Özet
Pnömonektomi, yüksek morbidite ve mortalite oranları ile seyreden majör bir cerrahi prosedürdür. Bu çalışmada, preoperatif tam kan sayımı parametrelerinden hesaplanan sistemik inflamatuar belirteçlerin, pnömonektomi sonrası erken dönemde gelişen majör postoperatif komplikasyonları öngörmedeki etkinliği araştırılmıştır. Ocak 2010 ile Şubat 2026 tarihleri arasında pnömonektomi uygulanan 202 hasta retrospektif olarak incelenmiştir. Çalışmada preoperatif dönemde hesaplanan Nötrofil-Lenfosit Oranı (NLR), Platelet-Lenfosit Oranı (PLR), Lenfosit-Monosit Oranı (LMR), Sistemik İmmün-İnflamasyon İndeksi (SII), Derive Nötrofil-Lenfosit Oranı (dNLR), Sistemik İnflamasyon Cevap İndeksi (SIRI), Sistemik Enflamasyonun Toplu İndeksi (AISI), Prognostik Nütrisyonel İndeks (PNI) ve CRP-Albümin Oranı (CAR) değerlerinin postoperatif otuz günlük süreçte gözlenen Clavien-Dindo Grade II ve üzeri komplikasyonlarla ilişkisi analiz edilmiştir. Hastaların %26,7'sinde (n=54) postoperatif komplikasyon gelişmiş olup en sık görülenler pnömoni (%12,4) ve kardiyak aritmidir (%9,9). Operatif mortalite oranı ise %4,9 olarak saptanmıştır. Analizler sonucunda yüksek NLR (p=0,004), PLR (p=0,01), SII (p=0,004), dNLR (p=0,009) ve SIRI (p=0,003) değerleri postoperatif komplikasyon gelişimi ile istatistiksel olarak anlamlı derecede ilişkili bulunmuştur. Komplikasyon gelişen hastaların 5 yıllık sağkalım oranı (%29), komplikasyon gelişmeyenlere (%45) göre anlamlı derecede daha düşük izlenmiştir (p=0,02). Sonuç olarak, preoperatif dönemde hesaplanan yüksek NLR, PLR, SII, SIRI ve dNLR değerleri, pnömonektomi sonrası komplikasyon gelişimi için anlamlı öngördürücülerdir. Bu düşük maliyetli belirteçler, riskli hastaların belirlenmesinde ve postoperatif takip stratejilerinin optimizasyonunda klinisyenlere yardımcı olabilir.

Pneumonectomy is a major surgical procedure associated with high morbidity and mortality rates. This study investigates the efficacy of systemic inflammatory markers derived from preoperative complete blood count parameters in predicting major postoperative complications that develop during the early period following pneumonectomy. A total of 202 patients who underwent pneumonectomy between January 2010 and February 2026 were retrospectively reviewed. The study analyzed the relationship between Clavien-Dindo Grade II and higher postoperative complications occurring within the first 30 days and the following values calculated in the preoperative period: Neutrophil-to-Lymphocyte Ratio (NLR), Platelet-to-Lymphocyte Ratio (PLR), Lymphocyte-to-Monocyte Ratio (LMR), Systemic Immune-Inflammation Index (SII), Derived Neutrophil-to-Lymphocyte Ratio (dNLR), Systemic Inflammation Response Index (SIRI), Aggregate Index of Systemic Inflammation (AISI), Prognostic Nutritional Index (PNI), and CRP-to-Albumin Ratio (CAR). Postoperative 30-day complications developed in 26.7% of the patients (n=54), with pneumonia (12.4%) and cardiac arrhythmia (9.9%) being the most frequently encountered. The operative mortality rate was found to be 4.9%. As a result of the analyses, high NLR (p=0.004), PLR (p=0.01), SII (p=0.004), dNLR (p=0.009), and SIRI (p=0.003) values were found to be statistically significantly associated with the development of postoperative complications. The 5-year survival rate of patients who developed complications (29%) was observed to be significantly lower compared to those who did not develop complications (45%) (p=0.02). In conclusion, high NLR, PLR, SII, SIRI, and dNLR values calculated in the preoperative period are significant predictors for the development of complications after pneumonectomy. These cost-effective markers can assist clinicians in identifying high-risk patients and optimizing postoperative follow-up strategies.
