Özet

Bu çalışmanın amacı, spastik serebral palsili (SP) bireylerde eksentrik egzersiz (EE) eğitiminin spastisite, yürüme ve denge üzerindeki etkilerini incelemektir. Çalışma bir fizyoterapi ve rehabilitasyon merkezinde, 6-18 yaş arasındaki spastik unilateral ve bilateral SP tanısı almış bireyler ile randomize kontrollü tek kör çalışma olarak gerçekleştirilmiştir. Kontrol grubuna sadece NGT (Nörogelişimsel Terapi) uygulanırken deneysel gruba haftada iki gün NGT+EE eğitimi uygulanmıştır. Çalışmaya her bir grupta 21 olmak üzere toplam 42 birey dahil edilmiştir. Her iki grup da tedavi öncesi ve sonrası değerlendirilmiştir. Kaba motor fonksiyon Kaba Motor Fonksiyon Ölçümü D ve E başlıkları (KMFÖ) ile; kas tonusu Modifiye Ashworth Skalası (MAS), spastisite Modifiye Tardieu Skalası (MTS) ile; eklem hareket açıklığı (EHA) evrensel elektrogonyometre ile; yürüme parametreleri Edinburgh Görsel Yürüme Analizi (EGYA) ile; denge Pediatrik Denge Ölçeği (PDÖ) ve stabilometre ile; fonksiyonel bağımsızlık ise Pediatrik Fonksiyonel Bağımsızlık Ölçütü (WeeFIM) ile ve selektif motor kontrol Boyd ve Graham'ın Selektif Motor Kontrol (SMK) testi ile değerlendirildi. Çalışma sonucunda NGT+EE eğitiminin NGT'ye göre kaba motor fonksiyonları geliştirmede (KMFÖ D ve D+E) (p<0,05); kas tonusunu azaltmada (kalça adduktor, diz ekstansor ve fleksor kaslarında) (p<0,05); EHA'yı arttırmada (p<0,05); yürüme paremetrelerini iyileştirmede (p<0,05) ve dengeyi geliştirmede (stabilometre parametreleri) (p<0,05) daha etkili olduğu bulundu. Sonuçlarımız, EE eğitiminin spastik SP'li bireylerde etkili bir tedavi seçeneği olduğunu ortaya koymaktadır. EE eğitimi spastik SP'li çocukların pediatrik fizyoterapi programına dahil edilmelidir.

The aim of this study is to investigate the effects of eccentric exercise (EE) training on spasticity, walking, and balance in individuals with spastic cerebral palsy (CP). The study was conducted as a randomized controlled single-blind trial at a physiotherapy and rehabilitation center with individuals diagnosed with spastic unilateral and bilateral CP, aged between 6 and 18 years. The experimental group received NDT+EE training twice a week, while the control group only received NDT. A total of 42 participants, 21 in each group, were included in the study. Both groups were assessed before and after the intervention. Gross motor function was evaluated using the Gross Motor Function Measure D and E (GMFM); muscle tone was measured using the Modified Ashworth Scale (MAS); spasticity was assessed with the Modified Tardieu Scale (MTS); joint range of motion (ROM) was measured with a universal electrogoniometer; walking parameters were assessed using the Edinburgh Visual Gait Analysis (EVGA); balance was evaluated using the Pediatric Balance Scale (PBS) and a stabilometer; functional independence was measured using the Pediatric Functional Independence Measure (WeeFIM); and selective motor control was assessed with Boyd and Graham's Selective Motor Control (SMC) test. The results showed that NDT+EE training was more effective than NDT alone in improving gross motor function (GMFM D and D+E) (p<0.05), reducing muscle tone (in hip adductors, knee extensors, and flexors) (p<0.05), increasing ROM (p<0.05), improving walking parameters (p<0.05), and enhancing balance (stabilometer parameters) (p<0.05). Our findings suggest that EE training is an effective treatment option for individuals with spastic CP. EE training should be incorporated into the pediatric physiotherapy program for children with spastic CP.
