Özet

Amaç: Fasiyal sinir yüz estetiğini etkileyen önemli bir yapıdır. Fasiyal sinir hasarı, sık karşılaşılan ve tedavisi klinik pratikte önemli yer tutan bir durumdur. Fasiyal paralizilerin tedavilerini geliştirmek amacıyla literatürde yeni araştırmalar bulunmaktadır. Bizde çalışmamızda travmatik fasiyal sinir hasarında eksozom ve deksometazonu lokal ve sistemik olarak ayrı ayrı uygulayarak tedavi edici etkilerini hem elektrofizyolojik hem de histopatolojik açıdan araştırmayı amaçladık Materyal-Metot: Çalışmamızda 45 adet Wistar Albino cinsi erkek rat kullanıldı. Hayvanlar; kontrol, lokal eksozom, lokal deksometazon, sistemik eksozom, sistemik deksometazon grupları olacak şekilde 5 adet eşit sayıda rat içeren gruplara ayrıldı. 1 adet rat çalışma esnasında öldüğünden çalışma dışı bırakıldı. Tüm ratlara 'crush injury' şeklinde fasiyal sinir travması uygulandı. Tedavi protokollerinde lokal eksozom ve lokal deksometazon uygulamaları hasar sonrası bir kez, sistemik uygulamalarda eksozom gün aşırı olacak şekilde toplam 4 gün, deksometazon ise 7 gün intraperitoneal uygulandı. Sekiz hafta sonrasında hasar gören fasiyal sinir segmenti çıkarılıp histopatolojik incelemeye gönderildi. Hasar öncesi, sonrası ve sekizinci haftada fasiyal sinir fonksiyonları elektrofizyolojik olarak kayda alındı. Bulgular: Travma sonrası erken dönemde EMG verileri gruplar arasında benzer bulunmuş, sinirde tam iletim kaybı oluşmadığı gözlemlenmiştir. Sekiz haftalık takip sonunda sistemik eksozom tedavisi, EMG amplitüdünde en yüksek artışı sağlarken; sistemik deksometazon ve lokal eksozom uygulamaları da anlamlı düzeyde iyileşme göstermiştir. Histopatolojik değerlendirmede, tedavisiz grupta en ileri düzeyde aksonal dejenerasyon izlenmiş; lokal eksozom grubunda ise bu hasar en düşük seviyede tespit edilmiştir. Akson çapı açısından lokal eksozom ve lokal deksametazon tedavi grupları sistemik tedavilere üstünlük göstermiştir. Sonuç: Çalışmamızda sistemik eksozom elektrofizyolojik iyileşmede, lokal eksozom ve lokal deksometazon tedavileri ise histolojik düzelmede etkili görünmektedir. Bu bulgular, sinir hasarlarında farklı tedavi modalitelerinin kombine kullanımının potansiyel faydasına işaret etmektedir.

Objective: The facial nerve is a critical structure that influences facial aesthetics. Facial nerve injury is a commonly encountered condition in clinical practice and holds significant importance in terms of treatment. There are ongoing studies in the literature aiming to improve the treatment of facial paralyses. In our study, we aimed to investigate the therapeutic effects of exosome and dexamethasone, administered both locally and systemically, in traumatic facial nerve injury, from both electrophysiological and histopathological perspectives. Materials and Methods: A total of 45 male Wistar Albino rats were used in our study. The animals were divided into five groups, each consisting of an equal number of rats: control, local exosome, local dexamethasone, systemic exosome, and systemic dexamethasone groups. One rat died and was excluded from the study. All rats underwent a“crush injury”applied to the facial nerve. In the treatment protocols, local exosome and local dexamethasone were administered once; systemic exosome was administered intraperitoneally every other day for a total of 4 doses, and systemic dexamethasone was administered intraperitoneally for 7 consecutive days. Eight weeks later, the injured segment of the facial nerve was harvested for histopathological evaluation. Electrophysiological recordings of facial nerve function were obtained before injury, after injury, and at the eighth week. Results: In the early post-traumatic period, EMG data were found to be similar between the groups, indicating that a complete conduction loss did not occur. At the end of the eight-week follow-up, systemic exosome therapy resulted in the highest increase in EMG amplitude, while systemic dexamethasone and local exosome treatments also demonstrated significant improvement. In the histopathological evaluation, the untreated group exhibited the most severe axonal degeneration, whereas the local exosome group showed the lowest level of damage. In terms of axonal diameter, the local exosome and local dexamethasone treatment groups outperformed the systemic treatments. Conclusion: In our study, systemic exosome therapy appeared to be effective in promoting electrophysiological recovery, while local exosome and local dexamethasone treatments were more effective in achieving histological improvement. These findings suggest the potential benefits of combining different therapeutic modalities in nerve injury treatment.
