Özet

Amaç: Araştırmanın amacı, Amyotrofik Lateral Skleroz (ALS) hastalığının doğrudan ve dolaylı maliyetlerini incelemek ve bu maliyetlerin hangi unsurlardan oluştuğunu tespit etmektir. Yöntem: Araştırmada nitel yöntem kullanılmış, ALS hastalarının tedavi ve bakım maliyetlerini belirlemek için 29 hasta ve hasta yakını ile yüzyüze görüşülmüştür. Veriler, hastaların geriye dönük ve kesitsel harcamaları ve kullanılan malzeme, cihazlar gibi doğrudan maliyetler ile hastalık nedeniyle kaybedilen iş gücü gibi dolaylı maliyetleri anket formu ile toplanmıştır. Bulgular: Araştırma grubundaki hastaların çoğunluğu erkek olup, en sık 66-79 yaş aralığında görülmüştür. Hastaların %51,7'sinde en az bir kronik hastalık bulunmakta ve toplam doğrudan tıbbi maliyetleri önemli ölçüde artırmaktadır. Tedavi ve bakım maliyetlerinin büyük bir kısmının doğrudan maliyetlerden oluştuğu ve bu maliyetlerin hastalık ilerledikçe arttığı belirlenmiştir. ALS hastasının bir yıllık maliyetinin 1.173,13 USD ile 95.159,63 USD arasında değiştiği ve toplam maliyetin %31,6'sının doğrudan, %68,3'ünün ise dolaylı maliyetlerden oluştuğu tespit edilmiştir. Araştırma bulgularına göre, 29 hastada doğrudan tıbbi maliyet 153.249,12 USD olup, bu maliyetin %51,7'si cepten ve %48,3'ü kamu tarafından karşılanmaktadır. Doğrudan tıbbi olmayan maliyet 63.478,54 USD olup, bu maliyetin %99,9'u cepten ve %0,1'i kamu tarafından karşılanmaktadır. Dolaylı maliyetler ise 467.039,99 USD olup, bu maliyetlerin %83,3'ü cepten ve %16,7'si kamu tarafından karşılanmaktadır. Sonuç: ALS hastalarının tedavi ve bakım maliyetleri aileler ve kamu yönünden önemli bir yük oluşturduğu belirlenmiştir. ALS hastalarının mali açıdan kamu tarafından desteklenmesi ve hastalığın tedavisinde kullanılan malzeme ve cihaz maliyetlerinin düşürülmesi için farklı sağlık politikalarının geliştirilmesi önerilir.

ABSTRACT Objective: The aim of this study is to examine the direct and indirect costs of Amyotrophic Lateral Sclerosis (ALS) and to identify the components of these costs. Method: A qualitative research method was used in this study. Face-to-face interviews were conducted with 29 patients and their caregivers to determine the treatment and care costs of ALS. Data were collected using a survey form, capturing patients' retrospective and cross-sectional expenditures, as well as direct costs such as materials and devices used, and indirect costs like lost work productivity due to the illness. Results: The majority of the patients in the study group were male, with the most common age range being 66-79 years. It was found that 51.7% of the patients had at least one chronic disease, which significantly increased the total direct medical costs. The study determined that a significant portion of the treatment and care costs consisted of direct costs, which increased as the disease progressed. The annual cost per ALS patient ranged from 1,173.13 USD to 95,159.63 USD, with 31.6% of the total costs being direct and 68.3% being indirect. The direct medical costs for 29 patients amounted to 153,249.12 USD, with 51.7% of these costs being out-of-pocket expenses and 48.3% covered by public funds. The direct non-medical costs totaled 63,478.54 USD, with 99.9% paid out-of-pocket and 0.1% covered by public funds. Indirect costs amounted to 467,039.99 USD, with 83.3% being out-of-pocket expenses and 16.7% covered by public funds. Conclusion: The study found that the treatment and care costs for ALS patients impose a significant financial burden on families and the public. It is recommended that ALS patients receive more financial support from the public sector and that different health policies be developed to reduce the costs of materials and devices used in the treatment of the disease.
