Özet

Bu prospektif randomize çalışma, 65 yaş ve üzeri hastalarda spinal cerrahi sırasında uygulanan farklı anestezi derinlik izleme yöntemlerinin (hemodinamik, numerik BIS ve Density Spectral Array [DSA]) postoperatif kognitif fonksiyonlar ve derlenme parametrelerine etkilerini karşılaştırmayı amaçlamaktadır. Çalışmaya genel anestezi altında elektif spinal cerrahi planlanan 75 hasta (her grupta 25) dahil edilmiştir. Hastalar rastgele şekilde üç gruba ayrılmıştır: Hemodinamik parametrelerle yönlendirilen grup (Grup Hemodinami), yalnızca sayısal BIS indeksi görülebilen grup (Grup nBIS) ve yalnızca DSA ekranı görülebilen grup (Grup DSA). Anestezi, propofol ve remifentanil ile hedef kontrollü infüzyon yöntemine göre uygulanmış, derlenme süreci, bilişsel değerlendirmeler ve deliryum taramaları grup bilgisinden habersiz anestezistler tarafından yapılmıştır. Postoperatif dönemde, DSA grubundaki hastaların sözel uyarana yanıt verme, koopere olma ve oryantasyon süreleri anlamlı derecede kısa bulunmuştur (p<0.001). Ayrıca DSA grubunda postanestezik bakım ünitesinde (PABÜ) kalış süresi daha kısa ve RASS ≤-2 skoruna sahip olma olasılığı daha düşüktür (p<0.001). Zippering fenomeni görülmeyen hastalarda PABÜ'de resedasyon oranı anlamlı düzeyde yüksek bulunmuş, zippering ile postoperatif deliryum (POD) arasında negatif korelasyon saptanmıştır (p<0.001). Burst supresyon insidansı genel olarak %60 olup, DSA grubunda bu oran anlamlı şekilde daha düşüktür (p=0.042). Ayrıca, burst supresyon süreleri ve oranları da DSA grubunda anlamlı şekilde daha azdır. Numerik BIS değerlerinin 40-50 aralığında geçirilmiş süre, nBIS grubunda daha fazlayken, 50-60 aralığı DSA grubunda baskındır (p<0.001). DSA grubunda hastalar bazal EEG kategorilerinin bir üst düzeyine daha sık çıkarken, daha az sıklıkla alt kategorilere inmektedir (p<0.001). İlaç tüketimi açısından DSA grubunda vücut ağırlığına göre düzeltilmiş propofol infüzyon miktarı diğer gruplara kıyasla anlamlı olarak daha düşüktür (p<0.01). Norepinefrin tüketimi de nBIS grubunda daha fazladır (p=0.027). Lojistik regresyon analizine göre, DSA grubunda burst supresyon gelişme riski anlamlı şekilde daha düşüktür (OR=0.31, p=0.050). POD insidansında azalma eğilimi olsa da bu fark anlamlı bulunamamştır (p=0.082). Ayrıca, burst supresyon gelişme riskinin en güçlü belirleyicileri toplam komorbidite sayısı (OR=1.75, p=0.020) ve DSA bazal kategorisidir (OR=0.13, p=0.005). ROC analizinde komorbidite sayısının burst supresyonu öngörme gücü orta düzeyde bulunmuştur (AUC=0.690, p=0.005). Sonuç olarak, DSA ile anestezi derinliği izlemi, daha hızlı derlenme, daha düşük burst supresyon insidansı ve ilaç tüketimi ile ilişkilidir. Ayrıca zippering fenomeni, POD gelişimi açısından koruyucu bir gösterge olarak öne çıkmaktadır.

This prospective randomized study aims to compare the effects of different anesthesia depth monitoring methods (hemodynamic-guided, numerical BIS, and Density Spectral Array [DSA]) on postoperative cognitive function and recovery parameters in patients aged 65 years and older undergoing spinal surgery. A total of 75 patients scheduled for elective spinal surgery under general anesthesia were enrolled, with 25 patients assigned to each group. Participants were randomly allocated into three groups: the Hemodynamic group (guided by hemodynamic parameters), the nBIS group (guided by numerical BIS index), and the DSA group (guided by the DSA display only). Anesthesia was administered using target-controlled infusions of propofol and remifentanil. Recovery assessments, cognitive evaluations, and delirium screenings were conducted by blinded anesthesiologists. In the postoperative period, patients in the DSA group demonstrated significantly shorter times to eye opening, command following, and orientation (p<0.001). Additionally, their length of stay in the post-anesthesia care unit (PACU) was shorter, and the incidence of RASS ≤–2 was lower compared to the other groups (p<0.001). Patients without the zippering phenomenon had significantly higher rates of re-sedation in the PACU, and a negative correlation was found between the presence of zippering and postoperative delirium (POD) (p<0.001). The overall burst suppression incidence was 60%, with a significantly lower rate in the DSA group (p=0.042). Both the duration and proportion of burst suppression were also significantly reduced in this group. Time spent with BIS values between 40–50 was highest in the nBIS group, while the 50–60 range was dominant in the DSA group (p<0.001). Patients in the DSA group more frequently shifted to a higher EEG baseline category and less frequently to a lower one (p<0.001). In terms of drug consumption, weight-adjusted propofol infusion rates were significantly lower in the DSA group (p<0.01). Norepinephrine usage was significantly higher in the nBIS group (p=0.027). According to logistic regression analysis, the risk of developing burst suppression was significantly lower in the DSA group (OR=0.31, p=0.050). Although the POD incidence showed a decreasing trend in the DSA group, the difference was not statistically significant (p=0.082). The strongest predictors for burst suppression were the total number of comorbidities (OR=1.75, p=0.020) and the DSA baseline category (OR=0.13, p=0.005). ROC analysis revealed a moderate predictive power of comorbidity count for burst suppression risk (AUC=0.690, p=0.005). In conclusion, DSA-guided anesthesia depth monitoring is associated with faster recovery, lower burst suppression incidence, and reduced anesthetic consumption. Furthermore, the presence of the zippering phenomenon appears to be a protective factor against the development of postoperative delirium.
