Özet

Amaç: Diyabetes mellitus (DM), santral sinir sisteminde oksidatif stres, nöroinflamasyon, demir homeostazında bozulma ve lipid peroksidasyonu gibi mekanizmalarla nöronal hasara yol açabilen metabolik bir hastalıktır. Bu çalışmada, streptozotosin (STZ) ile oluşturulan diyabetik sıçan modelinde propofol ve/veya deksmedetomidinin hipokampal nörotoksisite üzerine modülatör etkilerinin ve bu etkilerin ferroptozis ile ilişkili olası mekanizmalarının araştırılması amaçlanmıştır. Gereç ve Yöntem: Çalışmada 50 adet 8–10 haftalık erkek Wistar-Albino sıçan kullanıldı. Denekler her grupta 10 hayvan bulunacak şekilde rastgele beş gruba ayrıldı: Grup 1 (Kontrol), Grup 2 (STZ), Grup 3 (STZ + PRO), Grup 4 (STZ + DEK), Grup 5 (STZ + PRO + DEK). Kontrol grubuna hiçbir ilaç uygulanmadı. DM, 45 mg/kg tek doz STZ'nin intraperitoneal uygulamasıyla oluşturuldu. Diyabet indüksiyonundan yedi gün sonra 40 mg/kg propofol ve/veya 50 μg/kg deksmedetomidin intraperitoneal olarak uygulandı. Sekizinci günde hipokampal dokular toplandı. TNF-α, IL-1β, IL-6, demir, malondialdehit düzeyleri, indirgenmiş glutatyon/okside glutatyon (GSH/GSSG) ve glutatyon peroksidaz aktivitesi biyokimyasal olarak; nükleer faktör eritroid 2 ile ilişkili faktör 2 (Nrf2) ve transferrin reseptörü 1 (TfR1) immünreaktivitesi immünohistokimyasal olarak değerlendirildi. Histopatolojik değişiklikler hematoksilen-eozin boyamasıyla incelendi. Bulgular: STZ grubunda kontrol grubuna göre hipokampal demir, malondialdehit, TNF-α, IL-1β ve IL-6 düzeyleri anlamlı olarak artarken, glutatyon peroksidaz aktivitesi ile GSH/GSSG anlamlı olarak azaldı. İmmünohistokimyasal değerlendirmede Nrf2 immünoreaktivitesinde azalma, TfR1 immünoreaktivitesinde artış saptandı. Histopatolojik incelemede nöronlarda belirgin dejeneratif değişiklikler izlendi. Grup 2 (STZ)'ye kıyasla propofol ve/veya deksmedetomidin uygulanan gruplarda oksidatif stres, inflamasyon ve demir birikimi ile ilişkili parametrelerde gerileme; antioksidan savunma göstergelerinde ise artış gözlendi. Ayrıca Nrf2 immünoreaktivitesinin arttığı, TfR1 immünoreaktivitesinin azaldığı ve histopatolojik hasarın hafiflediği belirlendi. Kombinasyon grubunda düzelmenin daha belirgin olduğu saptandı. Sonuç: Bulgularımız, deneysel DM modelinde hipokampal nörotoksisitenin erken dönemde ortaya çıktığını ve bu hasarın oksidatif stres, nöroinflamasyon ve ferroptozis ile ilişkili mekanizmalar üzerinden geliştiğini düşündürmektedir. Propofol ve/veya deksmedetomidin uygulanması bu patolojik süreci baskılayarak diyabete bağlı erken dönem nörotoksisite üzerinde nöroprotektif bir rol üstlenebilir.

Objective: Diabetes mellitus (DM) is a major metabolic disorder that may cause neuronal damage in the central nervous system through mechanisms such as oxidative stress, neuroinflammation, disruption of iron homeostasis, and lipid peroxidation. This study aimed to investigate the modulatory effects of propofol and/or dexmedetomidine on hippocampal neurotoxicity and the possible mechanisms associated with ferroptosis in a streptozotocin (STZ)-induced diabetic rat model. Materials and Methods: Fifty 8–10-week-old male Wistar albino rats were used in the study. The animals were randomly assigned to five groups, with 10 rats in each group: Group 1 (Control), Group 2 (STZ), Group 3 (STZ + PRO), Group 4 (STZ + DEX), and Group 5 (STZ + PRO + DEX). No drug was administered to the control group. DM was induced by a single intraperitoneal (i.p.) dose of STZ at 45 mg/kg. Seven days after diabetes induction, propofol at 40 mg/kg and/or dexmedetomidine at 50 μg/kg were administered intraperitoneally. Hippocampal tissues were collected on the eighth day. The levels of TNF-α, IL-1β, IL-6, iron, malondialdehyde, reduced glutathione/oxidized glutathione (GSH/GSSG) and glutathione peroxidase activity were evaluated biochemically; nuclear factor erythroid 2–related factor 2 (Nrf2) and transferrin receptor 1 (TfR1) immunoreactivity were assessed immunohistochemically. Histopathological changes were examined using hematoxylin and eosin staining. Results: Compared with the control group, the STZ group showed significantly increased hippocampal levels of iron, malondialdehyde, TNF-α, IL-1β, and IL-6, whereas glutathione peroxidase activity and GSH/GSSG were significantly decreased. Immunohistochemical evaluation demonstrated reduced Nrf2 immunoreactivity and increased TfR1 immunoreactivity. Histopathological examination revealed marked degenerative changes in hippocampal neurons. Compared with Group 2 (STZ), the groups treated with propofol and/or dexmedetomidine exhibited reductions in parameters associated with oxidative stress, inflammation, and iron accumulation, along with increases in markers of antioxidant defense. In addition, Nrf2 immunoreactivity increased, TfR1 immunoreactivity decreased, and histopathological damage was attenuated. The improvement was more pronounced in the combination group. Conclusion: Our findings suggest that hippocampal neurotoxicity emerges in the early phase of experimental DM and that this damage may develop through mechanisms related to oxidative stress, neuroinflammation, and ferroptosis. Administration of propofol and/or dexmedetomidine may suppress this pathological process and thereby exert a neuroprotective effect against early diabetes-related neurotoxicity.
