Anticipatory postural control in individuals with stroke: Role of asymmetries and focused training

Impaired postural control is commonly observed in individuals with stroke. Although postural control has been extensively studied in people with stroke, information about the ability of individuals with stroke to generate anticipatory postural adjustments for control of posture is limited. Moreover, no information exists on whether anticipatory postural adjustments in individuals with unilateral stroke could be enhanced with specialized training. In this dissertation, we investigated the anticipatory postural control in individuals with stroke and the effect of focused training on improvement of balance control in people with stroke. Three studies were conducted. In the first study (Chapter 3), we studied the differences in postural control between people with stroke and healthy controls. Nine individuals with stroke and five control subjects were required to stand on the force platform and use their unaffected upper extremity to induce self-initiated perturbations or counteract external perturbations. The anticipatory muscle activation of the legs and trunk muscles in individuals with stroke was mainly seen in the muscles on the unaffected side of the body. However, control group subjects generated early muscle activation on both sides of the body. The outcome of the study revealed that individuals with stroke have impaired ability to generate anticipatory postural adjustments. The study provided a background for investigating a possibility of a focused training paradigm to improve anticipatory postural adjustments in individuals with stroke. In the second study (Chapter 4), we investigated the effect of a single session of targeted exercises on anticipatory postural adjustments during self-initiated and external perturbations. Individuals with stroke (N=9) participated in a single session of exercises xii involving pushing and stopping a medicine ball hanging from the ceiling. After training, individuals with stroke demonstrated a significantly earlier onset of anticipatory muscle activity as compared to pre-training. Furthermore, people with stroke showed an improvement in the muscle activation timing on both sides of the body. In the third study (Chapter 5), we investigated the effect of multi-session targeted training on postural control of individuals with stroke. Six participants were randomly divided into two training groups of two-week durations. Experimental group performed exercises involving pushing and stopping a medicine ball hanging from the ceiling. The control group performed home-based exercises. Individuals with stroke included in the experimental group demonstrated a tendency of improvement of anticipatory activities in the muscles on both sides of the body. Furthermore, there was a tendency of improvement of balance after the training in the experimental group.
