Sıçanda oluşturulan periferik nöropati modelinde brilliant Blue G'nin nöroinflamatuar süreçler ve nöropatik ağrı üzerine etkilerinin incelenmesi

Özet

Amaç: Pro-inflamatuar sitokinlerin santral ve periferik sensitizasyon mekanizmalarıyla nöropatik ağrı gelişiminde rol oynadığı gösterilmektedir. Brilliant Blue G (BBG), doku hasarı sonrası ATP'nin ekstrasellüler alanda yüksek konsantrasyona ulaşmasıyla aktifleşen ve pro-inflamatuar sitokinlerin salınımına sebep olan P2X7 reseptörünün non-kompetitif antagonistidir. Bu çalışma, sıçanda oluşturulan travmatik periferik nöropati sonrası BBG uygulanmasının, nöropatik ağrının erken dönemdeki duyusal ve geç dönemdeki duygusal boyutuna yönelik etkinliğini değerlendirmeyi amaçlamıştır. Gereç ve Yöntem: Çalışmada toplam 48 adet erkek Sprague-Dawley sıçan kullanıldı. Nöropatik ağrı modeli, safen sinire parsiyel ligasyon (SaPL) uygulanarak oluşturuldu. BBG, sinirin ligasyonundan sonra 1 hafta süreyle 50 mg/kg dozda intraperitoneal yolla uygulandı. Sham, SaPL, SaPL + SF ve SPL + BBG şeklinde 4 grup (n=12/grup) oluşturuldu. Mekanik ağrı eşiği, Dinamik Plantar Esteziyometre ile değerlendirildi. Termal ağrı eşiğini değerlendirmek üzere Artan Sıcaklıklı Hot Plate testi kullanıldı. Anksiyete benzeri davranışların incelenmesi için Açık Alan ve Yükseltilmiş Artı Labirent testleri yapıldı. Depresyon benzeri davranışlar, Modifiye Zorlu Yüzme Testi ile değerlendirildi. Serum ve beyin dokularındaki IL-1β ve IL-6 düzeyleri, ELISA yöntemiyle ölçüldü. Bulgular: Safen sinire parsiyel ligasyonun uygulanmasından sonra, 3. günden itibaren mekanik ağrı eşiğinde düşme tespit edildi (p<0,05). Safen sinirin parsiyel ligasyonundan sonra 1 hafta süreyle BBG uygulanmasının, mekanik ağrı eşiğindeki düşmeyi post-op 1. haftadan itibaren iyileştirdiği görüldü (p<0,05). BBG uygulanmasının termal ağrı eşiğini yükselttiği bulundu (p<0,05). Anksiyete benzeri davranışlar değerlendirildiğinde, gruplar arasında anlamlı fark görülmedi. Safen sinirin parsiyel ligasyonunun, depresyon benzeri davranışlara sebep olduğu tespit edildi (p<0,05). Ligasyon sonrası BBG uygulanmasının, depresyon benzeri davranışların gelişmesini önlediği gösterildi. Safen sinirin parsiyel ligasyonu sonrası, prefrontal korteks ve amigdalada IL-1β ve IL-6 düzeylerinin yükseldiği tespit edildi (p<0,05). BBG uygulanmasının, prefrontal kortekste IL-1β ve IL-6 düzeylerinin yükselmesini önlediği bulundu. Sonuç: BBG'nin nöropatik ağrı hayvan modelinde analjezik ve antidepresan etkinliği olduğu bulunmuştur. Nöropatik ağrı hayvan modellerinin, ağrıyı hem duyusal hem de duygusal yönleriyle değerlendirmesi, daha etkin tedavi modalitelerinin geliştirilebilmesine olanak sağlayabilir.

Aim: A growing literature suggests that pro-inflammatory cytokines play an integral role in the development of neuropathic pain via central and peripheral sensitization mechanisms. Brilliant Blue G (BBG) is a non-competitive antagonist of the P2X7 receptor, which is activated by the high concentrations of ATP in the extracellular space after tissue damage and causes the release of pro-inflammatory cytokines. This study aimed to evaluate the effectiveness of BBG administration on the early sensory and late affective dimensions of neuropathic pain after traumatic peripheral neuropathy in rats. Materials and Methods: A total of 48 adult male Sprague-Dawley rats were used in the study. Neuropathic pain was induced by the saphenous nerve partial ligation (SaPL) model. BBG was administered intraperitoneally at a dose of 50 mg/kg for one week after the nerve was ligated. Rats were randomly assigned to one of the following four groups (n=12/group): Sham, SaPL, SaPL + Saline, SPL + BBG. Mechanical pain threshold was assessed with the Dynamic Plantar Aesthesiometer. Increasing Temperature Hot Plate test was used to evaluate thermal pain threshold. Anxiety-like behaviors were assessed with the Open Field and Elevated Plus Maze tests. Modified Forced Swim Test was used to assess depression-like behaviors. IL-1β and IL-6 levels in serum and brain tissues were measured by ELISA. Results: After partial ligation of the saphenous nerve, a decrease in mechanical pain threshold was detected starting from the postoperative 3rd day (p<0.05). Administration of BBG for 1 week after partial ligation of the saphenous nerve alleviated the decrease in mechanical pain threshold starting from the 1st post-operative week (p<0.05). 7-day treatment of BBG was found to increase thermal pain threshold (p<0.05). When anxiety-like behaviors were evaluated, no significant difference was found between the groups. The partial ligation of the saphenous nerve induced depression-like behaviors (p<0.05). Administration of BBG after the ligation prevented the development of depression-like behaviors. After partial ligation of the saphenous nerve, IL-1β and IL-6 levels were increased in the prefrontal cortex and amygdala (p<0.05). Administration of BBG after the ligation inhibited the elevation in IL-1β and IL-6 levels in the prefrontal cortex. Conclusion: BBG was found to have analgesic and antidepressant effects in an animal model of neuropathic pain. Assessing both sensory and emotional aspects of neuropathic pain in animal models could lead to the development of more effective treatment modalities.
