Özet
Giriş: Sarkomlar, erişkin malignitelerinin %1'ini içeren, 100'den fazla histolojik alt tipe sahip mezenkimal kökenli tümörler olup yumuşak doku sarkomları (YDS) ve kemik sarkomları olarak iki ana alt grupta incelenirler. Her alt tiptte altta yatan moleküler ve genetik değişiklikler, hastalığın biyolojisi ve seyri, tedavi yanıtları birbirinden farklıdır. Özellikle son 10 yılda sarkom tanı ve tedavi algoritmalarında yer alan yeni nesil dizileme (NGS) testi sayesinde sarkomlarda hem diagnostik, hem prognostik hemde tedaviyi yönlendirici bilgiler giderek artmaktadır. Bu çalışmada YDS ve kemik sarkomu tanılı hastaların kapsamlı NGS yöntemi ile genomik profillerini incelemeyi amaçladık. Yöntem: Acıbadem Sağlık Grubu hastanelerinde takip ve tedavileri yapılan YDS ve kemik sarkomu tanısı alan 81 hastanın 4 ayrı ticari kit ile yapılmış kapsamlı NGS sonuçları retrospektif olarak değerlendirilmiştir. Elektronik bilgi sistemi kullanılarak, hastaların yaş, cinsiyet, tanı anındaki yaş gibi demografik veriler ve buna ek olarak histopatolojik tanı, tümör mutasyon yükü (TMB), mikrosatellit instabilite (MSI) gibi klinikopatolojik verileri incelenmiştir. NGS test tipi, saptanan toplam mutasyon sayısı, genomik değişiklik tipleri, hedeflenebilir mutasyonlar ve ilgili FDA onaylı tedavi seçenekleri kaydedilmiştir. Önemi belirsiz varyanlar çalışmaya dahil edilmemiştir. Bulgular: Olguların %51,8'i kadın, %48,2'si erkekti ve yaş aralığı 18-80 olan hastaların yaş ortalaması 45,26±15,68 idi. Toplam 61 hasta (%75,3) YDS tanılı idi ve en sık saptanan 3 YDS tip; indiferansiye pleomorfik sarkom (İPS) (%22,7), leiomyosarkom (%16) ve sinoviyal sarkom (%11,1) idi. Kemik sarkomu tanılı hasta sayısı ise 20 (%24,7) olup, en sık saptanan iki kemik sarkomu tanıları Ewing sarkomu (%13,6) ve osteosarkomdu (%7,4). Toplam 4 çeşit NGS testi kullanılarak 81 hastada 223 adet genomik değişiklik saptandı ve hasta başına düşen ortalama genomik değişiklik sayısı 2,74 (0-9) olarak hesaplandı. En az bir genomik değişiklik saptanan hasta sayısı 73 (%90,1) idi. Hedeflenebilir mutasyon saptanan ve FDA onaylı hedefe yönelik tedavi önerisi olan 18 (%22,2) hasta tespit edildi. En sık saptanan mutasyonlar TP53 (%38, n= 31), RB1 (%22, n= 18) ve CDKN2A (%14, n= 12 ) genlerinde idi. Genomik stabilite regülasyonu yolağında TP53 (%38) ve MDM2 (%8) geninde, hücre siklus regülasyonu yolağında RB1(%22), CDKN2A (%14), CDKN2B (%9), CDK4 (%5) ve CDKN2A/B (%1) geninde, DNA onarım yolağında RAD (%1) geninde, fosfoinozitid-3 kinaz yolağında PTEN (%8), PIK3CA (%4), mTOR (%1), ve RICTOR (%2) geninde, reseptör tirozin kinaz yolağında ALK (%2) ve FGFR (%4) geninde potansiyel hedeflenebilir genomik değişiklikler saptandı. En sık saptanan genomik değişiklikler kopya sayısı amplifikasyonu (%26,9, n= 60), kopya sayısı kaybı (%24,7, n= 55) ve kromozomal rearanjmanlar (%12,6, n= 28) idi. MSI verisine ulaşılabilen 79 hastanın tümü mikrosatellit stabildi. TMB verisine ulaşılabilen 78 (%96,3) hastanın ortalama TMB yükü 2,56±2,31 mut/Mb (0-12,1 mut/Mb) olup, TMB yükü orta seviye saptanan (12,1 mut/Mb) bir İPS tanılı hasta mevcuttu. İki olguda germline BLM ve TP53 mutasyonları saptanması üzerine olgular takip sürecinde Bloom sendormu ve Li-Fraumeni sendromu tanısı aldı. Sonuç: Çalışmamız NGS yöntemi ile YDS ve kemik sarkomu tanılı hastalarda saptanan mutasyonları göstermiş olup, genomik profilleme testlerinin sarkomların tanı, tedavi ve prognozuna sağlayabileceği potansiyel katkısı gösterilmiştir.

Introduction: Sarcomas which are classified as soft tissue sarcomas (STS) and bone sarcomas are mesenchymal tumors with more than 100 histological subtypes. With an incidence of 3.4/100.000, despite conventional chemoradiotherapy and advanced surgical techniques, the 5-year survival rate of metastatic disease of these rare cancers is %16. As the usage of next generation sequencing (NGS) testing for the diagnostic and therapeutic algorithm of sacromas has increased especially within the last decade, identification of known mutations, detection of novel mutations and trials for new targeted therapies has become possible. In this study, we aimed to investigate the mutation profiles and targeted therapy options of sarcoma patients who were tested by NGS. Method: A total of 81 patients diagnosed with STS and bone sarcoma who underwent NGS testing at Acıbadem Health Group hospitals were included in our retrospective and multicenter study. Demographic data such as age, gender, and age at diagnosis, as well as histopathological diagnosis, tumor mutation burden (TMB), and microsatellite instability (MSI) data was investigated from patients' electronic health records. The type of NGS test, total number of genomic alterations, types of alterations, targetable mutations, and relevant FDA-approved targeted therapy options were recorded. Variants of uncertain significance were excluded from the study. Results: The median age of the patients was 45.26 (range 18-90) , 51.8% of the cases were female while 48.2% were male. A total of 61 (75.3%) patients were diagnosed with STS and the most frequent diagnoses were undifferentiated pleomorphic sarcoma (UPS) (22.7%), leiomyosarcoma (16%), and synovial sarcoma (11.1%). A total of 20 (24.7%) patients were diagnosed with bone sarcoma and the most frequent diagnoses were Ewing sarcoma (13.6%) and osteosarcoma (7.4%). The total number of genomic alterations detected was 223 in 81 patients using 4 different NGS test kits and the average number of genomic alterations per patient was calculated as 2.74 (0-9). At least one genomic alteration was detected in 73 (90.1%) patients while FDA-approved targeted therapy options were available in 18 (22.2%) patients. The most frequent genomic alterations were detected in TP53 (38%, n=31), RB1 (22%, n=18), and CDKN2A (14%, n=12) genes. Potentially targetable genomic alterations were detected in regulators of genomic stability TP53 (38%) and MDM2 (8%), regulators of cell cycle RB1 (22%), CDKN2A (14%), CDKN2B (9%), CDK4 (5%), and CDKN2A/B (1%), DNA repair genes RAD (1%), genes in the phosphoinositide-3 kinase pathway PTEN (8%), PIK3CA (4%), mTOR (1%), and RICTOR (2%) and receptor tyrosine kinases such as ALK (2%) and FGFR (4%). The most frequent type of genomic alteration detected were copy number amplifications (26.9%, n=60), copy number losses (24.7%, n=55), and chromosomal rearrangements (12.6%, n=28). All of the 79 patients with MSI data available were microsatellite stable. TMB data was available for 78 patients and the average TMB was 2.56 ± 2.31 mut/Mb (0-12.1 mut/Mb). One patient diagnosed with UPS had intermediate TMB (12.1 mut/Mb). Germline BLM and TP53 mutations were detected in two patients who were later diagnosed with Bloom syndrome and Li-Fraumeni syndrome after further investigation. Conclusion: We have demonstrated the genomic alterations detected in patients with STS and bone sarcomas using NGS profiling. Our study shows the potential contribution of genomic profiling tests in the diagnosis, prognosis and treatment of sarcoma.
