Özet
Bu çalışmanın amacı, nöbet şikayetiyle kliniğe getirilen köpek ve kedilerin metabolik toksik ensefalopatinin tanı ve sağaltım aşamasında klinik, laboratuvar, görüntüleme teknikleriyle elektroensefalografi (EEG) kullanımının etkinliği araştırıldı. Olgu çalışmasını 15 kedi ve 5 köpek oluşturdu. Anamnez, klinik ve nörolojik muayeneler ve kan incelemeleri, nöbet geçiren bir hastanın tanısal çalışmalarının temel parçalarıdır. Bu tanısal çalışmalarla ve radyolojik görüntüleme ile EEG analizleriyle olgularda 12 kediye hepatik ensefalopati, 2'sine portosistemik şant ve 1 kediye üremik ensefalopati teşhisi, çalışmayı oluşturan köpek olgularının tamamına ise hepatik ensefalopati teşhisi konuldu. Olguların ilk EEG kayıtları nöbet şikayetiyle geldikten maksimum 48 saat sonra sağaltıma başlanmadan ve ikinci EEG kayıtları sağaltımın 15. gününde tekrarlandı. EEG çekimi 5 kanallı bipolar montaj ve timsah uçlu elektrotlar yardımıyla kafa derisine tutturularak gerçekleştirildi. Alınan kayıtlar hem kalitatif hem kantitatif olarak değerlendirildi. Alınan kayıtlarda epileptiform aktivite (diken dalga, keskin dalga, çoklu diken, yavaş diken), arka plan aktivitesi değerlendirildi. Kantitatif EEG değerlendirmesinde tüm bantların kafatası bölgelerine göre FFT analizi ile spektral güçleri belirlenerek karşılaştırmalı olarak değerlendirildi. Olguların ilk EEG değerlendirmesinde %100'ünde epileptiform aktivite (anormal dalgaların en az ikisi) ve %52'sinde arka plan ritminde azalma ve qEEG değerlendirmesinde baskın delta ve teta güçleri tespit edildi. Sağaltım sonucu yapılan ikinci değerlendirmede olguların %40'ında epileptiform aktivitenin kaybolduğu diğer %60'ında tamamen kaybolmadığı ancak azaldığı ve qEEG değerlendirmesinde delta ve teta güçlerinin yine baskın olduğu ancak sağaltım öncesine göre azaldığı gözlendi. Çalışma sonucunda EEG kullanımının tanı ve sağaltım aşamasında başarı ile kullanılabileceği ve antiepileptik ilaç kullanımının sonlandırılmasında yararlı olabileceği belirlendi.

The aim of this study was to investigate the effectiveness of using clinical, laboratory and imaging techniques and electroencephalography (EEG) in the diagnosis and treatment of metabolic toxic encephalopathy in dogs and cats brought to the clinic with seizure complaints. The case study consisted of 15 cats and 5 dogs. Anamnesis, clinical and neurological examinations, and blood tests are the main parts of the diagnostic work of a patient suffering from seizures. With these diagnostic studies and radiological imaging and EEG analyses, 12 cats were diagnosed with hepatic encephalopathy, 2 had portosystemic shunt and 1 cat was diagnosed with uremic encephalopathy, and all of the dog cases that made up the study were diagnosed with hepatic encephalopathy. The first EEG recordings of the cases were made a maximum of 48 hours after the seizure complaint and before the start of treatment and the second EEG recordings were repeated on the 15th day of treatment. EEG recording was performed by attaching to the scalp with the help of a 5-channel bipolar mount and alligator-tipped electrodes. The records were evaluated both qualitatively and quantitatively. Epileptiform activity (spike wave, sharp wave, multiple spikes, slow spikes) and background activity were evaluated were evaluated with the recordings taken. In the quantitative EEG evaluation, the spectral strengths of all bands according to the skull regions were determined by FFT analysis and evaluated comparatively. Epileptiform activity (at least two abnormal waves) was detected in 100% of the cases in the first EEG evaluation, and a decrease in background rhythm in 52%, and dominant delta and theta spectral power were detected in the qEEG evaluation. In the second evaluation performed as a result of recovery, it was observed that epileptiform activity disappeared in 40% of the cases, and in the other 60% it did not disappear completely, but decreased, and delta and theta spectral power were again dominant in the qEEG evaluation, but decreased compared to before recovery. As a result of the study, it was determined that the use of EEG could be used successfully in the diagnosis and treatment stages and could be useful in ending the use of antiepileptic drugs.
