Özet
Bu çalışmanın amacı down sendromu tanısı almış çocukların alacakları hippoterapi uygulamasının fonksiyonel mobilite ve denge üzerinde etkili olup olmadığını incelemek üzere yapılan deneysel bir çalışmadır. Katılımcıların alınma ve alınmama kriterleri değerlendirilerek, 17 kişi kontrol ve 17 kişi deney (hippoterapi) grubu olmak üzere randomize olarak iki gruba ayrılmıştır. Kontrol grubuna 6 hafta boyunca, haftada 3 gün denge egzersizlerini içeren fizyoterapi uygulanmıştır. Hippoterapi grubuna ise denge egzersiz programına ek olarak, 6 hafta boyunca haftada 1 seans, 20 – 30 dk at yardımı ile denge egzersizleri uygulanmıştır. 6 haftalık uygulamanın sonucunda Pediatrik Berg Denge Ölçeği, Zamanlı Kalk ve Yürü Testi, Pediatrik Fonksiyonel Bağımsız Ölçeği tekrar uygulanarak veriler elde edilmiştir. Çalışmaya dahil edilen katılımcıların %44,1'i erkek, %55,9'u kız çocuklarından oluşmaktadır. Hippoterapi grubunun %76,5'i kız, %23,5'i erkek ve kontrol grubunun %35.3'ü kız, %64.7'si erkek çocuktan oluşmaktadır ve cinsiyet değişkeni gruplara göre farklılaşmaktadır. Pediatrik Berg Denge Ölçeği (t=2,370; p<0,05) ve Pediatrik Fonksiyonel Bağımsızlık Ölçeğinde (t=2,487; p<0,05) istatistiksel olarak anlamlı fark bulunmuştur. Zamanlı Kalk ve Yürü Testinde istatistiksel olarak anlamlı fark bulunmamıştır (p˃0,05). Çalışmaya katılan her iki grupta da Pediatrik Berg Denge Ölçeği ön test- son test delta fark puanlarında (t=-5,278; t=-3,887; p<0,01) istatistiksel olarak anlamlı fark bulunmuştur. Çalışmaya katılan katılımcılardan, hippoterapi grubunda Pediatrik Fonksiyonel Bağımsızlık Ölçeği ön test- son test delta fark puanlarında istatistiksel olarak anlamlı fark bulunmuştur (t=-2,443; p<0,05). Kontrol grubunda Pediatrik Fonksiyonel Bağımsızlık Ölçeği ön test- son test delta fark puanlarında ise istatistiksel olarak anlamlı fark bulunmamıştır (t=-1,376; p>0,05). Her iki grupta da Zamanlı Kalk ve Yürü Ölçeği ön test- son test delta fark puanlarında istatistiksel olarak anlamlı fark bulunmuştur (t=3,231; t=2,229; p<0.01). Hippoterapi grubunda Pediatrik Fonksiyonel Bağımsızlık Ölçeği ön test- son test delta fark puanlarında istatistiksel olarak anlamlı fark bulunmuştur (t=2,053; p<0.05). Elde edilen bulgular down sendromlu çocuklarda uygulanan hippoterapinin DS'li çocuklarda fonksiyonel mobilite ve dengeyi arttıracağı yönündeki hipotezi destekler niteliktedir. Yapılan çalışma sonucunda Down sendromlu çocuklarda ev egzersiz programı ve hippoterapinin denge ve fonksiyonel mobiliteyi arttırdığı görülmüştür. Deney ve kontrol grubundaki bireylerin denge gelişiminin arttığı görülmüştür. Hippoterapi alan grubun denge gelişiminin kontrol grubundan daha fazla olduğu belirlenmiştir.Deney ve kontrol grubundaki bireylerin fonksiyonel mobilitelerinin arttığı ve deney grubunun kontrol grubundan daha fazla gelişim gösterdiği bulunmuştur. The aim of this study is an experimental study to examine whether the hippotherapy application of children diagnosed with Down syndrome has an effect on functional mobility and balance. Considering the inclusion and exclusion criteria of the participants, they were randomly divided into two groups, 17 as control and 17 as the experiment (hippotherapy) group. Physiotherapy including balance exercises was applied to the control group for 6 weeks, 3 days a week. In the hippotherapy group, in addition to the balance exercise program, balance exercises were applied with the aid of a horse for 1 session per week for 20-30 minutes for 6 weeks. At the end of the 6-week application, the data were obtained by applying the Pediatric Berg Balance Scale, Timed Get Up and Go Test, and the Pediatric Functional Independent Scale again. 44.1% of the participants included in the study are boys and 55.9% are girls. The hippotherapy group consists of 76.5% girls, 23.5% boys and 35.3% girls and 64.7% boys of the control group, and the gender variable differs according to the groups. A statistically significant difference was found in the Pediatric Berg Balance Scale (t = 2.370; p <0.05) and the Pediatric Functional Independence Scale (t = 2.487; p <0.05). There was no statistically significant difference in Timed Get Up and Go Test (p˃0.05). A statistically significant difference was found in the Pediatric Berg Balance Scale pretest-posttest delta difference scores (t = -5.278; t = -3.887; p <0.01) in both groups participating in the study. Among the participants in the study, a statistically significant difference was found in the delta difference scores of the Pediatric Functional Independence Scale pre-test-post-test in the hippotherapy group (t = -2.443; p <0.05). No statistically significant difference was found in the Pediatric Functional Independence Scale pretest-posttest delta difference scores in the control group (t = -1.376; p> 0.05). A statistically significant difference was found in the Timed Get Up and Go Scale pretest-posttest delta difference scores in both groups (t = 3.231; t = 2.229; p <0.01). In the hippotherapy group, a statistically significant difference was found in the Pediatric Functional Independence Scale pretest-posttest delta difference scores (t = 2.053; p <0.05). The findings obtained support the hypothesis that hippotherapy applied in children with Down syndrome will increase functional mobility and balance in children with DS. As a result of the study, it was observed that home exercise program and hippotherapy increase balance and functional mobility in children with Down syndrome. It was observed that the balance development of the individuals in the experimental and control groups increased. It was determined that the balance development of the hippotherapy group was more than the control group. It was found that the functional mobility of the individuals in the experimental and control groups increased and the experimental group showed more improvement than the control group.
