Özet
Amaç: Korozif özofajit, dünya pediyatrik popülasyonunda önlenebilir morbidite ve mortalite nedenlerinden biridir. Korozif maddeler ile yaralanmaların yaklaşık %50-80'i pediatrik yaş grubunda karşımıza çıkmaktadır. Ağız yoluyla alınan korozif ajanlar özofagus mukozasında nekroz, ülserasyon, inflamasyon ve ilerleyen dönemde fibrozis ile karakterize darlık oluşumuna neden olur. Klinik uygulamada kullanılan mevcut tedavi seçenekleri arasında steroidler, antibiyotikler, dilatasyon gibi yöntemler yer almaktadır. Çalışmamızda sıçan modelinde oluşturulan korozif özofajit tedavisinde, mezenkimal kök hücre (MKH) tedavisinin etkinliğini göstermeyi amaçladık. Gereç ve Yöntem: 30 erkek sıçan randomize olarak 3 gruba ayrılmıştır. Sham, dışındaki gruplara %15 NaOH ile korozif özofajit oluşturup tedavi grubuna 2 doz mezenkimal kök hücre tedavisi verilmiştir; Postoperatif birinci gün ve yedinci gün hWJ-MKH 1×10⁶ hücre/mL olacak şekilde hazırlanan hücre süspansiyonları M grubu deneklere intraperitoneal yoldan 1cc olarak uygulandı. Birinci hafta ve deney sonunda alınan serum örneklerinden ELISA ile sitokinler (TNF-α, IL-1Ra, IL-1β, IL-6, IL-10) ölçülmüştür. Histopatolojik inceleme ile inflamasyon ve fibrozis değerlendirilmiştir. Değişkenlerin istatistiksel dağılımı Kolmogorov Simirnov ve Shapiro-Wilk testi ile ölçülerek nicel bağımsız verilerin analizinde Kruskal-Wallis H ve Mann-Whitney U testleri kullanılmıştır. Bağımlı veriler Wilcoxon işaretli sıralar testi ile karşılaştırılmıştır. Bulgular: Çalışmamızda doku hasarını takiben birinci haftada elde edilen verilere göre, IL-1β düzeyleri sham grubuna kıyasla hem kontrol hem de tedavi grubunda istatistiksel olarak anlamlı şekilde yüksek bulunmuştur. Ancak TNF-α ve IL-6 düzeylerinde benzer bir artış gözlenmemiştir. Üçüncü haftada ise IL-1Ra/IL-1β oranı tedavi ve sham gruplarında kontrol grubuna kıyasla anlamlı derecede yüksek saptanmıştır. Ayrıca, tedavi ve sham gruplarında TNF-α, IL-1β ve IL-6 düzeyleri birinci haftaya göre üçüncü haftada anlamlı olarak azalmışken, kontrol grubunda bu sitokinlerde anlamlı bir değişiklik izlenmemiştir. Sonuç: Bu çalışma mezenkimal kök hücrelerin özofagus dokusundaki enflamatuvar yanıtı ve fibrozisi azaltabileceğini destekleyen veriler sunmuştur. Kontrol grubunda IL-1β artışının uzun vadeli devamı inflamatuar fazın devam ettiğine işaret edebileceğini düşündürmektedir. Kronik dönemde kontrol grubuna kıyasla tedavi grubunda IL-1Ra/IL-1β oranın anlamlı yüksek olması mezenkimal kök hücrelerin enflamatuvar süreci dengeleyici etkisini düşündürmektedir. Bu çalışma MKH tedavisinin korozif özofajit modelinde iyileşme sürecine katkı sağlayabileceği sonucunu desteklemektedir. Mezenkimal kök hücrelerin doku hasarını sınırlayıcı ve iyileşmeyi destekleyici etkileri, daha ileri düzeyde araştırmaları teşvik edici niteliktedir.

Aim: In our study, we aimed to demonstrate the efficacy of mesenchymal stem cells in the treatment of corrosive esophagitis in rats. This study aimed to demonstrate the potential of mesenchymal stem cell-based therapies for corrosive esophagitis and to contribute to the literature. Materials and Methods: Thirty male rats were randomly divided into three groups. Corrosive esophagitis was induced in all groups except the sham group, and the treatment group received two doses of mesenchymal stem cell therapy. On the first and seventh postoperative days, cell suspensions prepared at a concentration of hWJ-MKH 1×10⁶ cells/mL were administered intraperitoneally at 1 cc to the M group subjects. Serum samples collected on the first week and at the end of the experiment were analyzed by ELISA to measure cytokines. Fibrosis was evaluated by histopathological examination. The statistical distribution of variables was measured using the Kolmogorov-Smirnov and Shapiro-Wilk tests, and the Kruskal-Wallis H and Mann-Whitney U tests were used to analyze quantitative independent data. Dependent data were compared using the Wilcoxon signed-rank test. Results: According to the data obtained in the first week following tissue damage in our study, IL-1β levels were found to be statistically significantly higher in both the control and treatment groups compared to the sham group. However, no similar increases were observed in TNF-α and IL-6 levels. In the third week, the IL-1Ra/IL-1β ratio was found to be significantly higher in both the treatment and sham groups compared to the control group. Additionally, TNF-α, IL-1β, and IL-6 levels decreased significantly in the treatment and sham groups compared to the first week, while no significant changes were observed in these cytokines in the control group. Conclusion: This study has provided data supporting the potential of mesenchymal stem cells to reduce inflammatory response and fibrosis in esophageal tissue. The prolonged increase in IL-1β in the control group suggests that the inflammatory phase may persist. In the chronic phase, the significantly higher IL-1Ra/IL-1β ratio in the treatment group compared to the control group suggests that mesenchymal stem cells have a balancing effect on the inflammatory process. This study supports the conclusion that mesenchymal stem cell therapy may contribute to the healing process in a corrosive esophagitis model. The tissue-protective and healing-promoting effects of mesenchymal stem cells are encouraging for further research.
