Hasta grubunun 24'ü atak, 26'sı remisyon, 25'i ise progresif evrededir. Anahtar Kelimler:Multiple Skleroz, Oksidatif Stres, Kognitif Bozulma, Progresif MS,Paraoksonaz
Özet (Çeviri)
Introduction and Objective: Multiple sclerosis (MS) is a chronic disease of the central nervous system characterised by inflammatory, demyelinating and neurodegenerative features, with clinical subtypes such as relapsing-remitting (RRMS) and secondary progressive (SPMS). The role of oxidative stress in the immunopathogenesis of MS is increasingly being emphasised, and it has been suggested that the disruption of the balance between free radical production and antioxidant defence systems may accelerate the progression of the disease by causing damage to myelin and axons. In addition, cognitive impairment is common in MS and significantly affects patients' quality of life and functional independence. This study was designed to determine oxidative stress levels in different stages of MS (relapsing-remitting, remission, progressive), to reveal the relationship between these parameters and cognitive function and depression levels, and to evaluate the discriminatory power of oxidative biomarkers in predicting the progressive stage of MS. Method: A total of 100 individuals were included in the study, comprising 45 RRMS, 30 SPMS and 25 healthy controls. Of the patient group, 24 were in the attack phase, 26 in remission and 25 in the progressive phase. Participants' cognitive performance was assessed using the Brief International Cognitive Assessment of MS (BICAMS) test battery, and their depression levels were evaluated using the Beck Depression Scale. Serum samples were analysed for total antioxidant level (TAS), total oxidant level (TOS), oxidative stress index (OSI), and paraoxonase-1 (PON-1) levels. These parameters were compared between groups, and correlations and ROC analyses were performed to investigate the relationships between oxidative stress and clinical/cognitive variables. Results: TAS (p<0.001) and PON-1 (p=0.014) levels were statistically significantly lower, while OSİ (p<0.001) levels were significantly higher in the SPMS group. TOS levels were statistically significantly lower in SPMS than in the RRMS group. (p=0.001). Patients in the progressive phase had lower TAS, TOS, and PON-1 and higher OSİ values compared to those in the attack and remission phases (p<0.001). BICAMS scores were lower in all patient groups compared to the control group; the lowest scores were observed in the SPMS group (Total BICAMS: SPMS 70.7±23.9; RRMS 101.5±25.4; Control 131.9±16.9; p<0.001). Correlation analysis revealed that TAS and OSİ levels were significantly associated with SDMT (r=0.33 vs r=–0.35), BVMT-R (r=0.47 vs r=–0.48), and Total BICAMS (r=0.31 vs r=–0.32) scores, particularly. In the ROC analysis, the highest discriminatory power in predicting the progressive phase was obtained with TAS (cut-off ≤0.69; AUC=0.896; 98% specificity) and OSİ (cut-off >75.98; AUC=0.872; 100% specificity). Conclusion: Oxidative stress increases significantly, particularly in the progressive stage of MS, and appears to be closely related to cognitive performance. Biomarkers such as TAS and OSİ have the potential to be used in determining the stage of the disease and predicting cognitive impairment. The findings suggest that OSİ can be used not only as a diagnostic tool but also as a biochemical indicator of progression, thereby contributing to the literature. Our findings emphasise the importance of monitoring oxidative stress in the treatment of MS.
