Özet (Çeviri)
-Objective: This study aims to evaluate the risk of developing neurological deficits before and after surgical treatment in patients who develop cerebral malperfusion due to acute type A aortic dissection, and to assess the surgical outcomes in this patient population. -Materials and Methods: The study was conducted retrospectively on data from 57 patients at the University of Health Sciences, Seyrantepe Hamidiye Etfal Training and Research Hospital. Participants were divided into two groups: those with preoperative cerebral malperfusion and those without preoperative cerebral malperfusion. In the SMP (+) group, cerebral malperfusion was present preoperatively, whereas in the SMP (–) group, it was not observed preoperatively. Cerebral perfusion status, laboratory findings, presenting symptoms, and complications were evaluated. -Results: Significant differences were found between the groups in terms of presenting symptoms, operative time, laboratory findings, and postoperative cerebral malperfusion. Back pain was significantly more common in the SMP (–) group compared to the SMP (+) group (51.3% vs. 15%), and this difference was statistically significant (p = 0.007). The operative time was longer in the SMP (–) group (467 ± 148 minutes) compared to the SMP (+) group (384 ± 85 minutes) (p = 0.0352). Postoperative cerebral malperfusion (POSMP) was observed only in the SMP (+) group and was statistically significant (15%; p = 0.039). The mean platelet volume (MPV) and platelet distribution width (PDW) values at admission were significantly higher in the SMP (+) group compared to the SMP (–) group (10.8 ± 1.2 vs. 9.9 ± 1.3 fL, p = 0.0103 and 16.6% ± 0.5 vs. 15.9% ± 1, p = 0.0019, respectively). -Conclusion: This study demonstrates that cerebral hypoperfusion is frequently accompanied by prominent neurological symptoms, which should be carefully evaluated during the diagnostic process. In the prevention of cerebral malperfusion (CMP), meticulous planning of cerebral perfusion strategies, effective use of intraoperative neuromonitoring, and consideration of individual anatomical variations appear to be of critical importance. Moreover, the significantly elevated levels of mean platelet volume (MPV) and platelet distribution width (PDW) observed in patients who developed CMP suggest that platelet activation may represent a key pathophysiological mechanism in this subgroup. In this context, platelet count, MPV, and PDW may serve as non-invasive, easily accessible, and cost-effective potential biomarkers for assessing the risk of CMP.The findings also indicate that, in addition to inflammation, platelet activation and microthrombus formation may play important roles in the development of CMP. Notably, MPV and PDW could serve as early warning indicators during the perioperative evaluation of high-risk patients. These data may contribute to the advancement of clinical decision support systems and provide a basis for generating new hypotheses in future research. -
