Özet

Articulatio femoropatellaris; patella ile condylus medialis femoris ve condylus lateralis femoris arasında gerçekleşir ve articulatio genu'nun bir bölümü olarak kabul edilir. Patella vücudun en büyük sesamoid kemiğidir ve m.quadriceps femoris'in tendonu içinde bulunur. Articulatio genu'yu dış etkilerden korur ve ligamentum patellae'nin kuvvet kolunu artırır. Femurun distalinde bulunan condylus femoris'lerin üzerinde facies patellaris bulunur. Sulcus trochlearis, femurun distalinde iki eklem yüzünü birbirinden ayırır ve arkada fossa intercondylaris ile devam eder. Patella'nın facies articularis'i, femoral facies patellaristen daha küçüktür ve vücuttaki en uyumsuz eklemlerden biridir. Eklem, transvers eksende konkav ve sagittal düzlemde konveks olduğundan, asimetrik bir sellar yapıdadır. M. quadriceps femoris articulatio genu'ya lifleri birbirleriyle kaynaşmaya başlayarak patellaya doğru uzanır. Patellanın alt tarafından uzanarak devam eden tendinöz yapı aşağıda tuberositas tibia'da sonlanır ve ligamentum patellae olarak adlandırılır. Patella, m. quadriceps femoris tendonunu transvers eksenden uzaklaştırarak tuberositas tibiaya yapışma açısını artırır. Bu bir çeşit kaldıraç oluşturarak kasın kuvvetini artırır. Osgood-Schlatter Hastalığı; 1903 yılında Robert B. Osgood ve Carl Schlatter tarafından tanımlanan, büyümekte olan adölesanlarda diz ağrısının sık nedenlerinden biridir. Genellikle dizin önünde ağrılı bir şişlik/kemik çıkıntı ile kendini belli eder. Etiyolojisi tam olarak bilinmemekle birlikte literatürde aşırı kullanıma bağlı tuberositas tibia'da gelişen bir traksiyon apofiziti olarak kabul görmektedir. Sıklıkla aşırı kullanıma bağlı olduğu düşünülse de fiziksel olarak aktif/sporcu olmayan bireylerde de görülebilmektedir. Bunun hastaların patellofemoral yerleşimindeki anatomik farklılıklar ile ilişkili olabileceği düşünülmektedir. Literatürde bu konu ile ilgili radyografik değerlendirme ile yapılmış çalışmalar mevcuttur. Bu çalışmada Osgood-Schlatter hastalığı olan hastaların articulatio femoropatellaris ve ligamentum patellae anatomisinin Manyetik Rezonans Görünütüleme ile yaş ve cinsiyet açısıdan eşleştirilmiş bir kontrol grubu ile karşılaştırmalı olarak değerlendirilmesi planlanmaktadır. Böylece Osgood Schlatter hastalığı olanlarda sağlıklı kontrollere göre radyolojik farklılıkların ortaya konularak ve anatomik risk faktörlerinin belirlenebileceği düşünülmüştür.

Articulatio femoropatellaris is a joint between the patella and the condylus medialis femoris and condylus lateralis femoris and is considered a part of the articulatio genu. Patella is the largest sesamoid bone of the body and is located within the tendon of m.quadriceps femoris. It protects the articulatio genu from external influences and increases the leverage of the ligamentum patellar. Facies patellaris is located on the condylus femoris in the distal part of the femur. Sulcus trochlearis separates the two articular surfaces in the distal femur and continues with the fossa intercondylaris at the back. The facies articularis of the patella is smaller than the femoral facies patellaris and that makes the articulatio femoropatellaris one of the most incongruent joints in the body. Since the joint is concave in the transverse axis and convex in the sagittal plane, it has an asymmetric sellar structure. M. quadriceps femoris fibers begin to fuse with each other and extend towards the patella. The tendinous structure, which extends from the underside of the patella, ends below at the tuberosity of the tibia and is called ligamentum patellae. The patella moves the m.quadriceps femoris tendon away from the transverse axis, increasing the angle of attachment to the tuberosity of the tibia. This increases the strength of the muscle by creating a type of leverage. Osgood-Schlatter Disease, described by Robert B. Osgood and Carl Schlatter in 1903, is a common cause of knee pain in growing adolescents. It usually manifests itself with a painful swelling/bony protrusion in front of the knee. Although its etiology is not fully known, it is accepted as a traction apophysitis that develops in the tuberosity of the tibia due to overuse. It can also be seen in individuals who are not physically active/athletes. It is thought that this may be related to anatomical differences in the patients' patellofemoral placement. There are studies in the literature on this subject using radiographic evaluations. In this study, it is planned to evaluate the articulatio femoropatellaris and ligamentum patellae anatomy of patients with Osgood-Schlatter disease comparatively with a control group matched in terms of age and gender by Magnetic Resonance Imaging. Thus, it was thought that radiological differences in patients with Osgood Schlatter disease compared to healthy controls could be revealed and anatomical risk factors could be determined.
