Özet
Kronik yaralar, uzamış iyileşme süreleri, bozulmuş doku rejenerasyonu, enfeksiyona yatkınlıkları ve oluşturdukları önemli sosyoekonomik yük nedeniyle ciddi bir sağlık problemi oluşturmaktadır. Akut yaralardan farklı olarak kronik yaralar; sürekli inflamasyon, artmış oksidatif stres, bozulmuş anjiyogenez, yüksek proteolitik aktivite ve düzensiz hücre dışı matriks (ECM) yeniden yapılanması ile karakterizedir. Diyabetes mellitus, vasküler yetmezlik, basınca bağlı iskemi ve yaşlanma gibi durumlar yara iyileşme sürecini daha da geciktirmekte ve ileri düzey yara bakım sistemlerine olan ihtiyacı artırmaktadır. Bu nedenle günümüzde yalnızca fiziksel koruma sağlayan pasif yara örtülerinin ötesine geçerek yara iyileşmesini aktif şekilde destekleyebilen çok fonksiyonlu yara örtüsü sistemlerinin geliştirilmesine yönelik çalışmalar önem kazanmıştır. Hidrojel bazlı yara örtüleri, yüksek su içerikleri, biyouyumlulukları, ECM benzeri yapıları ve yara bölgesinde nemli ortam sağlayabilmeleri nedeniyle son yıllarda büyük ilgi görmektedir. Farklı hidrojel biyomalzemeleri arasında gelatin methacryloyl (GelMA), fotokürlenebilir yapısı, ayarlanabilir fizikokimyasal özellikleri ve biyolojik fonksiyonelliği sayesinde özellikle dikkat çekmektedir. GelMA hidrojelleri; hücre adezyonu, proliferasyonu, migrasyonu ve besin difüzyonu için uygun mikroçevreler sağlarken aynı zamanda fotopolimerizasyon yöntemleri ile yapısal kontrol imkânı sunmaktadır. Yapısal biyomalzemelere ek olarak biyolojik aktif moleküllerin yara örtüsü sistemlerine dahil edilmesi, terapötik etkinliğin artırılması açısından önemli bir yaklaşım olarak değerlendirilmektedir. Biyoaktif peptitler; biyobozunabilirlikleri, düşük immünojenisite göstermeleri ve doku rejenerasyonunda rol alan çeşitli hücresel süreçleri düzenleyebilmeleri nedeniyle rejeneratif tıp uygulamalarında önemli bir yere sahiptir. Ayrıca bazı peptit sekansları kendi kendine organize olabilme özelliği göstererek doğal ECM yapısına benzer nanofibriler ağlar oluşturabilmektedir. Bu çalışmada kronik yara tedavisine yönelik çok fonksiyonlu peptit bazlı bir hidrojel yara örtüsü sistemi geliştirilmiştir. Çalışma kapsamında GelMA, kendi kendine organize olabilen IVFK (İzolösin-Valin-Fenilalanin-Lizin) tetrapeptidi ve biyoaktif peptit kompleksi GHK-Cu bir araya getirilerek biyolojik olarak aktif, yapısal destek sağlayabilen ve üç boyutlu yazdırılabilir özellikte bir hidrojel sistem tasarlanmıştır. Çalışmanın temel amacı, kronik yaraların birden fazla patolojik özelliğini aynı anda hedefleyebilen yeni nesil bir yara örtüsü platformu geliştirmektir. Kendi kendine organize olabilen IVFK peptidi, hidrojel sistemin hem yapısal hem de biyolojik özelliklerini geliştirmek amacıyla hidrojel matriksine dahil edilmiştir. IVFK, fizyolojik koşullar altında hidrofobik ve elektrostatik etkileşimler aracılığıyla spontan şekilde nanofibriler yapılar oluşturabilmektedir. Bu ECM benzeri nanoyapılar hücre adezyonu, proliferasyonu ve migrasyonunu desteklerken aynı zamanda hidrojel ağ yapısının organizasyonuna ve stabilitesine katkıda bulunmaktadır. Sistemin rejeneratif kapasitesini artırmak amacıyla GHK-Cu biyolojik aktif terapötik bileşen olarak hidrojel sistemine dahil edilmiştir. GHK-Cu'nun anjiyogenezi desteklediği, antioksidan ve antiinflamatuvar etki gösterdiği, kolajen sentezini artırdığı ve ECM yeniden yapılanmasını düzenlediği bilinmektedir. Literatürde yer alan çalışmalar, GHK-Cu'nun fibroblast proliferasyonunu ve doku rejenerasyonunu desteklediğini göstermektedir. Bu özellikleri nedeniyle GHK-Cu kronik yara iyileşmesi açısından umut verici bir terapötik ajan olarak değerlendirilmektedir. Bununla birlikte serbest halde bulunan GHK-Cu'nun fizyolojik koşullar altında hızlı difüzyona uğraması ve sınırlı stabilite göstermesi uzun süreli terapötik etkinliğini azaltabilmektedir. Bu sınırlamaların giderilmesi amacıyla GHK-Cu, çift emülsiyon çöktürme yöntemi kullanılarak polivinil alkol (PVA) ve polikaprolakton (PCL) bazlı mikropartiküller içerisine enkapsüle edilmiştir. Bu sistemde PVA hidrofiliklik ve biyouyumluluk sağlarken, PCL ise yavaş bozunabilen hidrofobik bariyer görevi görerek kontrollü ve sürdürülebilir salım davranışına katkıda bulunmaktadır. Hidrojel biyomürekkebi; GelMA, IVFK peptidi, GHK-Cu yüklü mikropartiküller ve lityum fenil-2,4,6-trimetilbenzoilfosfinat (LAP) fotobaşlatıcısının bir araya getirilmesiyle hazırlanmıştır. Elde edilen formülasyon, Digital Light Processing (DLP) tabanlı üç boyutlu yazdırma uygulamaları için homojenlik, yazdırılabilirlik ve yapısal stabilite açısından optimize edilmiştir. Yara örtüsü yapılarının üretimi DLP tabanlı üç boyutlu yazdırma teknolojisi kullanılarak gerçekleştirilmiştir. DLP, yüksek çözünürlükte hidrojel yapıların hızlı şekilde üretilebilmesini sağlayan vat fotopolimerizasyon tabanlı bir eklemeli üretim yöntemidir. Ekstrüzyon tabanlı biyoyazdırma yöntemlerine kıyasla DLP teknolojisi, biyomoleküller üzerinde oluşabilecek kesme stresini azaltmakta ve karmaşık yapılı hidrojel sistemlerinin hassas şekilde üretilmesine olanak sağlamaktadır. Ayrıca DLP yöntemi sayesinde gözenek yapısı, geometri ve iç mimari yüksek hassasiyetle kontrol edilebilmekte; böylece oksijen difüzyonu, besin taşınımı, hücresel infiltrasyon ve doku rejenerasyonu açısından uygun mikroçevreler oluşturulabilmektedir. Son yıllarda hidrojel üretim teknolojilerindeki gelişmeler, farklı yara tiplerine uygun hasta spesifik ve yara spesifik yara örtülerinin geliştirilmesine odaklanmaktadır. Bu açıdan DLP tabanlı biyoyazdırma teknolojisi; hızlı üretim kapasitesi, yüksek yapısal hassasiyeti ve özelleştirilebilir hidrojel yapılar oluşturabilme yeteneği sayesinde önemli avantajlar sunmaktadır. Peptit bazlı biyomalzemelerin DLP biyoyazdırma teknolojisi ile birleştirilmesi, kronik yara tedavisine yönelik yeni nesil kişiselleştirilmiş yara örtüsü platformlarının geliştirilmesi açısından umut verici bir yaklaşım oluşturmaktadır. Geliştirilen hidrojel yara örtüsü sistemi; Fourier dönüşümlü kızılötesi spektroskopisi (FTIR), taramalı elektron mikroskobu (SEM), enerji dağılımlı X-ışını spektroskopisi (EDS), yüksek performanslı sıvı kromatografisi (HPLC) ve kütle spektrometrisi (MS) gibi çeşitli analiz yöntemleri ile karakterize edilmiştir. Hidrojellerin mekanik ve fizikokimyasal özellikleri ise reolojik analizler, kompresyon testleri, şişme çalışmaları, bozunma analizleri ve in vitro salım deneyleri ile değerlendirilmiştir. Geliştirilen sistemin biyolojik performansı, L929 fibroblast hücreleri kullanılarak gerçekleştirilen in vitro sitouyumluluk ve scratch yara iyileşme analizleri ile incelenmiştir. Hücre canlılığı çalışmaları hidrojel sistemlerinin uygun biyouyumluluk özellikleri sergilediğini gösterirken, scratch yara iyileşme testleri hidrojel sistemlerinin hücre migrasyonu ve yara kapanması üzerindeki etkilerini değerlendirmek amacıyla gerçekleştirilmiştir. Geliştirilen çok fonksiyonlu hidrojel sistemin, kronik yaraların çeşitli patolojik özelliklerini aynı anda hedeflemesi amaçlanmıştır. Hidrate GelMA matriksi yapısal destek sağlarken yara bölgesinde nemli ortam oluşturmaktadır. Kendi kendine organize olabilen IVFK peptidi ise hücresel etkileşimleri destekleyen ECM benzeri nanofibriler yapılar oluşturmaktadır. Enkapsüle edilmiş mikropartiküllerden kontrollü şekilde salınan GHK-Cu'nun ise anjiyogenezi, kolajen birikimini, fibroblast aktivitesini ve inflamatuvar regülasyonu desteklemesi beklenmektedir. Bu çalışma, IVFK bazlı kendi kendine organize olabilen peptit hidrojellerin, PVA/PCL ile enkapsüle edilmiş GHK-Cu sistemi ile birlikte DLP tabanlı üç boyutlu yazdırılmış yara örtüsü platformunda bir araya getirildiği ilk çalışmalardan biri olma özelliği taşımaktadır. Peptit mühendisliği, hidrojel biyomalzemeleri, kontrollü salım sistemleri ve ileri biyoyazdırma teknolojilerinin birleştirildiği bu çalışma ile gelecekteki rejeneratif tıp ve kronik yara tedavisi uygulamaları için umut verici, çok fonksiyonlu ve kişiselleştirilebilir bir yara örtüsü stratejisi sunulmaktadır.

Chronic wounds represent a major healthcare challenge due to their prolonged healing duration, impaired tissue regeneration, susceptibility to infection, and significant socioeconomic burden. Unlike acute wounds, chronic wounds are characterized by persistent inflammation, excessive oxidative stress, impaired angiogenesis, elevated proteolytic activity, and disrupted extracellular matrix (ECM) remodeling, all of which interfere with the normal wound healing process. The delay in chronic wound healing is observed in conditions such as diabetes mellitus, vascular insufficiency and pressure-induced ischemia. Old age also further contributes to this delay. Therefore, the need for advanced therapeutic wound care systems is increasing. As a result, there is a growing demand for multifunctional wound dressing platforms which are capable of actively supporting tissue regeneration rather than merely providing passive wound coverage. Hydrogel-based wound dressings have high-water content, which provides a moist wound environment and are biocompatible. Along with these, their ECM-like structure has attracted considerable attention in recent years for effective wound healing. Gelatin methacryloyl (GelMA), between various hydrogel biomaterials, is a considerably promising candidate because of its photocrosslinkable nature, tunable physicochemical properties and biological availability. GelMA-based hydrogels further provide suitable environments for cell adhesion, proliferation and migration, which are crucial for wound healing. At the same time, enabling of structural control through photopolymerization processes makes it a multi-functional and effective biomaterial. In addition to structural biomaterials, incorporation of bioactive molecules has become an important strategy for enhancing the therapeutic performance of wound dressings. Bioactive peptides are especially attractive for regenerative medicine applications due to their biodegradability, low immunogenicity, and ability to regulate various cellular activities involved in tissue repair. Furthermore, certain peptide sequences possess self-assembling characteristics that enable the formation of nanofibrous structures resembling the architecture of the native ECM. In this study, a multifunctional peptide-based hydrogel wound dressing system was developed for chronic wound treatment by integrating GelMA with the self assembling tetrapeptide IVFK (Ile-Val-Phe-Lys) and the bioactive peptide complex GHK-Cu. The primary objective of the study was to develop a biologically active, structurally supportive, and three-dimensionally printable hydrogel system capable of simultaneously addressing multiple pathological characteristics of chronic wounds. The self-assembling peptide IVFK was incorporated into the hydrogel matrix to improve both structural and biological properties of the system. IVFK is capable of spontaneously forming nanofibrous structures under physiological conditions through hydrophobic and electrostatic interactions. These ECM-mimicking nanostructures support cellular attachment, proliferation, and migration while contributing to the overall stability and organization of the hydrogel network. To further enhance the regenerative potential of the system, GHK-Cu was incorporated as a bioactive therapeutic component. GHK-Cu is famous for its antioxidant, angiogenic and anti-inflammatory properties. It also regulates ECM. GHK-Cu promotes collagen synthesis proved by studies in recent years. They have also demonstrated that GHK-Cu promotes collagen synthesis, fibroblast proliferation, as well as tissue remodeling and wound regeneration. Overall, these properties make it a promising therapeutic candidate to be used for chronic wound healing applications. The long-term therapeutic effectiveness and activity of GHK-Cu may become limited if it is found in free form, which could make it diffuse rapidly and unstable in ECM. In order to overcome these restrictions, GHK-Cu was encapsulated inside poly(vinyl alcohol) (PVA) and polycaprolactone (PCL)-based microparticles via a double emulsion precipitation technique. In this system, PVA contributes to hydrophilicity and biocompatibility, while PCL acts as a slowly degrading hydrophobic barrier capable of enabling a controlled and sustained release behavior for GHK-Cu. The hydrogel bioink was prepared by combining GelMA, IVFK peptide, GHK-Cu loaded microparticles, and lithium phenyl-2,4,6-trimethylbenzoylphosphinate (LAP) photoinitiator. The resulting formulation was optimized in terms of homogeneity, printability, and structural stability for Digital Light Processing (DLP)-based 3D printing applications. Fabrication of the wound dressing constructs was performed using DLP 3D printing technology. DLP is a vat photopolymerization-based additive manufacturing method capable of producing highly precise hydrogel constructs with rapid fabrication speed and high printing resolution. Compared with extrusion-based bioprinting approaches, DLP minimizes shear stress during fabrication and enables the production of structurally complex hydrogel systems containing sensitive biomolecules. Furthermore, DLP technology allows precise control over pore architecture, geometry, and internal construct organization, all of which are important for oxygen diffusion, nutrient transport, cellular infiltration, and tissue regeneration during wound healing. Recent advances in hydrogel fabrication increasingly emphasize the importance of developing patient-specific and wound-specific wound dressing systems with controllable structural properties tailored to different wound environments. In this regard, DLP-based bioprinting provides significant advantages due to its rapid fabrication capability, high structural precision, and ability to produce customized hydrogel constructs with tunable geometries and internal architectures. The integration of peptide-based biomaterials with DLP bioprinting technology therefore represents a promising strategy for the development of next-generation personalized wound dressing platforms for chronic wound treatment applications. The developed hydrogel wound dressing system was characterized using various analytical techniques, including Fourier-transform infrared spectroscopy (FTIR), scanning electron microscopy (SEM), energy-dispersive X-ray spectroscopy (EDS), high-performance liquid chromatography (HPLC), and mass spectrometry (MS). Mechanical and physicochemical properties of the hydrogels were further evaluated through rheological analysis, compression testing, swelling analysis, degradation studies, and in vitro release experiments. The biological performance of the developed system was investigated through in vitro cytocompatibility and scratch wound healing analyses using L929 fibroblast cells. Cell viability studies demonstrated favorable cytocompatibility profiles of the hydrogel systems, while scratch wound healing assays were performed to evaluate their effects on fibroblast migration behavior associated with tissue regeneration and wound closure processes. The developed multifunctional hydrogel system aims to simultaneously address several pathological characteristics of chronic wounds. The hydrated GelMA matrix provides structural support and maintains a moist wound environment, while the self assembling IVFK peptide contributes ECM-mimicking nanofibrous architectures capable of supporting cellular interactions. Sustained release of GHK-Cu from encapsulated microparticles is expected to promote angiogenesis, collagen deposition, fibroblast activity, and inflammatory regulation throughout the healing process. To the best of our knowledge, this study represents one of the first examples of integrating IVFK-based self-assembling peptide hydrogels with PVA/PCL encapsulated GHK-Cu within a DLP-printed wound dressing platform for chronic wound treatment applications. By combining peptide engineering, hydrogel biomaterials, controlled release systems, and advanced biofabrication technologies, this work proposes a promising multifunctional and personalized wound dressing strategy for future regenerative medicine and chronic wound healing applications.
