Özet

Devletimizin tarihsel geçmişine baktığımızda toprak mücadelesine bağlı olarak, sağlık olgusu başta olmak üzere birçok alanda devletimiz sancılı süreçler geçirmiştir. Sağlık kavramı hayatımızda var olan birçok kavram ile etkileşim içindedir. Ülkenin sosyo-ekonomik yapısı; sağlık hizmetinin kalitesini, kültürel yapısı ise; hastalıklara olan bakış açısını şekillendirmektedir. Sağlık olgusu, insanoğlunun var oluşundan itibaren hayatımızda yerini almış bir olgudur. Birey yaşamını sürdürebilmek için mevcut sağlığını dengede tutmak adına, sağlık kavramını sürdürülen hayat diliminde her zaman ilk sırada tutmuştur. Geçmiş çağlardan itibaren insanoğlu bedeninde normal dışı oluşan (psikolojik ve/veya fizyolojik normal dışı durumlar) rahatsızlıklara, hastalıklara, yaralanmalara karşı deneme yanılma yolu ile gerek doğal yöntemler gerekse farklı karışımlarla tedavi etme yoluna gitmiştir. Bu geleneksel yöntemler, sorunları ortadan kaldırdıkça (tedavi) nesilden nesille aktarılarak bilimsel bilgiler ile harmanlanıp günümüze kadar sağlık alanında birçok gelişmenin zeminini oluşturmuştur. Tarihte devletimizin zorlu savaşlar ile mücadelesinde oluşan, yaralı halk ve askerlerin tedavisine ağırlık verilirken, ilerleyen zaman diliminde yoksulluk ve imkansızlığın doğurduğu zorlu salgın hastalıklar ile mücadele de sağlıkta atılan yeni adımlara öncülük etmiştir. Hekimlerimizin yeni sağlık sorunları ile karşılaşması, kendilerini yeni çözümler üretmeye itmiştir. Ülkemizin bugün ki sağlık sistemine bakıldığında teknolojinin önemli gelişmeler kaydetmesine paralel olarak sağlık sistemi ve sağlık hizmetleri oldukça önemli yol kat etmiştir. Ülkemiz elindeki imkanlar dahilinde Avrupa ülkelerindeki sağlık sektöründe yaşanan gelişimi yakından takip etmiş, dışa bağımlılığın kalkması için birçok yeniliği imkanlar dahilinde ülkemize kazandırmıştır. Sağlık hizmetlerinin gelişmesi, tedavisi zor hastalıkların (verem, kolera, kanser, böbrek rahatsızlıkları, görme kayıpları vb.) tedavi imkanlarının yükseltilmesi ve sağlık sektöründe hizmet veren kurumların (hastanelerin), çalışanların (hekim, hemşire, ebe, eczacı vb.) geliştirilerek en üst seviyeye yükseltilmesi için mevcut geleneksel yöntemlere, bilimsel gelişmeler eklenerek ilerleme kaydedilmiştir. Bu çalışma ile Osmanlıdan günümüze dek geçen sürede sağlık sisteminde yer alan kavramların tarihsel süreç içerisinde kaydedilen gelişmeleri ele alınmıştır. Çalışmamız nitel bir çalışma olup, sağlık sisteminin alt başlıkları olan; sağlık, tanı, tedavi, sağlık hizmetleri, hekimlerin yetiştirilmesi, tıp eğitiminin sunulması ve kurum/kuruluşların hizmet kalitesi gibi birçok kavramın geleneksel yöntemlerden, modern gelişmeler ile empoze edilme sürecinin yazılı kaynaklardan derleyerek bir araya getirilmesini amaçlamıştır. Bu bağlam da ülkemiz (Türkiye) genelinde sağlık sektörünün gelişimi ele alınarak sınırlandırılmıştır. Yapılan birçok çalışmanın incelenmesi sonucu elde edilen veriler gösteriyor ki dönemin devlet yöneticisi, sağlık bakanı, görevli çalışan (Hekimler, hemşireler vs.) personellerin iş birliği ile sağlık sektörünün yenilikleri benimsemesi, Avrupa ülkelerindeki gelişmelerin takip edilmesi, sağlık sektörünün resmiyet kazanması açısından kanunların çıkarılması, hızla ilerleyen teknolojik gelişmelerin sektöre dahil edilmesi gibi birçok önemli adımlar ile sağlık sektörü giderek iyileştirilmiştir.

When we look at the historical past of our state, it has gone through painful processes in many areas, especially in the health phenomenon due to the land struggle. The concept of health interacts with many concepts in our lives. The socio economic structure of the country shapes the quality of health services, while its cultural structure shapes the perspective on diseases. The concept of health is a phenomenon that has taken its place in our lives since the existence of human beings. In order to maintain their current health in balance in order to continue their lives, the individual has always kept the concept of health in the first place in the life period they continue. Since the past ages, they have resorted to trial and error methods, both natural methods and different mixtures, to treat abnormal (psychological and/or physiological abnormal conditions) ailments, diseases and injuries in their bodies. As these traditional methods eliminate problems (treatment), they have been transferred from generation to generation and blended with scientific knowledge, and have formed the basis for many developments in the field of health up to the present day. In history, while the treatment of wounded people and soldiers, which occurred in the struggle of our state with difficult wars, was emphasized, in the following period, innovations were taken in health in the fight against difficult epidemic diseases caused by poverty and impossibility. Our physicians' encounter with new health problems has pushed them to produce new solutions. When we look at our country's current health system, the health system and health services have made significant progress in parallel with the significant developments in technology. Our country has closely followed the developments in the health sector in European countries within the possibilities it has, and has brought many innovations to our country within the possibilities it has in order to eliminate external dependency. Progress has been made by adding scientific developments to existing traditional methods in order to develop health services, increase the treatment opportunities of difficult-to-treat diseases (tuberculosis, cholera, cancer, kidney diseases, vision loss, etc.), and improve the institutions (hospitals) providing service in the health sector and raise their employees (physicians, nurses, midwives, pharmacists, etc.) to the highest level. This study examines the developments in the concepts in the health system from the Ottoman Empire to the present day. Our study is a qualitative study and aims to bring together the concepts of health, diagnosis, treatment, health services, training of physicians, provision of medical education and service quality of institutions/organizations, which are the subheadings of the health system, from traditional methods to the process of imposition with modern developments, by compiling them from written sources. In this context, the development of the health system in our country (Turkey) has been addressed and limited. The data obtained as a result of examining many studies show that many important steps have been taken such as the health sector adopting innovations with the cooperation of the state administrator of the period, the minister of health, and the personnel working in charge (physicians, nurses, etc.), following the developments in European countries, enacting laws for the formalization of the health sector, and incorporating rapidly advancing technological developments into the sector.
