Özet
Giriş ve Amaç: Bu çalışmada transsfenoidal hipofiz cerrahisi geçirecek hastalarda deksmetedomidin ile opioid içermeyen anestezi uygulamasının opioid içeren anestezi uygulamasına göre postoperatif kognitif fonksiyonlar üzerine etkisinin karşılaştırılması amaçlanmıştır. Gereç ve Yöntem:Çalışma prospektif, randomize kontrollü olarak iki grup şeklinde planlandı. 18-65 yaş aralığındaki ASA I, II ve III olan, okuma yazma bilen hastalar çalışmaya dahil edildi. Hastalara preoperatif dönemde standardize mini mental test (SMMT) uygulandı ve sonuçları kaydedildi. İndüksiyonda propofol ve remifentanil / deksmedetomidin uygulandı. Anestezi idamesinde propofol ve remifentanil kullanılan hastalar Grup R, propofol ve deksmedetomidin kullanılan hastalar Grup D olarak sınıflandırıldı. BİS değerlerinin 40-60 aralığında tutulması hedeflendi. İntraoperatif SKB, DKB, KAH ve BİS değerleri; anestezi indüksiyonundan önce, entübe edildikten sonra ve takip eden 15. dk, 30. dkve 1. saatte kayıt altına alındı. Hastaların toplam anestezi, ekstübasyon ve göz açma süreleri kaydedildi. Postoperatif 24. saat ve 3. haftada SMMT tekrarlandı ve sonuçları kaydedildi. Bulgular:Hastaların demografik verileri istatistiksel olarak benzerdi. Postoperatif SMMT skorları arasında istatistiksel olarak fark gözlenmedi (p>0,05). Grup içi SMMT skorlarının karşılaştırılmasında her iki grupta da postoperatif 3. haftada, preoperatif döneme kıyasla istatistiksel olarak anlamlı birartış gözlendi (p<0,05) Ortalama anestezi süresi açısından gruplar arasında istatistiksel olarak farklılık gözlenmedi (p>0.05). Ekstübasyon ve göz açma süresi, Grup D'de Grup R'ye göre anlamlı olarak daha uzun bulundu (p<0,001).Komplikasyon gelişimi açısından gruplar karşılaştırıldığında, istatistiksel olarak anlamlı bir fark gözlemlenmedi (p>0,05) KAH ve OAB ölçümleri istatistiksel olarak benzerdi (p>0,05) BİS ölçümleri açısından yalnızca entübasyonu takiben 15. dakikada gruplar arasında istatistiksel olarak anlamlı bir fark vardı (p<0,05). Sonuç: Bu çalışmanın bulguları transsfenoidal hipofiz cerrahisinde deksmedetomidin kullanılarak opioid içermeyen anestezi uygulamasının remifentanil ile opioid içeren anesteziye göre postoperatif kognitif fonksiyonlar üzerindeki etkilerinin benzer olduğunu göstermiştir.

Introduction:This study aims to compare the effects of opioid-free anesthesia using dexmedetomidine versus opioid-based anesthesia on postoperative cognitive functions in patients undergoing transsphenoidal pituitary surgery. MaterialsandMethods:The study was designed as a prospective, randomized controlled trialwith two groups. Patients aged between 18 and 65 years, classified as ASA I, II, or III, and who were literate were included in the study. All patients underwent a standardized Mini-Mental State Examination (MMSE) in the preoperative period, and the results were recorded. During induction, propofol was administered in combination with either remifentanil or dexmedetomidine. Patients who received propofol and remifentanil for anesthesia maintenance were a ssigned to Group R, while those who received propofol and dexmedetomidine were a ssigned to Group D. The BIS values were targeted to be maintained within the range of 40–60. Intraoperative measurements of systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP), and BIS values were recorded before anesthesia induction, after endotracheal intubation, and at the 15th minute, 30th minute, and 1st hour following intubation. Total anesthesia duration, extubation time, and eye-opening time were also recorded.The MMSE was repeated at postoperative 24 hours and at the 3rd week, and the results were documented. Results:The demographic characteristics of the patients were statistically comparable between groups. No statistically significant difference was observed in postoperative MMSE scores between the groups (p>0.05). In the within-group comparison of MMSE scores, a statistically significant increase was observed in both groups at postoperativeweek 3 compared to the preoperative period (p<0.05). There was no statistically significant difference between the groups in terms of meanduration of anesthesia (p>0.05).Extubation and eye-opening times were significantly longer in Group D compared to Group R (p<0.001). When complication rates were compared between the groups, no statistically significantdifference was observed (p>0.05).Mean arterial pressure (MAP) and heart rate (HR) measurements were statistically similar between the groups (p>0.05).Regarding BIS values, a statistically significant difference was observed only at the 15th minute following intubation (p<0.05). Conclusion:The findings of this study demonstrated that opioid-free anesthesia using dexmedetomidine had similar effects on postoperative cognitive functions compared to opioid-based anesthesia with remifentanil in transsphenoidal pituitary surgery.
