Özet

Amaç:Prostat cerrahisi, genellikle ileri yaş ve eşlik eden hastalıkları olan erkeklerde gerçekleştirilen bir cerrahidir. Bu hasta grubunda, perioperatif dönemde uygulanacak anestezi yönteminin koagülasyon ve fibrinolizis üzerine etkileri büyük önem taşımaktadır. Literatürde, bu konuda karşılaştırmalı çalışmalar sınırlıdır. Bu çalışmanın amacı, prostat cerrahisi sırasında uygulanan genel ve spinal anestezi yöntemlerinin koagülasyon ve fibrinolizis parametreleri üzerindeki etkilerini karşılaştırmaktır. Yöntem:Bu prospektif, randomize çalışmaya, prostat cerrahisi planlanan 76 hasta dahil edilmiştir. Hastalar randomize olarak genel anestezi (n=38) ve spinal anestezi (n=40) gruplarına ayrılmıştır. Her hastadan operasyon öncesi, operasyon sonrası 1. saat ve 24. saatte PT, aPTT, INR, fibrinojen ve D-dimer düzeyleri ölçülmüştür. Ayrıca, intraoperatif dönemde kan basıncı, kalp hızı ve oksijen satürasyonu gibi hemodinamik parametreler kaydedilmiştir. İstatistiksel analizlerde SPSS 28.0 programı kullanılmış ve p<0.05 anlamlı kabul edilmiştir. Bulgular: Her iki grup arasında yaş, vücut kitle indeksi, ASA skoru ve ek hastalıklar bakımından anlamlı fark saptanmamıştır (p>0.05). Spinal anestezi grubunda, anestezi başlama süresi ve recovery süresi genel anestezi grubuna kıyasla anlamlı derecede daha kısa bulunmuştur (p<0.05). Koagülasyon parametreleri açısından, her iki anestezi grubunda postoperatif dönemde PT, aPTT, INR ve D-dimer düzeylerinde artış gözlenmiştir. Ancak, bu artış gruplar arası karşılaştırmalarda anlamlı farklılık göstermemiştir (p>0.05). Spinal anestezi grubunda yalnızca fibrinojen değerinde operasyon sonrası 1. saatte anlamlı bir artış saptanmıştır (p=0.035). Her iki grupta da D-dimer düzeylerinde postoperatif artış anlamlı bulunmuş, ancak gruplar arasında fark izlenmemiştir. Hemodinamik olarak, gruplar arasında kan basıncı ve kalp hızı açısından anlamlı fark saptanmamış, ancak spinal anestezi grubunda daha stabil seyir izlendiği gözlemlenmiştir. Hipotansiyon ve bradikardi gibi komplikasyonlar genel anestezi grubunda daha sık görülmüşse de bu fark istatistiksel olarak anlamlı değildir (p>0.05). Sonuç: Prostat cerrahisi geçiren hastalarda spinal anestezi, recovery süresini kısaltması ve hemodinamik stabilite sağlaması açısından avantaj sağlamaktadır. Koagülasyon parametreleri üzerinde her iki anestezi yöntemi de benzer etkilere sahip olmakla birlikte, spinal anestezi uygulanan hastalarda postoperatif fibrinojen düzeylerinde anlamlı artış gözlenmiştir. Bu sonuçlar, yüksek riskli hastalarda anestezi yöntemi seçiminde klinik kararları destekleyebilir. Ancak, çalışmanın tek merkezli ve sınırlı örneklemle yapılmış olması, genellenebilirliğini kısıtlamaktadır. Dolayısı ile daha geniş ölçekli ve çok merkezli çalışmalara ihtiyaç vardır.

Objective: Prostate surgery is commonly performed in elderly male patients who often have multiple comorbidities. In this patient population, the choice of anesthetic technique during the perioperative period plays a crucial role in coagulation and fibrinolysis. However, there are limited comparative studies in the literature on this topic. The aim of this study is to compare the effects of general and spinal anesthesia on coagulation and fibrinolysis parameters during prostate surgery. Methods: This prospective, randomized study included 76 patients scheduled for prostate surgery. Patients were randomly assigned to receive either general anesthesia (n=38) or spinal anesthesia (n=40). Coagulation parameters including PT, aPTT, INR, fibrinogen, and D-dimer levels were measured preoperatively, and at the 1st and 24th postoperative hours. Additionally, intraoperative hemodynamic parameters such as blood pressure, heart rate, and oxygen saturation were recorded. Statistical analyses were performed using SPSS 28.0 software, and p<0.05 was considered statistically significant. Results: There were no statistically significant differences between the groups in terms of age, body mass index, ASA scores, or comorbidities (p>0.05). The spinal anesthesia group had significantly shorter anesthesia initiation and recovery times compared to the general anesthesia group (p<0.05). Both groups showed postoperative increases in PT, aPTT, INR, and D-dimer levels; however, these changes were not significantly different between the groups (p>0.05). A significant increase in fibrinogen levels was observed only at the 1st postoperative hour in the spinal anesthesia group (p=0.035). Although both groups had significant postoperative increases in D-dimer levels, intergroup differences were not statistically significant. Hemodynamically, there was no significant difference in blood pressure and heart rate between the groups, though the spinal anesthesia group demonstrated more stable parameters. Hypotension and bradycardia were more common in the general anesthesia group, but this difference was not statistically significant (p>0.05). Conclusion: In patients undergoing prostate surgery, spinal anesthesia appears advantageous due to its shorter recovery time and better hemodynamic stability. While both anesthesia techniques had similar effects on coagulation parameters, spinal anesthesia was associated with a significant postoperative increase in fibrinogen levels. These findings may guide clinical decision-making regarding anesthetic technique in high-risk patients. However, the single-center design and limited sample size of the study may restrict the generalizability of the results. However, larger multicenter studies are needed.
