Özet
AMAÇ: Migren, baş ağrısı atakları (iktal) ve ataklar arası (interiktal) dönemlerin olduğu bir primer baş ağrısı bozukluğudur. İktal ve interiktal fotofobi, migrenle ilişkili yaygın bir semptomdur ve hastaların yaşam kalitesini olumsuz etkileyebilir. Fotofobinin eşlik ettiği migren hastalarında elektrofizyolojik yanıtların değişebileceği düşünülmektedir. Patern uyarımla elde edilen görsel uyarılmış potansiyel (GUP) yanıtlarının bileşenleri; ilk negatif komponent olan N75 görsel korteksten, ilk pozitif komponent olan P100 dorsal ekstrastriat korteksten kaynaklanmaktadır. Görsel uyarılmış potansiyel çalışmalarında elde edilen kortikal yanıtlar, retino-talamo-kortikal duyusal ağlarda eşik değer değişkenliklerine bağlı olarak farklılık gösterebilir. Bu çalışma, değişen luminans değerlerinde elde edilen GUP yanıtlarında migrene bağlı fotofobi şiddeti için objektif bir parametre tanımlamayı amaçlamıştır. YÖNTEM: Bu çalışmaya 24 epizodik migren (EM) ve 20 kronik migren (KM) olmak üzere toplam 44 hasta, 20 sağlıklı katılımcı kontrol grubu (KG) olarak dahil edildi. Hastalar 12 maddelik Utah Fotofobi Semptom Skalası (UPSIS-12), Fotofobi ve Allodini 4'lü Likert skalaları, Allodini Semptom Anketi (ASC-12), Migren Özürlülük Değerlendirme Ölçeği (MIDAS), Baş Ağrısı Etki Ölçeği (HIT-6), Görsel Analog Skala (VAS) ile değerlendirildi. Fotofobi şiddeti, UPSIS-12 skoruna göre ≥30 şiddetli ve <30 hafif olarak derecelendirildi. Dama tahtası uyarısı, iki farklı beyaz kare luminans değerlerinde tasarlandı: düşük luminans (DL:45 cd·m-2) ve yüksek luminans (YL:85 cd·m-2). Epizodik migren hastalarında GUP yanıtları interiktal dönemde kaydedildi. Kronik migren hastalarında interiktal dönemin nadir olması nedeniyle bu hastaların GUP yanıtları baş ağrısız veya baş ağrısı şiddetinin yüksek olmadığı dönemde kaydedildi. BULGULAR: Şiddetli fotofobi skoru olan migrenli katılımcılarda luminans değerlerinden bağımsız olarak P100 latansı hafif fotofobi grubuna kıyasla anlamlı olarak kısa bulundu (DL için p=0,013; YL için p=0,033). Şiddetli fotofobi skoru olan migrenli katılımcılarda DL ve YL kayıtlarında elde edilen N75 latans ölçüm farkı hafif fotofobi grubuna kıyasla anlamlı olarak artmış bulundu (p=0,011). Kronik migren grubunda DL ve YL kayıtlarında elde edilen N75 latans ölçüm farkı KG ve EM grubuna kıyasla anlamlı olarak artmış bulundu (p=0,001). Düşük luminans ve yüksek luminans N75 latans ölçüm farkına etki eden değişkenlerin incelendiği lineer regresyon analizi modelinde UPSIS-12 skorunun ve kronik migren grubunda olmanın anlamlı bir etkisi olduğu saptandı (sırasıyla p=0,006; p=0,044). SONUÇ: Bulgularımız farklı luminans değerlerinde elde edilen GUP yanıtlarının migren ilişkili fotofobinin şiddetine ve kronik migren grubunda olmaya bağlı değişkenlik gösterdiğini saptamıştır. Dorsal ekstrastriat korteks (P100'ün kaynağı) ve ve görsel korteks (N75'in kaynağı) ile ilgili retino-talamo-kortikal duyusal ağların modülasyonu migrenle ilişkili fotofobinin nörobiyolojisinde rol oynuyor olabilir. Kronik migren patofizyolojisinde ise görsel korteks (N75'in kaynağı) ile bağlantılı duyusal yolakların regülasyonu yer alıyor olabilir. Bu bulgular, migrenle ilişkili fotofobi için bir biyobelirteç olarak kullanılması açısından umut vericidir.

OBJECTIVE: Migraine is a primary headache disorder characterized by episodes of headache attacks (ictal) and periods between attacks (interictal). Ictal and interictal photophobia is a common migraine-associated symptom and can negatively impact patients' quality of life. It is thought that electrophysiological responses may change in migraine patients accompanied by photophobia. The components of the pattern stimulated visual evoked potential (VEP) responses are as follows; the first negative component (N75) is generated in the striate cortex, while the initial positive component (P100) is generated in the dorsal extrastriate cortex. Cortical responses obtained in VEP studies can vary depending on variable threshold retino-thalamo-cortical sensory networks. The objective of this study was to establish a biomarker for assessing the severity of migraine-associated photophobia when exposed to visual evoked potential (VEP) at different levels of luminance. METODS: This study included a total of 44 patients, consisting of 24 patients with episodic migraine (EM) and 20 patients with chronic migraine (CM), along with 20 healthy controls as a control group (CG). The patients completed the 12-item Utah Photophobia Symptom Scale (UPSIS-12), Photophobia and Allodynia 4-point Likert scales, Allodynia Symptom Checklist (ASC-12), Migraine Disability Assessment Scale (MIDAS), Headache Impact Test-6 (HIT-6), Visual Analog Scale (VAS). The patient's photophobia was graded as severe (UPSIS-12 ≥30) and mild (UPSIS-12<30). The checkerboard stimulus consisted of two variants with distinct white square luminance values: low luminance (LL:45 cd·m-2) and high luminance (HL:85 cd·m-2). VEP was recorded during a interictal period in EM patients. Due to rarity of the interictal period in patients with CM, VEP were recorded during times when they were headache-free or their headaches were not severe in intensity. RESULTS: In migraine participants with severe photophobia scores, the P100 latency was found to be significantly shorter compared to the mild photophobia group, regardless of luminance values (p=0.013 for LL; p=0.033 for HL). In migraine participants with severe photophobia scores, the difference in N75 latency measurements obtained in LL and HL recordings was significantly increased compared to the mild photophobia group (p=0.011). In the CM group, the difference in N75 latency measurements obtained in LL and HL recordings was significantly increased compared to the CG and EM groups (p=0.001). In the linear regression analysis model in which the variables affecting the N75 latency measurement difference in LL and HL were examined, it was found the UPSIS-12 score and being in the CM group had a significant effect (p=0.006 and p=0.044, respectively). CONCLUSION: Our findings indicate that VEP responses at variable luminance values vary depending on the severity of migraine-related photophobia and being in the chronic migraine group. The modulation of retino-thalamo-cortical sensory networks related to the dorsal extrastriate cortex (the source of P100) and the visual cortex (the source of N75) may play a role in the neurobiology of migraine-associated photophobia. In the pathophysiology of chronic migraine, regulation of sensory pathways associated with the visual cortex (the source of N75) may be involved. These findings are promising potential of using changes in VEP responses at different luminance levels as a biomarker for migraine-associated photophobia.
