. Patients and Methods: A total of 1500 SLE patients, who met the 2019 EULAR/ACR criteria, followed between 1988 and 2023 were evaluated. 264 LN patients diagnosed according to the 2003 ISN/RPS criteria and analyzed by the Pathology Department of Istanbul Medical Faculty were included in the study. Results: When comparing proliferative LN with membranous LN, the initial eGFR was lower in proliferative LN (83.1±38.8 ml/min/1.73m² vs. 114.5±23.5 ml/min/1.73m²; p<0.001), SLEDAI (17.7±4.6 vs. 13.6±6.1; p<0.001), renal SLEDAI (10.6±2.5 vs. 8.6±3.6; p<0.001), and the activity index in renal pathology (9.1±3.8 vs. 0.7±0.9; p<0.001), although there was no difference in treatment response times between the two groups. No difference was found in the relapse frequency, chronic kidney disease (CKD), and the development of damage between the groups. When cumulative patient survival and renal survival were assessed, no difference was found between proliferative and membranous LN. This indicates that membranous LN can have a severe course similar to proliferative LN. When patients with class 3, 4, 5, and combined LN were evaluated, low eGFR at the initial renal involvement (OR:0.978; CI:0.961–0.996; p=0.016) and high-dose cyclophosphamide treatment during induction (OR:1.422; CI:1.184–1.707; p<0.001) were identified as 4 independent factors for achieving complete remission according to DORIS criteria. In proliferative LN patients (p<0.001), those with SLEDAI ≥12 at initial renal involvement (p=0.046) and those receiving cyclophosphamide treatment (p=0.001) reached DORIS complete remission later. The absence of the need for hemodialysis at first renal presentation (OR: 0.12; CI: 0.019–0.76; p = 0.024) and the presence of low-level proteinuria (OR:0.88; CI:0.787–0.983; p=0.024) were identified as predictors of achieving complete treatment response before the 12th month. Relapses were more frequent under azathioprine treatment compared to mycophenolate mofetil (44.4% vs 26.4%; p<0.001). Younger age at SLE diagnosis (OR:0.941; CI:0.911–0.971; p<0.001) and failure to achieve DORIS complete remission (OR:0.099; CI:0.013–0.752; p=0.025) were found to be risk factors for relapse. Class 4 LN (OR:6.122; CI:1.301–28.803; p=0.022) and increased hospitalizations (OR:2.741; CI:1.235–6.081; p=0.013) were risk factors for renal damage, while achieving complete response before 6 months (OR:0.133; CI:0.033–0.541; p=0.005) was associated with a lower risk of renal damage. Twenty patients in the cohort died, with the most common causes of death being infection, followed by cardiovascular disease. High damage scores (OR:1.469; CI:1.03–2.095; p=0.034), severe wireloop findings in renal pathology (OR:13.642; CI:2.157–86.26; p=0.005), high chronicity index (OR:1.969; CI:1.089–3.558; p=0.025), and a history of infection requiring hospitalization (OR:15.653; CI:2.666–91.904; p=0.002) were identified as predictors of mortality. In survival analyses, shorter survival times were found in patients with associated antiphospholipid syndrome (p=0.032) and those with a history of infection (p = 0.016). There was no difference in survival based on the presence or absence of relapse and damage. Conclusion: Our study shows that, contrary to the general belief, membranous LN can have a course as severe as proliferative LN. In relapsed patients, CKD, infections, and hospitalizations were more frequent, and damage scores were higher. High damage scores were identified as a risk factor for mortality. These findings highlight the importance of preventing relapse and damage in LN
