Özet
Amaç: Otolog hamstring tendon grefti kullanılarak artroskopik ön çapraz bağ rekonstruksiyonu yapılan 30 hastanın sonuçları değerlendirilmeyi amaçladık. Gereç ve Yöntem: Ankara Atatürk Eğitim ve Araştırma Hastanesi 3. Ortopedi Kliniğinde Kasım 2007 - Mayıs 2009 tarihleri arasında artroskopik olarak ön çapraz bağ tamiri yapılan 30 hastanın tamamı değerlendirmeye alınmıştır. Takip süresi ortalama 19,3±5,4 (12-31) aydır ve bu hastaların değerlendirilmesinde stabilite testleri, subjektif kriterler içeren değerlendirme testleri, kullanılan turnikenin graft boyuna etkisi incelenmiştir. Bulgular: Çalışmaya alınan 22-46 yaş aralığındaki 30 hastanın yaş ortalaması 30,8±6,5 dur. Hastaların 4'ü (%13,3) kadın, 26'sı (%86,6) erkek iken 6 hastanın (%20) sol dizi, 24 hastanın (%80) sağ dizi opere edilmiştir Cerrahiden önceki Lachman testi, Ön çekmece testi ve Pivot-shift testi tüm hastalarda cerrahi sonrasında anlamlı ölçüde artmıştır. Hastaların Lysholm skoru ortalama 50,9±18,1'den 84,2±10,7'ye (p=0,004), Tegner skoru ortalama 2,1±0,8'den 4,4±1,1'e (p=0,001), Cincinnati skoru ortalama 247,3±31,7'den 355,6±38,3'e (p<0,001) yükselmiştir. Çalşmaya alınan 30 hastanın 7'sine (%23,3) pnömotik turnike, 23'üne (%76,6) steril sitokinetli çorap turnike kullanılmıştır. Çorap turnike kullanılan hastaların ortalama tendon boyu 22,3±1,63cm, pnömotik turnike kullanılan hastaların ortalama tendon boyu 20,6±0,7 cm (19-21,5) olarak bulunmuştur. Cerrahi işlemde çorap turnike kullanılan hastaların ortalama cerrahi süresi 83,6±23,5dk., pnömotik turnike kullanılan hastaların cerrahi süresi 97,8±20,9 dk. olarak bulunmuştur. Sonuç: Gelişen femoral tespit materyalleri sayesinde hamstring tendonu ile yapılan ön çapraz bağ rekonstruksiyonu diğer yöntemlere göre daha avantajlıdır. Cerrahi işlem sırasında steril sitokinetli çorap turnike kullanımı greft boyunu anlamlı düzeyde artırmakta (p=0,0025), fakat cerrahi süreyi anlamlı düzeyde kısaltmamaktadır (p=0,642).

Objective: We aim to evaluate 30 patient's results of artroscopic anterior cruciate ligament reconstruction which we used otolog hamstring greft. Material and methods: We evaluate all of the 30 patients whom made reconstruction of anterior cruciate ligament with artoscopic surgery between november 2007 and may 2009 in Ankara Atatürk Education and Research Hospital 3. orthopedy clinic. The mean following period is 19,3 ± 5,4 (12-31) months and the assesment of these patients we analyzed stability tests, the evaluation tests which include subjective criteria, the effect of employed tourniquet to graft lenght. Evidence: The 30 patients who in our study are between 22-46 years old mean 30,8±6,5. The 4 of them (%13,3) were female, 26 of them (%86,6) were male and we have operated 6 patients (%20) from left knee, 24 patients (%80) form right knee. The Lachmann test, front drawer test and Pivot-shift test were increase after surgery. The Lyshom score of patients were increase from mean 50,9±18,1 to 84,2±10,7 (p=0,004) The Tegner score rise from 2,1±0,8 to 4,4±1,1 (p꞊0,004), the Cincinnati score rise from 247,3±31,7 to 355,6±38,3 (p<0,001) in our study. The 7 of 30 patient's of our study (%23,3) had used pneumotic tourniquet, 23 of them (%76,6) had used sterile citochineted socks tourniquet. The mean tendon lenght of the patients who used socks tourniquet were found 22,3±1,63 cm and who used pneumotic tourniquet were found 20,6±0,7 cm (19-21,5) The mean surgery time in surgery practice which we used socks tourniquet were found 83,6±23,5 minute, which we used pneumotic tourniquet were found 97,8±20,9 minute. Conclusion: By means of developed femoral anchoring materials anterior cruciate ligament reconstriction which has making with hemstring tendon is beter than teh other methods. During surgery procedure using of sterile citochineted socks increase the greft lenght (p꞊0,0025) but it doesn't shorten the surgery time (p꞊0,642).
