Özet
AMAÇ: Prematüre döneminde optik koherens tomografi ile tespit edilen maküla ödeminin 1.2. ve 3. yaştaki refraksiyon üzerine etkisini saptamak. MATERYAL METOD: Ocak 2013-Ocak 2016 tarihleri arasında Antalya Eğitim ve Araştırma Hastanesi prematüre retinopatisi tanı ve tedavi merkezinde takip ve tedavi yapılan bebeklerin dosyaları geriye dönük incelendi. Çalışma kapsamına prematüre döneminde Optik koherens tomografi çekilen 217 bebeğin 434 gözü dahil edildi. Optik koherens tomografide tespit edilen ödem; ödem yok, maküla konturunu bozmayan Tip A ödem ve makula konturunu bozan kistoid maküler Tip B ödem olarak üç grupta incelendi. Çalışmaya dahil edilen bebeklerin 1., 2., ve 3. yaş refraksiyon ölçümleri kaydedildi. BULGULAR: Bir yaşta ödem olmayan bebeklerin ortalama sferik ekivalan değerleri -0,57±2,11 olarak görülürken Tip A ödem tespit edilen bebeklerde 0,91±1,29; kistoid maküler Tip B ödem görülen bebeklerde 0,209±1,81 olarak tespit edildi. Tip A ödem olan bebeklerin daha hipermetropik sferik ekivalan değerlerine sahip olduğu görüldü ve istatistiksel olarak anlamlı saptandı (p: 0,001). İki yaşta ise ödem olmayan bebeklerde ortalama sferik ekivalan değerleri 1,05±2,63 saptanırken Tip A ödem olan bebeklerde 1,76±1,51; Tip B ödem olanlarda 1,41±2,99 tespit edildi. Bir yaş bulgulara benzer şekilde Tip A ödem olan bebeklerde daha hipermetropik sferik ekivalan değerlerine rastlandı fakat istatistiksel olarak anlamlı saptanmadı. Üç yaşta gözde ödem olmayan bebeklerde ortalama sferik ekivalan 2,32±1,24 iken Tip A ödem olan bebeklerde 1,02±1,47; kistoid maküler Tip B ödem olan bebeklerde 2,27±1,02 olarak tespit edildi. Tip A ödem olan gözlerde sferik ekivalan değerlerinde miyopa kayma olduğu görüldü ve istatistiksel olarak anlamlı tespit edildi (p: 0,019). SONUÇLAR: Prematüre döneminde tespit edilen maküla ödeminin emetropizasyon sürecini etkileyerek 1,2 ve 3 yaştaki refraksiyon üzerine etkisi olduğu tespit edildi.

PURPOSE: The purpose of the study was to determine the effect of macular edema detected through optical coherence tomography in the premature period on refraction at 1, 2 and 3 years of age. MATERIAL AND METHOD: The files of infants who were followed-up and treated at the premature retinopathy diagnosis and treatment center of Antalya Training and Research Hospital from January 2013 to January 2016 were reviewed retrospectively. 434 eyes from 217 infants who underwent optical coherence tomography in the premature period were included in the study. The edema detected through optical coherence tomography were divided into three groups which were absence of edema, Type A edema not impairing the macular contour and Type B cystoid macular edema impairing foveal contour. Refraction measurements of infants included in the study at 1, 2 and 3 years of age were recorded. RESULTS: At 1 years of age, the mean spherical equivalent value of infants without edema was found to be -0,57±2,11; while it was 0,91±1,29 for infants with Type A edema and 0,209±1,81 for infants with Type B edema. Infants with Type A edema had more hypermetropic spherical equivalent values, which was statistically significant (p: 0,001). At 2 years of age, the mean spherical equivalent value of infants without edema was found to be 1,05±2,63; whereas it was 1,76±1,51 for infants with Type A edema and 1,41±2,99 for infants with Type B edema. Similar to the infants at 1 years of age, infants at 2 years of age in infants with Type A edema had more hypermetropic spherical equivalent values; however, this was not statistically significant. At 3 years of age, the mean spherical equivalent value of infants without edema was 2,32±1,24; while it was 1,02±1,47 for infants with Type A edema and 2,27±1,02 for infants with Type B edema. The spherical equivalent values of those eyes with Type A edema were observed to have a shift to myopia, which was statistically significant (p: 0,019). CONCLUSIONS: Macular edema detected in the premature period was found to influence the emmetropization process and have an impact on refraction at 1, 2 and 3 years of age.
