Özet
Amaç: Bu çalışmada sleeve gastroktemi geçiren bireylerin operasyondan önce ve postoperatif birinci yıl kontrollerinde verdiği rutin tetkiklerde ölçülen serum TSH, FT3, FT4, FT3/FT4, HOMA-IR, APRI, FIB-4 skoru ve NLO değişimlerinin değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Çalışmamız tek merkezli retrosepktif olarak planlandı. Çalışmamıza Hatay Mustafa Kemal Üniversitesi Hastanesi'nde alanında tecrübeli bir hekim tarafından aynı teknikle sleeve gastroktemi ameliyatı geçirmiş 64 hasta dahil edildi. Hastaların demografik özellikleri, preoperatif dönem ve postoperatif dönem 1. yıl kontrolünde bakılan tam kan sayımı, tiroid hormon profili (TSH, FT3, FT4), lipid profili (trigliserit, LDH, HDL, total kolesterol), APRI ve FIB-4 skorunun hesaplanması için karaciğer enzim değerleri (AST, ALT), HOMA-IR hesaplanması için açlık glukoz ve açlık insülin değeri, HbA1c ve VKİ değerleri araştırmacı tarafından hazırlanan hasta veri formuna kaydedildi. Bulgular: Çalışmamızdaki hastaların 16'sı (%25,0) erkek, 48'i (%75,0) kadın idi. hastaların yaş ortalaması 35,73 yıl (minimum:22-maksimum:60) idi. Kadın hastaların preoperatif total kolesterol değerlerinin erkek hastalara kıyasla daha yüksek olduğu saptandı (p=0,025). Çalışma sonucuna göre postoperatif 1. ayda bireylerin açlık glukoz, açlık insülin, HbA1c ve HOMA-IR değerlerinde anlamlı azalma saptandı (sırasıyla p<0,001, p<0,001, p<0,001, p<0,001). Postoperatif 1. ayda bireylerin nötrofil, NLO, AST ve ALT değerlerinde anlamlı azalma saptandı (sırasıyla p<0,001, p<0,001, p<0,001, p<0,001). Postoperatif 1. ayda bireylerin APRI ve FIB-4 skorlarında anlamlı azalma saptandı (sırasıyla p<0,001, p=0,002). Sonuç: Çalışmamızda sleeve gastroktemi yapılan hastaların TSH ve FT4 hormon düzeylerinde operasyon öncesine göre operasyon sonrası 1. yılda anlamlı bir değişiklik olmadığı, FT3 ve FT3/FT4, HOMA-IR, APRI ve FIB-4 skoru ve NLO değerlerinde ise operasyon öncesine göre operasyon sonrası 1. yılda azalma olduğu bulundu. Sleeve gastrektomi operasyonlarının ağırlık kaybının yanında metabolik parametrelerde iyileşme sağladığı bulgularımız ile desteklenmiştir. Bu sonuç doğrultusunda, SG operasyonlarının metabolik parametrelerde iyileşme sağlamak için bir seçenek olarak uygulanabileceği düşünülmüştür.

Aims: In this study, it was aimed to evaluate the changes in serum TSH, FT3, FT4, FT3/FT4, HOMA-IR, APRI, FIB-4 score and NLR measured in the routine examinations given in the preoperative and first-year controls of individuals who had sleeve gastrocemia. Materials and Methods: Our study was planned as a single-center, prospective study. Our study included 64 patients who had undergone sleeve gastroctomia surgery with the same technique by an experienced physician at Hatay Mustafa Kemal University Hospital. Demographic characteristics of the patients, preoperative period and postoperative period 1st year control, complete blood count, thyroid hormone profile (TSH, FT3, FT4) and lipid profile (triglyceride, LDH, HDL, total cholesterol) were recorded in the patient data form prepared by the researcher. Liver enzyme values (AST, ALT) were recorded to calculate APRI and FIB-4 scores, and fasting glucose and fasting insulin values, HbA1c and BMI values were recorded for HOMA-IR calculations. Results: Of the patients in our study, 16 (25.0%) were male and 48 (75.0%) were female.The mean age of the patients was 35.73 years (minimum:22-maximum:60). Preoperative total cholesterol values of female patients were found to be higher than male patients (p=0.025). According to the results of the study, a significant decrease was found in the fasting glucose, fasting insulin, HbA1c and HOMA-IR values of the individuals in the postoperative 1st month (p<0.001, p<0.001, p<0.001, p<0.001, respectively). There was a significant decrease in the neutrophil, NLR, AST and ALT values of the individuals in the first month postoperatively (p<0.001, p<0.001, p<0.001, p<0.001, respectively). A significant decrease was found in the APRI and FIB-4 scores of the individuals at the postoperative 1st month (p<0.001, p=0.002, respectively). Conclusion: In our study, there was no significant change in TSH and FT4 hormone levels in the 1st year after the operation compared to the preoperative period of the patients who underwent sleeve gastrocemia, and in the FT3 and FT3/FT4, HOMA-IR, APRI and FIB-4 scores and NLR values after the operation compared to the preoperative 1st year. found to decrease per year. It is supported by our findings that sleeve gastrectomy operations provide improvement in metabolic parameters as well as weight loss. In line with this result, it was thought that SG operations could be applied as an option to improve metabolic parameters
