Özet
Giriş ve Amaç: Diffüz büyük B hücreli lenfoma (DBBHL), erişkinlerde en sık görülen non-Hodgkin lenfoma olup belirgin klinik ve biyolojik heterojenite göstermektedir. 2022 DSÖ sınıflandırmasında MYC ve BCL2 yeniden düzenlenmesini birlikte taşıyan olguları yüksek agresivite nedeniyle“HGBL-MYC/BCL2”başlığı altında ayrı bir kategoriye alırken, MYC+BCL6 yeniden düzenlenmesi olan hastaların heterojen yapısı nedeniyle bu grubun DBBHL, NOS veya HGBL, NOS (High-Grade B-Cell Lymphoma, NOS) içerisinde değerlendirilmesini önermektedir. Bu değişiklikler, double/triple ekspresör lenfoma (DEL/TEL) ve double/triple hit lenfoma (DHL/THL) fenotiplerinin prognostik önemini artırmıştır. Bu çalışmada, merkezimizdeki DBBHL hastalarında bu alt tiplerin sıklığı ile tedavi yanıtları ve sağkalım üzerine etkileri değerlendirilmiştir. Gereç ve Yöntem: Mayıs 2020–Temmuz 2025 arasında merkezimizde takip edilen 18 yaş ve üzeri 242 DBBHL hastası retrospektif olarak incelendi. 242 hastanın 36'sı DEL, 50'si TEL, 3'ü DHL, 4'ü THL olarak değerlendirildi. Klinik özellikler, tedavi yanıtları, immünohistokimyada bakılan MYC/BCL2/BCL6 ekspresyonları ve ekspresör hastalarda yapılan FISH analiz sonuçları değerlendirildi. Sağkalım analizleri Kaplan–Meier yöntemiyle yapıldı. Bulgular: DBBHL tanılı 242 hastanın medyan yaşı 61 (ortalama 58,9 ± 15,1 yıl) olup, hastaların %52'si erkektir. Olguların %66,9'unda tanı anında ileri evre (Evre III–IV) olarak değerlendirildi. Toplam DBBHL olgusunun %61,6'sını (n=149) kontrol DBBHL grubunu, %14,9'unu (n=36) double ekspresör, %20,7'sini (n=50) triple ekspresör, %1,2'sini (n=3) double hit ve %1,7'sini (n=4) triple hit fenotipli hastalar oluşturmuştur. Ekspresör fenotipli 93 hastadan 11 hastada FISH analizi yapılamamış olup kalan 82 hastanın %8,5'i hit lenfoma olarak sınıflandırılmıştır. Hastalar ortalama 610,6 ± 468,6 gün boyunca izlenmiş olup, en uzun takip süresi 2477 gündür. Tedavi sonu değerlendirmede en yüksek tam yanıt (CR) oranı kontrol grubunda (%87,4) gözlenirken; ekspresör gruplarda bu oran double ekspresörlerde %76, triple ekspresörlerde %75,7 olarak saptanmıştır. Hit fenotiplerinde ise double hit grubunda CR oranı %50 iken, triple hit grubunda tedavi alabilen iki olgunun tamamında tam yanıt elde edilmiştir. Tedavi sonu yanıt açısından DBBHL alt grupları arasında anlamlı fark saptanmadı (p=0.10). Progresyonsuz sağkalım (PFS) medyan süreleri kontrol DBBHL grubunda daha uzun (403 gün) iken; double ekspresör 176 gün, triple ekspresör 196 gün, double hit 153 gün ve özellikle triple hit grubunda 81 gün belirgin şekilde daha kısaydı. PFS açısından anlamlı fark kontrol ve triple hit grupları arasında saptandı (p<0,05). Toplam takip süresi medyan değerleri ise kontrol DBBHL grubunda 645 gün, double ekspresör grupta 455 gün, triple ekspresör grupta 315 gün, double hit grupta 567 gün ve triple hit grupta 251 gün olarak hesaplandı. Bu farklılık da kontrol grubu ile triple hit grubu arasında anlamlıydı (p<0,05). Gruplar arasında medyan PFS değerlerinde belirgin farklılıklar görülmekle birlikte, Kaplan–Meier eğrilerinin log-rank testi ile karşılaştırılmasında bu farkların istatistiksel olarak anlamlı olmadığı saptandı. Genel sağkalım analizinde kontrol grubunda ortalama OS 1801 gün iken; double ekspresör, triple ekspresör, double hit ve triple hit gruplarında sırasıyla 1193, 1043, 1019 ve 394 gün olarak hesaplandı. Hit ve ekspresör fenotiplerinde daha kısa sağkalım eğilimi gözlense de bu fark log-rank testinde göre istatistiksel olarak anlamlı bir farklılık göstermedi (p=0.25). Double ekspresör (n=36) grubunda yapılan analizlerde BCL2⁺ olgularda erken mortalite eğilimi, BCL6⁺ olgularda ise daha uzun sağkalım süresi gözlenmiştir.Ancak mevcut örneklem büyüklüğünde bu bulgular istatistiksel düzeyde desteklenememektedir. Sonuç: Bu çalışma, DBBHL'de ekspresör ve hit fenotiplerinin belirgin bir klinik heterojenite gösterdiğini ve her iki alt grupta da progresyonsuz sağkalım (PFS), genel sağkalım (OS) ve toplam takip süresinin kontrol DBBHL olgularına kıyasla belirgin şekilde daha kısa olduğunu ortaya koymuştur. Çalışmamızda biyolojik olarak en kötü seyrin DHL/THL olgularında izlendiği görülmüş, ancak bu alt grubun sınırlı sayısı istatistiksel gücü kısıtlamıştır. Bu nedenle sonuçların geniş örneklemli prospektif çalışmalarla doğrulanması gerekmektedir. Ekspresör grupta MYC⁺/BCL2⁺/BCL6⁻ olgularının erken progresyon ve daha kısa sağkalımla seyretmesi, buna karşılık MYC⁺/BCL6⁺/BCL2⁻ olgularının daha iyi klinik sonuçlar göstermesi, ekspresör grubun heterojen olduğunu ve risk sınıflamasında MYC+BCL2 temelli ayrımın daha anlamlı sonuçlar verebileceğini düşündürmektedir.

Introduction: Diffuse large B-cell lymphoma (DLBCL) is the most common type of non-Hodgkin lymphoma in adults and exhibits marked clinical and biological heterogeneity. In the 2022 WHO classification, cases harboring concurrent MYC and BCL2 rearrangements were categorized separately under“HGBL-MYC/BCL2”due to their highly aggressive behavior, whereas cases with MYC+BCL6 rearrangements were recommended to be classified within DLBCL, NOS or HGBL, NOS (High-Grade B-Cell Lymphoma, NOS) because of their heterogeneous nature. These revisions have increased the prognostic relevance of double/triple expressor lymphomas (DEL/TEL) and double/triple hit lymphomas (DHL/THL). In this study, we evaluated the frequency of these subtypes, as well as their treatment responses and survival outcomes, in patients with DLBCL followed at our center. Materials and Methods: Between May 2020 and July 2025, a total of 242 DLBCL patients aged 18 years and older who were followed at our center were retrospectively analyzed. Of these patients, 36 were classified as DEL, 50 as TEL, 3 as DHL, and 4 as THL. Clinical characteristics, treatment responses, MYC/BCL2/BCL6 expression profiles assessed by immunohistochemistry, and FISH results obtained in expressor cases were evaluated. Survival analyses were performed using the Kaplan–Meier method. Findings: Among the 242 patients diagnosed with DLBCL, the median age was 61 years (mean 58.9 ± 15.1), and 52% of the cohort were male. At the time of diagnosis, 66.9% of the patients were evaluated as having advanced-stage disease (Stage III–IV). Of all DLBCL cases, 61.6% (n=149) constituted the control DLBCL group, while 14.9% (n=36) were classified as double expressor, 20.7% (n=50) as triple expressor, 1.2% (n=3) as double hit, and 1.7% (n=4) as triple hit lymphoma. Among the 93 patients with an expressor phenotype, FISH analysis could not be performed in 11 patients; among the remaining 82 cases, 8.5% were categorized as hit lymphoma.The mean follow-up duration was 610.6 ± 468.6 days, with the longest follow-up reaching 2477 days. At end-of-treatment evaluation, the highest complete response (CR) rate was observed in the control group (87.4%), whereas the CR rates were 76% in the double expressor group and 75.7% in the triple expressor group. Within the hit phenotypes, the CR rate was 50% in the double hit group, while both patients who were able to receive treatment in the triple hit group achieved complete response. There was no statistically significant difference in end-of-treatment response rates among the DLBCL subgroups (p = 0.10).Median progression-free survival (PFS) was longer in the control DLBCL group (403 days), whereas median PFS values were 176 days in the double expressor, 196 days in the triple expressor, 153 days in the double hit, and markedly shorter at 81 days in the triple hit group. A statistically significant difference in PFS was detected between the control and triple hit groups (p < 0.05). Median total follow-up durations were calculated as 645 days in the control group, 455 days in the double expressor group, 315 days in the triple expressor group, 567 days in the double hit group, and 251 days in the triple hit group; this difference was also significant between the control and triple hit groups (p < 0.05).Although substantial differences were observed in median PFS values among the subgroups, comparison of the Kaplan–Meier curves using the log-rank test did not reveal statistically significant differences. In overall survival (OS) analysis, the mean OS was 1801 days in the control group, whereas it was 1193, 1043, 1019, and 394 days in the double expressor, triple expressor, double hit, and triple hit groups, respectively. Despite a trend toward shorter survival in the expressor and hit phenotypes, this difference did not reach statistical significance in the log-rank test (p = 0.25). In the double expressor group (n = 36), subgroup analyses indicated a tendency toward early mortality among BCL2⁺ cases, whereas BCL6⁺ cases showed a trend toward longer survival. However, these findings could not be supported statistically due to the limited sample size. Conclusion: This study demonstrates that the expressor and hit phenotypes in DLBCL exhibit substantial clinical heterogeneity, with both subgroups showing markedly shorter progression-free survival (PFS), overall survival (OS), and total follow-up durations compared with the control DLBCL cohort. Although the biologically poorest outcomes were observed in DHL/THL cases, the limited number of patients within this subgroup restricted the statistical power of the analyses. Therefore, validation of these findings in larger, prospective cohorts is warranted. In the expressor group, the observation that MYC⁺/BCL2⁺/BCL6⁻ cases were associated with early progression and inferior survival, whereas MYC⁺/BCL6⁺/BCL2⁻ cases demonstrated more favorable clinical outcomes, highlights the heterogeneity within this phenotype. These findings further suggest that MYC+BCL2-based stratification may provide more meaningful prognostic discrimination within expressor subtypes.
