Özet
Bu araştırma, tiroidektomi ameliyatı geçiren hastaların ameliyat sonrası dönemde yaşadıkları sorunların belirlenmesi amacıyla tanımlayıcı olarak yapılmıştır. Araştırma, 22 Mayıs 2015 – 24 Haziran 2016 tarihleri arasında Gazi Üniversitesi Sağlık Araştırma ve Uygulama Merkezi Genel Cerrahi Kliniklerinde benign tiroid hastalık nedeniyle ilk kez tiroidektomi ameliyatı geçiren 60 hasta ile yürütülmüştür. Verilerin toplanmasında araştırmacı tarafından literatürden yararlanılarak hazırlanan ve iki bölümden oluşan veri toplama formu kullanılmıştır. Veri toplama formu, ameliyat sonrası 1. gün hastalarla yüz yüze görüşülerek, 1., 2., 3. ve 4. haftalarda ise telefonla izlem yapılarak uygulanmıştır. Verilerin değerlendirilmesinde tanımlayıcı istatistikler (sayı, yüzde, ortalama, standart sapma), ShaphiroWilk, Ki kare, Fisher Exact, Olabilirlik oranı, Freidman ve Cochran Q testleri kullanılmıştır. Araştırmada, ameliyat sonrası 1. günde hastaların tamamı ağrı ve çalışma-eğlenme, %98,3'ü yeme-içme, %96,7'si kişisel hijyen-giyim, %95,0'ı solunum, %93,3'ü hareket, %40,0'ı uyku ve %10,0'ı beden imajı ile ilgili sorunlar yaşamıştır. Hastaların izlemlere göre en çok yaşadıkları sorunlar ameliyat sonrası 1. haftada ağrı ve çalışma – eğlenme; 2. haftada çalışma – eğlenme ve iletişim; 3. haftada çalışma – eğlenme, beden imajı ve iletişim; 4. haftada ise hareket, çalışma – eğlenme ve beden imajı olarak belirlenmiştir. Yapılan istatistiksel değerlendirmede; hastaların ameliyat sonrası 1. gün, 1., 2., 3. ve 4. haftalarda yaşadıkları tüm sorunların zaman içinde anlamlı düzeyde azaldığı (p<0.05) saptanmıştır. Araştırmadan elde edilen sonuçlara göre, hastaların ameliyat sonrası 1. gün ve 1. haftada sorun yaşama oranlarının fazla olduğu, ameliyat sonrası 2., 3. ve 4. haftalarda yaşanan sorunların oranlarının giderek azaldığı belirlenmiştir. Araştırma sonuçları doğrultusunda, hastaların ameliyat sonrası yaşayabilecekleri sorunlar ve erken taburculuk sonrası bu sorunları çözümleyebilmelerine ilişkin telefonla izlem yapılması ve danışmanlık hizmeti verilmesi önerilmektedir.

The aim of this descriptive study was to determine problems experienced by patients after thyroidectomy. The study was carried out between May 22nd, 2015 and June 24th, 2016 in General Surgical Clinics of Gazi University Health Research and Practice Centre in Ankara. This study included 60 patients undergoing thyroidectomy for the first time for benign thyroid diseases. The data collection tool consists of two questionnaires was developed by the researcher based on a literatary approach. Questionnaires were applicated to the patients on postoperative 1st day by face to face and tele-follow up visits on 1st, 2nd, 3rd and 4th weeks. Descriptive statistical methods (frequency, percentage, mean, standart deviation) and Shaphiro Wilk, Chi Square, Fisher Exact, Likelihood Ratio, Freidman and Cochrane Q tests were used in evaluating the data. According to the results of the study, it was found that on the postoperative 1st day, patients experienced problems related pain (100,0%), work&fun (100,0%), nutrition (98,3%), individual hygiene&clothing (96,7%), breathing (95,0%), physical activity (93,3%), sleeping (40,0% ) and body image (10,0%). In accordance with monitoring results of the study, patients mostly suffered from pain and work&fun on the first week; work&fun and communication on the second week; work&fun, body image and communication on the third week; and physical activity, work&fun and body image on the fourth week. Depending on the statistical analyses; all experienced problems on the postoperative first day, 1st, 2nd, 3rd, and 4th weeks were significantly (p<0.05) declined. According to the results the ratio of problems which experienced by patients on the postoperative 1st day and 1st week was higher than other follow-ups. On the contrary the ratio of problems which experienced by patients on 2nd, 3rd and 4th weeks were gradually reduced. With regard to findings of this study, we suggest tele-follow up and consultancy to the patients for resolving the problems experienced postoperatively and early after discharge.
