Hyponatremia is the most common electrolyte disorder, seen in 3-6% of emergency department visits. There are few emergency department studies on this subject, and existing studies have focused on epidemiological data and risk factors. This study is both a research aiming to contribute to the literature by identifying the descriptive characteristics of patients presenting to the emergency department with moderate or severe hyponatremia, and a bold challenge to the 48-hour criterion used in differentiating hyponatremia symptoms according to their onset time. The study included 745 patients who presented to the emergency department between 01/01/2022 and 31/12/2023 with serum sodium levels <130 mEq/L. Based on the onset time of hyponatremia, presentations were divided into 3 groups: <48 hours (acute), 48 hours-1 month (subacute), and >1 month (chronic) hyponatremia. Chi-square test was used to compare categorical variables, Kruskal-Wallis test for multiple groups, and Mann-Whitney U test for paired groups. According to serum sodium levels, severe hyponatremia was detected in 35.7% of presentations, and moderate hyponatremia in 64.3%. The median age was 69. 68.7% of all patients and 76.3% of symptomatic patients were ≥60 years old. Hyponatremia was 6.8% more frequent and more severe in women than man. Hypovolemia was the most common volume status (63.6%). Symptoms were observed in 19.9% of patients, with no symptoms detected in the patient group aged <30 years. All symptoms improved with treatment. No deaths due to hyponatremia were detected, while the mortality rate from other diseases was 9.9%. Acute hyponatremia was detected in 13.2% of cases, subacute in 38.5%, and chronic in 48.3%, with median serum sodium values of 121, 125, and 128 mEq/L, respectively, in the order listed. No relationship was found between time of onset of hyponatremia and age, recovery time, or mortality. In the subacute group, compared to the acute group; presentations were higher, the number of symptomatic patients was equal, and the number of patients with severe symptoms and those receiving 3% hypertonic saline treatment was similar. In subacute hyponatremia, the onset time of symptoms, except for headache and muscle cramps, was >48 hours and ≤1 week. Although many experts agree that protective mechanisms for preventing brain edema occur within 48 hours, it is difficult to determine a period for acute hyponatremia. The results of this study suggest that completion of adaptive mechanisms may take longer than 48 hours, and especially the period between 48 hours-1 week shoulds to be evaluated differently from chronic hyponatremia.
