Özet

Amaç: İleri evre endometrium kanseri (EK) nedeniyle opere edilen olgularda maksimal, optimal ve suboptimal sitoredüktif cerrahinin sağkalım avantajlarının karşılaştırılması amaçlanmıştır. Yöntem: Çalışmaya 1 Ocak 2000- 31 Aralık 2020 tarihleri arasında Sağlık Bilimleri Üniversitesi Ankara Etlik Zübeyde Hanım Kadın Hastalıkları Eğitim ve Araştırma Hastanesi'nde ileri evre (Evre 3B-4) endometrium kanseri (EK) tanısı alan, cerrahi ve postoperatif bakımı kurumumuzda sağlanan ve verilerine ulaşılabilen hastalar dahil edilmiştir. Hastaların tanı yaşları, vücut kitle indeksleri, tanı tarihi, cerrahi prosedür, nihai histopatolojileri, patolojik dereceleri (grade), myometriyal invazyon derecesi, tümör boyutu, total pelvik-paraaortik lenf nodu sayıları, FIGO evreleri, adjuvan tedavi durumu, rekürrens durumu, varsa rekürrens tarihi, ölüm durumu, ölüm gerçekleştiyse ölüm tarihi ve ilişkili bilgiler kaydedilmiştir. Çalışmamızdaki karşılaştırmalarda hem FIGO 2009 evrelemesi sistemi kullanılmış, hem de güncellenen FIGO 2023 evreleme sistemi de kullanılarak hastaların eski ve yeni evreleri belirtilmiştir. Hastalıksız sağkalım, rekürrens gelişmeyenlerde tanı tarihi ile son kontrol tarihi arasındaki süre (ay), rekürrens gelişenlerde tanı tarihi ile rekürrens tarihi arasındaki süre (ay) olarak hesaplanmıştır. Genel sağkalım, tanı tarihi ile hastanın ölüm tarihi, ölüm yoksa son kontrol tarihi dikkate alınarak hesaplanmıştır. Bulgular: Çalışmada 1 Ocak 2000 ile 31 Aralık 2020 tarihleri arasında hastanemizde ileri evre (Evre 3-4) EK nedeniyle sitoredüksiyon cerrahisi (CRS) uygulanan 161 hasta retrospektif olarak değerlendirilmiştir. Bu hastalardan 10 tanesi sarkom tanısı nedeniyle çalışma dışı tutulmuştur. Çalışmaya dahil edilen 151 hasta maksimal, optimal ve suboptimal CRS yapılma durumuna göre 3 gruba ayrılmıştır. Hastalarımızın %58,9'u 60 yaş altı, %41,1'i 60 yaş üstüydü. Hastalarımız FIGO 2023 evreleme sistemine göre en sık görülen evre %39,1 oranla Evre 3C2 en sık histolojik tip ise %60,9 oranla Endometrioid tip olmuştur. Hastalarımızın %57'sine maksimal, %35,8'ine optimal ve %7,3'üne suboptimal CRS uygulanmıştır. Nihai patolojilerine göre en sık gözlemlenen histolojik Grade %47,7 oranla Grade 3 olup, bu hastalarında %45,8'ine maksimal %45,8'ine optimal ve %8,4'üne suboptimal CRS uygulanmıştır. Grade 1 hastaların %75,9'una maksimal, %24,1'ine optimal CRS, uygulanmış olup bu grupta suboptimal CRS izlenmemiştir. Grade 2 hastalar ise %68,6'sına maksimal, %28,6'sına optimal ve %2,8'ine suboptimal CRS uygulanmıştır. Sağkalım analizlerinde maksimal grubunda ortalama sağkalım süresi 191,9 ay, optimal grubunda 85,6 ay, suboptimal grubunda 12 ay olarak gözlemlenmiştir. Endometrioid tip EK'da maksimal CRS uygulanan hastaların sağkalım süreleri 207,9 ay, optimal cerrahi uygulanan hastaların 134,7 ay, suboptimal CRS uygulanan hastaların sağkalım süreleri 10,8 ay olarak gözlemlenmiştir. Grade 1-2 grupta maksimal CRS sağkalım süresi ortalama 221,1 ay, optimal CRS ortalama sağkalım 134,2 ay iken suboptimal grupta 7 ay olarak saptanmıştır. Grade 3 grupta Maksimal CRS yapılan grupta genel sağkalım 135,3 ay iken, optimal grupta 43,7 ay suboptimal grupta ise 11,5 ay olarak saptanmıştır. Evre 3 hastalık varlığında genel sağkalım ortalama 158,4 ay, Evre 4 varlığında ise 105,1 ay olarak saptanmıştır. Evre 3 hastalık grubunda maksimal grupta ortalama sağkalım 189,7 ay, optimal grupta ortalama genel sağkalım 96,7 ay, suboptimal grupta ise 11 ay olarak izlenmiştir. Evre 4 hastalıkta maksimal grupta ortalama sağkalım 161,6 ay, optimal grupta ortalama sağkalım 73,7 ay iken suboptimal grupta 12,5 ay olarak saptanmıştır. Sonuç: Bu çalışma, ileri evre endometrium kanserinde sitoredüktif cerrahinin hastaların sağkalımı üzerindeki hayati önemini güçlü bir şekilde ortaya koymaktadır. Özellikle maksimal sitoredüksiyonun sağkalım avantajını belirgin bir şekilde artırdığı, optimal sitoredüksiyonun ise daha sınırlı bir fayda oranı saptadığı tespit edilmiştir. Suboptimal cerrahi ise sağkalım süresini ciddi bir şekilde olumsuz etkilemiştir. Bu bulgular göstermiştir ki; tecrübeli ellerde yapılacak etkin bir cerrahi tedavi, metastatik endometrium kanserinde tedavinin en kritik basamağıdır.

Objective: The aim of this study was to determine and compare the potential survival benefits of maximal, optimal, and suboptimal cytoreductive surgery in patients undergoing surgical treatment for advanced-stage endometrial cancer (EC) in gynaecological oncology practice. Methods: The study included all patients diagnosed with advanced-stage endometrial cancer (FIGO stages 3B-4) between January 1, 2000, and December 31, 2020, at the University of Health Sciences Ankara Etlik Zübeyde Hanım Women's Health Training and Research Hospital. Data collected from patients' medical records included age at diagnosis, body mass index, date of diagnosis, surgical procedures performed, final histopathological findings, tumour grade, depth of myometrial invasion, tumour size, lymphovascular space invasion, metastatic and total pelvic-paraaortic lymph node counts, FIGO stage, adjuvant treatment status, recurrence status (and date, if applicable), survival status, date of death (if applicable), and related information. Both the FIGO 2009 and updated FIGO 2023 staging systems were used, and patients' stages were reported according to both systems. Progression-free survival was defined as the time (in months) from diagnosis to the last follow-up date or the date of recurrence, and overall survival was defined as the time (in months) from diagnosis to the date of death or the last follow-up date. Results: A total of 161 patients who underwent cytoreductive surgery for advanced-stage (stage 3-4) endometrial cancer between January 1, 2000, and December 31, 2020, were reviewed. Of these, 10 patients with a diagnosis of sarcoma were excluded, leaving 151 patients with stage 3-4 endometrial cancer. These patients were divided into three groups based on the type of cytoreductive surgery (CRS) performed: maximal, optimal, and suboptimal. Among the patients, 58.9% were under 60 years of age, and 41.1% were 60 years or older. According to FIGO 2023 staging system, the most common stage was Stage 3C2 with 39.1% and the most common histological type was Endometrioid type with 60.9%. Stage 4A was not observed in any patients. Maximal CRS was performed in 57%, optimal CRS in 35.8%, and suboptimal CRS in 7.3% of the patients. Based on final pathology reports, grade 3 was the most frequently observed tumor grade (47.7%); among these patients, 45.8% underwent maximal CRS, 45.8% optimal CRS and 8.4% suboptimal CRS. In Grade 1 patients, maximal CRS was applied to 75.9%, optimal CRS to 24.1% and suboptimal CRS was not observed in this group. In Grade 2 patients, 68.6% received maximal, 28.6% optimal and 2.8% suboptimal CRS. Survival analysis showed that the mean overall survival was 191.9 months in the maximal group, 85.6 months in the optimal group, and 12 months in the suboptimal group. For endometrioid-type EC, the mean survival was 207.9 months for maximal, 134.7 months for optimal, and 10.8 months for suboptimal cytoreduction. The mean survival of grade 1-2 patients was 221.1 months for maximal, 134.2 months for optimal, and 7 months for suboptimal cytoreduction. In grade 3 patients, mean survival was 135.3 months for maximal, 43.7 months for optimal, and 11.5 months for suboptimal cytoreduction. Patients with stage 3 disease had a mean survival of 158.4 months, while those with stage 4 disease had a mean survival of 105.1 months. In stage 3 patients, mean survival was 189.7 months for maximal, 96.7 months for optimal, and 11 months for suboptimal cytoreduction. In stage 4 patients, mean survival was 161.6 months for maximal, 73.7 months for optimal, and 12.5 months for suboptimal cytoreduction. Conclusion: This study powerfully highlights the critical impact of cytoreductive surgery on survival outcomes in advanced-stage endometrial cancer. Maximal cytoreduction significantly improved survival, while optimal cytoreduction provided more limited benefits. Suboptimal cytoreduction had a markedly negative impact on survival. These findings emphasize that effective surgical management, performed by experienced surgeons, is the most crucial component of treatment for metastatic endometrial cancer.
