Özet
Amaç: Opioidsiz anestezi, son dönemde sıklıkla araştırılan bir yöntem olmakla birlikte, kalp cerrahisinde prospektif veriler sınırlıdır. Çalışmamızda, bölgesel anestezi ile kombine edilen kardiyopulmoner baypas ile açık kalp cerrahisi geçiren hastalarda; opioidsiz anestezinin postoperatif ağrı skorları, derlenme ölçütleri ve intraoperatif hemodinamik parametreler üzerindeki etkileri klasik opioid anestezisi ile karşılaştırılmıştır. Gereç ve Yöntem: Prospektif, gözlemsel ve kohort tasarıma sahip çalışma, yerel etik kurul onayı (TABED 1-24-95, 03.04.2024) ve ClinicalTrials (NCT06683911) kaydı sonrası, Sağlık Bakanlığı Ankara Bilkent Şehir Hastanesi Kalp ve Damar Cerrahisi Ameliyathanesinde bilgilendirilmiş onamı alınan hastalarla yürütülmüştür. Açık kalp cerrahisi geçiren hastalar anestezi yöntemlerine göre iki gruba ayrılmıştır. İntraoperatif dönemde fentanil, remifentanil kullanılan 49 hasta opioid anestezisi grubuna; deksmedetomidin, lidokain, magnezyum kullanılan 48 hasta opioidsiz anestezi grubuna dahil edilmiştir. Hastaların preoperatif ve intraoperatif verileri kaydedilmiş, postoperatif veriler 3., 6., 12. ve 24. saatlerde ve ekstübasyonun 24. saatinde QoR-15 değerlendirmesi ile toplanmıştır. İstatistiksel analizler SPSS 25.0 programı ile yapılmıştır. Gruplar arası karşılaştırmalarda; sürekli değişkenler için bağımsız örneklem t testi veya mann-whitney U testi, kategorik veriler için ki-kare veya fisher's exact testi kullanılmıştır. Tekrarlayan ölçümler varyans analizi ile, sferisite varsayımı mauchly testi ile değerlendirilmiştir. p<0.05 değeri istatistiksel olarak anlamlı kabul edilmiştir. Bulgular: Hastaların preoperatif ve operasyonlara ait verileri benzerdi. Bradikardi insidansı gruplar arasında benzerken, tekrarlayan ölçüm analizlerinde opioidsiz grupta kalp atım hızları daha düşük ölçülmüştür (p=0,002). Taşikardi insidansı ise opioid grubunda daha yüksekti. (χ²=3.911, p=0.048). İntraoperatif efedrin kullanımı opioidsiz grupta daha yüksek bulunsa da (p=0.030) diğer hemodinamik müdahaleler ve tekrarlayan ölçüm analizlerine göre ortalama arter basıncı, BIS ve kan şekeri düzeyleri gruplar arası benzerdi. Postoperatif ağrı skorları 3., 6. ve 24. saatlerde opioid grubunda anlamlı şekilde daha yüksek saptandı (sırasıyla p=0.005, p=0.003, p=0.001). QoR-15 skoru opioidsiz grupta daha yüksekti (p=0.0018). Ek analjezik kullanımı ve diğer postoperatif derlenme ölçütleri gruplar arasında benzerdi. Sonuç: Kalp cerrahisinde opioidsiz anestezinin, intraoperatif dönemde opioid anestezisine benzer güvenle uygulanabildiği ve daha iyi postoperatif ağrı kontrolü ile postoperatif derlenme üzerinde olumlu etki sağladığı görülmüştür. Opioidsiz anestezinin kalp cerrahisi sonrası hızlandırılmış iyileşme protokollerine entegre edilmesiyle postoperatif derlenmede daha etkili klinik sonuçlar elde edilebileceği kanısındayız.

Objective: Although opioid-free anesthesia has been frequently investigated in recent years, prospective data in cardiac surgery remain limited. In our study, we compared the effects of opioid-free anesthesia combined with regional anesthesia on postoperative pain scores, recovery parameters, and intraoperative hemodynamic variables with conventional opioid-based anesthesia in patients undergoing open-heart surgery with cardiopulmonary bypass. Materials and Methods: This prospective, observational cohort study was conducted following local ethics committee approval (TABED 1-24-95, 03.04.2024) and registration at ClinicalTrials.gov (NCT06683911), at the Cardiovascular Surgery Operating Rooms of Ankara Bilkent City Hospital, Ministry of Health, Turkey. Informed consent was obtained from all participants. Patients undergoing open-heart surgery were divided into two groups according to the anesthesia protocol used. Forty-nine patients who received fentanyl or remifentanil during the intraoperative period were assigned to the opioid anesthesia group, while forty-eight patients who received dexmedetomidine, lidocaine, and magnesium were included in the opioid-free anesthesia group. Preoperative and intraoperative data were recorded, and postoperative data were collected at the 3rd, 6th, 12th, and 24th hours, as well as at 24 hours after extubation, using the QoR-15 questionnaire. Statistical analyses were performed using SPSS version 25.0. For between-group comparisons, the independent samples t-test or mann–whitney u test was used for continuous variables, and the chi-square or fisher's exact test was used for categorical variables. Repeated measures were analyzed using repeated measures analysis of variance, with mauchly's test used to assess the assumption of sphericity. A p-value <0.05 was considered statistically significant. Results: Preoperative and intraoperative characteristics of the patients were comparable between the groups. Although the incidence of bradycardia was similar, repeated measures analysis revealed significantly lower heart rates in the opioid-free group (p=0.002). The incidence of tachycardia was higher in the opioid group (χ²=3.911, p=0.048). Intraoperative ephedrine use was significantly higher in the opioid-free group (p=0.030); however, other hemodynamic interventions and repeated measures analyses showed no significant differences between the groups in terms of mean arterial pressure, BIS, and blood glucose levels. Postoperative pain scores at the 3rd, 6th, and 24th hours were significantly higher in the opioid group (p=0.005, p=0.003, and p=0.001, respectively). QoR-15 scores were significantly higher in the opioid-free group (p=0.0018). The need for additional analgesics and other postoperative recovery parameters were similar between the groups. Conclusion: Opioid-free anesthesia in cardiac surgery appears to be as safe as opioid-based anesthesia during the intraoperative period, while providing better postoperative pain control and positively influencing recovery outcomes. We believe that integrating opioid-free anesthesia into enhanced recovery protocols following cardiac surgery may lead to more effective clinical recovery results.
